Clinical significance of pathological eye movements in diagnosing posterior fossa stroke.

CONCLUSION: Close observation of pathological eye movements such as disconjugate eye movements, multi-directional gaze nystagmus, and persistent unilateral gaze nystagmus may facilitate the effort of clinicians to arrange magnetic resonance imaging (MRI) study, because physical examinations may overlook the posterior fossa lesions. OBJECTIVE: This paper reviews our experience of patients with posterior fossa stroke via observation of pathological eye movements over the past 10 years. METHODS: Seventy patients with posterior fossa stroke manifested as acute vertiginous attack were admitted. A

Subjective visual vertical deviation in patients with early-onset direction-changing horizontal positional nystagmus.

OBJECTIVE: Otolithic dysfunction is investigated in cases of direction-changing horizontal positional nystagmus (DCHPN) due to peripheral vestibular disorders. METHODS: The static-subjective visual vertical (S-SVV) was conducted in DCHPN cases within 48 h after onset. RESULTS: The absolute values of S-SVV deviations of patients with Light cupula and lateral canal-benign paroxysmal positional vertigo-cupulolithiasis (L-BPPV-Cup) were significantly different from those of healthy subjects (p < 0.001, p < 0.05, respectively), whereas there were no significant differences in those of patients wi

Fluctuating Posterior Canal Function in Benign Paroxysmal Positional Vertigo Depending on How and Where Otoconia Are Disposed.

OBJECTIVE: Though fluctuations in vestibular function represent a common finding in Menière's disease, we describe how benign paroxysmal positional vertigo (BPPV) may result in fluctuations of vestibulo-ocular reflex for the involved canal depending on the disposition of otoliths. PATIENT: A 54-year-old woman suffering from refractory posterior canal (PC)-BPPV resulting in fluctuating PC function. INTERVENTIONS: Diagnostic evaluation and rehabilitative treatment for BPPV involving the affected PC. MAIN OUTCOME MEASURES: Video-Frenzel and video-head impulse test (vHIT) findings before and af

Ocular stability and set-point adaptation.

A fundamental challenge to the brain is how to prevent intrusive movements when quiet is needed. Unwanted limb movements such as tremor impair fine motor control and unwanted eye drifts such as nystagmus impair vision. A stable platform is also necessary to launch accurate movements. Accordingly, nature has designed control systems with agonist (excitation) and antagonist (inhibition) muscle pairs functioning in push-pull, around a steady level of balanced tonic activity, the set-point Sensory information can be organized similarly, as in the vestibulo-ocular reflex, which generates eye moveme

Causes and characteristics of horizontal positional nystagmus.

Direction changing horizontal positional nystagmus can be observed in a variety of central and peripheral vestibular disorders. We tested sixty subjects with horizontal positional nystagmus and vertigo on the Epley Omniax(®) rotator. Monocular video recordings were performed with the right or left ear down, in the supine and prone positions. Nystagmus slow-phase velocity (SPV) was plotted as a function of time. Thirty-one subjects diagnosed with horizontal canalolithiasis had paroxysmal horizontal geotropic nystagmus with the affected ear down (onset 0.8 ± 1 s, range 0-4.9 s, duration 11.7-47.

Characteristics and mechanism of perverted head-shaking nystagmus in central lesions: Video-oculography analysis.

OBJECTIVE: Perverted downbeat head-shaking nystagmus (pdHSN) has been considered a sign of central pathology. Two hypotheses have been advanced as the mechanisms of pdHSN without proper validation. This study aimed to elucidate the mechanism of pdHSN. METHODS: Eighteen patients with pdHSN due to central lesions were subjected to analyses of their oculographic characteristics. The peak velocity, temporal features including the duration and time constant (TC), rotational axis of pdHSN were analyzed. To determine the most relevant mechanism of pdHSN, we compared the TCs of pdHSN with those of do

Bedside evaluation of dizzy patients.

In recent decades there has been marked progress in the imaging and laboratory evaluation of dizzy patients. However, detailed history taking and comprehensive bedside neurotological evaluation remain crucial for a diagnosis of dizziness. Bedside neurotological evaluation should include examinations for ocular alignment, spontaneous and gaze-evoked nystagmus, the vestibulo-ocular reflex, saccades, smooth pursuit, and balance. In patients with acute spontaneous vertigo, negative head impulse test, direction-changing nystagmus, and skew deviation mostly indicate central vestibular disorders. In

Characteristics and mechanism of apogeotropic central positional nystagmus.

