Assessment and treatment of nutritional status in stroke patients.

Stroke illness imposes a heavy and costly work load on hospital and community care services, and life after stroke can be a miserable affair for stroke patients and their carers. Nutritional factors may have an important role in acute stroke and its outcome. From the limited amount of research undertaken it has been shown that a significant number of patients with stroke are undernourished on admission and their nutritional status deteriorates further as an in-patient. This is especially so for those who are most dependent, in whom undernutrition is also associated with increasing morbidity an

Assessment of nutritional status in cancer patients: widely neglected?

Frank cachexia is usually evident in the clinical evaluation, but important metabolic alterations in tumour-bearing patients with slight weight changes are frequently overlooked. Since several factors are involved in cancer-induced malnutrition (marasmus type descriptive of general inadequate food intake, kwashiorkor type descriptive of inadequate protein intake/increased turnover), medical/dietary history and physical examination (dynamic weight loss, vitamin/mineral deficiency) should be complemented by simple additional laboratory tests. Minimal baseline testing may include: weight loss (>

The relationship between maternal nutritional status and spontaneous abortion.

BACKGROUND: The incidence of pregnancy wastage is high among poor women. Though the aetiology of spontaneous abortion is multifactorial, nutritional deficiency is considered to be an important contributory factor. METHODS: A case-control study was conducted to assess the relationship between maternal vitamin status and spontaneous abortion. RESULTS: The incidence of anaemia (15%), and riboflavin (84%) and folate deficiency (24.5%) were similar among the cases and controls. Vitamin A levels were higher in the study group compared to those in controls. CONCLUSION: The results suggest that var

The nutritional status of 70 year olds in Jerusalem.

Inadequate nutrition is a major problem of elderly people today, a rapidly growing segment of western societies. Yet information on the nutritional state of the elderly in the community is much needed. We surveyed the nutritional status of a cohort of 70 year olds living in western Jerusalem. A total of 605 persons were interviewed in their home regarding socioeconomic variables, education, self-assessed global health status, use of medicines, and activities of daily living (ADL). In the second stage, a subsample of 463 of the above attended an outpatient geriatric clinic where a medical histo

[Effect of postoperative complications on nutritional status: therapeutic consequences].

The occurrence of a postoperative complication represents an additional stress factor for patients and leads in many cases rapidly to a malnutrition status. Thus a nutritional support is required as soon as the foreseeable duration of starvation has a longer duration than one week. Considering its lower risk of septic complications and lower cost, enteral feeding should be initiated as soon as possible. Appraisal of caloric needs with standard formulas often leads to inappropriate nutritional management. Therefore the requirements should be assessed by indirect calorimetry if available. Nutrit

Nutritional status of patients with oral and maxillofacial malignancies.

A nutritional assessment battery consisting of the patient's history, anthropometric measurements, and laboratory tests was used to characterize the nutritional status of 127 patients with oral and maxillofacial malignancies. Forty-three percent of these patients had good nutrition, 21% fair, and 36% poor. The pattern of nutritional impairment was a protein-calorie deficiency. A close correlation with malnutrition was found for diminished oral intake and tumor stage. Gender, tobacco or alcohol consumption, job, and living place were not related to nutritional status.

School performance, nutritional status and trichuriasis in Jamaican schoolchildren.

Severe infections by the geohelminth Trichuris trichiura detrimentally affect young children's growth and development. There is concern that mild to moderate infections may affect older children's school performance and nutritional status. We therefore examined the relationship between varying intensities of infection and school achievement, attendance and nutritional status in 616 schoolchildren. A total of 409 children with Trichuris infection of intensities greater than 1200 eggs per gram of stool (epg) were first identified, then for every 2 infected children in a class, an uninfected chil

The nutritional status of patients receiving home enteral feeding.

19 patients receiving home enteral nutrition (HEN) were nutritionally assessed, using anthropometry and blood analysis. Blood was taken for measurement of serum albumin, magnesium, zinc, copper, selenium and vitamins A and E. 8 patients had anthropometric evidence of protein and energy depletion, of whom 6 were severely depleted. 15 patients had biochemical evidence of trace element or vitamin depletion of whom 6 had multiple (>/= 3) depletion. Depletion occurred in all diagnostic categories, even in patients with normal anthropometric measurements, and was independent of duration, volume or m

Nutritional status and risk of infection among Ethiopian children.

This is a prospective study on the possible association between protein energy malnutrition and risk of infection in a pastoralist and in an agricultural community in southern Ethiopia. A total of 425 children under 5 years of age were observed for 8 months. In general, malnourished children had a significantly higher incidence of diarrhoeal disease, even after controlling for possible confounding social, economic, and environmental effects, as well as for past history of illness. However, the extent of this association between nutritional state and morbidity differed between the two study pop

Nutritional status, suckling behavior, and prolactin release during lactation.

This experiment was conducted to determine whether suckling-induced prolactin (PRL) values in chronically food-restricted lactating rats resulted from the inability of their litters to suckle vigorously. Rats were randomly assigned to be fed ad lib (AL) or to be chronically restricted (CR) to 50% of AL before and during pregnancy and lactation. Rats nursed their own litter (own) or a litter of the opposite dietary treatment group (other) for 90-min periods at early or peak lactation. Blood was collected regularly through a catheter implanted several days earlier. Suckling contributed to differ

[The nutritional status and immunological situation of cancer patients].

A study was performed on the nutritional state of a sample of 138 patients with digestive and extradigestive neoplasias. The study protocol was based on anthropometric, analytical and immunological tests. The main criteria for malnutrition were as follows: loss of 10% or more in normal weight and/or albumen levels of less than 3.5 gr/dl. A global malnutrition percentage of 56.5% was obtained. The highest malnutrition percentages were found among patients with neoplasia of the oesophagus (78.9%) and the lowest in neoplasias of the breast (9.1%). From the nutritional standpoint, neoplasias of th

Nutritional status, tooth eruption, and dental caries: a review.

Animal studies have shown that early malnutrition affects tooth structure, delays tooth eruption, and results in increased dental caries. However, epidemiologic evidence in support of these findings has been elusive. Cross-sectional surveys show that populations with a higher prevalence of caries in their deciduous teeth also show a lower prevalence of caries in their permanent teeth. However, longitudinal data from individuals show exactly the opposite. Caries development is also delayed as a consequence of a delayed tooth eruption and thus the bell-shaped curve that results from plotting dec

Nutritional status and unfavourable pregnancy outcome in Egyptian women.

The nutritional status in 43 cases of repeated spontaneous (habitual and threatened) abortion with both favorable and unfavorable results was assessed by the 24-hour recall method, as compared to 19 normal terminating pregnancies, all of which were in women of low socioeconomic class and who were considered to be malnourished. All pregnant women were monitored at 2 points during gestation, namely 6-12 weeks and 12-16 weeks, by measuring human chorionic gonadotropin as the main hormonal marker in pregnancy and by total and fractional serum protein estimation as a biochemical probe to both the n

Does prolonged breastfeeding adversely affect a child's nutritional status?

In 202 children who visited a children's hospital in the city of Accra, Ghana, breastfeeding beyond the age of 19 months was found to be associated with malnutrition. The effect of weaning on food intake was then studied in 15 breastfed malnourished children in a rural community. Before weaning (complete cessation of breast-feeding) protein and energy intakes of all the malnourished children were about half those of 5 normal children. 10 of the malnourished children were weaned, and their intakes rose to the levels of the normal children; the 5 who continued breastfeeding maintained their low

Tryptophan and nutritional status of patients with senile dementia.

The nutritional status of 23 severely demented patients was compared with that of 23 similarly aged controls in the community. A 3-day weighed intake on all subjects showed lower mean intakes of energy, protein, ascorbic acid and nicotinic acid in the mean intakes of energy, protein, ascorbic acid and nicotinic acid in the patient group. This group had lower levels of plasma ascorbic acid and red cell folate and of urinary N-methylnicotinamide excretion relative to creatinine. Over a third of both controls and patients had evidence of thiamin deficiency, as judged by a raised percentage erythr

[The nutritional status in malabsorption syndrome following gastrointestinal operations].

The nutritional status which includes selected biochemical, anthropometric and immunologic parameters was determined in 36 patients with malabsorption syndromes following gastrointestinal operations. The examinations revealed: Postoperative malabsorption syndromes often lead to considerable nutritional disturbances which cannot be sufficiently estimated by the sole determination of body mass. The nutritional status is a precious tool in judging deficiency symptoms, in instituting a targeted alimentary or substitutive therapy, and in evaluating the therapeutic accomplishment. Optimal long-time a

Characteristics and determinants of child nutritional status in Nepal.

Herein, we examine characteristics and determinants of child malnutrition in the districts of Bara and Rautahat of the Terai region of Nepal. The sample studied consists of 510 rural children ranging in age from 3 to 10 yr. The Nepali children were found to have one of the highest reported prevalences of stunting (65% were less than 90% National study for Health Statistics median height for age). The study children were also 1 to 1.5 kg lighter when compared to US children of the same height. Fat deposits, as measured by anthropometric variables and Hb levels were also very low. Chloramphenicol pharmacokinetics in Ethiopian children of differing nutritional status.

The pharmacokinetics of i.v. chloramphenicol succinate and oral chloramphenicol palmitate were studied in Ethiopian children with different nutritional states. In children with kwashiorkor the plasma clearance of chloramphenicol was significantly lower than in children of normal weight (4.16 ml/min/kg versus 7.53 ml/min/kg). In consequence the mean half-life was prolonged (3.76 h versus 2.85 h) and this led to somewhat higher plasma levels in the kwashiorkor children. The influence of the pathophysiological changes offset one another so that plasma concentrations within the therapeutic range w