Here we characterize persistent apogeotropic type of central positional nystagmus, and compare it with the apogeotropic nystagmus of benign paroxysmal positional vertigo involving the lateral canal. Nystagmus was recorded in 27 patients with apogeotropic type of central positional nystagmus (22 with unilateral and five with diffuse cerebellar lesions) and 20 patients with apogeotropic nystagmus of benign paroxysmal positional vertigo. They were tested while sitting, while supine with the head straight back, and in the right and left ear-down positions. The intensity of spontaneous nystagmus wa

Vestibular implications of noise-induced hearing loss.

An extensive vestibular examination was carried out in a group of 29 noise-exposed technicians. A spontaneous nystagmus was found in 18 persons, and 24 had a positional nystagmus exceeding a velocity of the slow phase of 5 degrees/s in three or more positions. In 17 subjects a cervical nystagmus could be provoked, while a nystagmus preponderance of more than 20% in the rotation test was found in seven persons. A difference in excitability between the labyrinths of more than 20% was shown by seven subjects. None of the subjects showed pathology in the tests for central vestibular disorders. The

3D analysis of nystagmus in peripheral vertigo.

Three-dimensional analysis of nystagmus was carried out in patients with peripheral vestibular diseases using a computerized image recognition technique developed by us. In the present study, we analyzed data from patients with Meniere's disease and vestibular neuritis with the central premise of localizing the pathology in the peripheral vestibular organs. In Meniere's disease, the recordings of all subjects showed two components of eye movements, namely the horizontal and torsional components. On the other hand, most of the patients with vestibular neuritis exhibited all three components of

[Dizziness related to ophthalmological pathology].

Dizziness stands for vertigo (rotatory and nautic) and dysbalance/unstability. Vertigo originates from the vestibular central nervous system; no eye disease, but nystagmus is often present. Dysbalance/unstability: normal ophthalmological findings exclude ophthalmological pathogenesis. Ophthalmological pathology, especially recently developed may be involved. Uncorrected glasses, eye drops (by systemic absorption), flickering light, and migraine may also cause dysbalance/unstability. Rapid referral to specialist is recommended by fast development of: decreased visual acuity, visual field defect

Vestibular dysfunction in Gulf War syndrome.

METHODS: Vestibular complaints of Gulf War veterans were characterized by a nested case-control study of 23 veterans with 3 different Gulf War syndromes and 20 matched control subjects. All subjects completed a standardized symptom questionnaire and underwent standard audiovestibular tests administered by audiologists blinded to group identities. RESULTS: The prevalence of reported dizzy spells was higher in veterans with Gulf War syndromes 1 (100%), 2 (85%), and 3 (100%) than in controls (25%, P < 0.0001). Dizzy spells were more frequent, lasted longer, and involved a wider variety of accomp

Downbeating nystagmus and other ocular motor defects caused by lithium toxicity.

We report the clinical and neuropathologic findings of a 63-year-old woman who died following an accidental lithium overdose that produced coma, respiratory depression, horizontal gaze palsy, and downbeating nystagmus. She also had mild hypomagnesemia. The pathology was cytotoxicity, predominantly in the regions of the nuclei prepositus hypoglossi and medial vestibular nucleus. Damage to this area with kainate and ibotenate in rhesus monkeys has produced horizontal gaze palsy and downbeating nystagmus. In addition, we report our clinical experience during the past 6 years with other examples o

[Automatic analysis by computer of the visual suppression test of pendular rotatory vestibular nystagmus].

This study presents an automatic computerized analysis of the visual suppression test of vestibular nystagmus. Visual suppression is measured during rotatory nystagmus examination. The amplitude variations and the frequency of the nystagmus are computed in the dark and in the light. This allows the computer to furnish with the help of an algorithm the percentage of nystagmus suppressed by ocular fixation. The results of the computerized analysis are compared to a qualitative evaluation. A percentage of 70% and more indicates a normal suppression reflex and corresponds qualitatively to a total

Central Conditions Mimicking Benign Paroxysmal Positional Vertigo: A Case Series.

BACKGROUND AND PURPOSE: Benign paroxysmal positional vertigo (BPPV) is the most common cause of positional vertigo. The term "benign" is consistent with a peripheral vestibular disorder that does not carry the potentially sinister sequelae of a central nervous system (CNS) cause. However, in 12% to 20% of cases, positional vertigo may be attributed to CNS pathology, including tumors of the cerebellum. CASE DESCRIPTION: Here, we present a series of 3 cases in which positional vertigo and nystagmus were the only presenting features in 2 cases of cerebellar tumor and 1 case of obstructive hydroc

Vestibular habituation in student pilots.

The dynamics of vestibular nystagmus were measured in 42 military student pilots (age 19 to 21). Their responses were compared with 40 non-flying subjects of similar age who were also fulfilling requirements for the mandatory Swiss military service. The following differences emerged: the time constant of the nystagmus response after vestibular stimulation was shorter (p less than 0.001; t-test) in student pilots, whereas the gain tended to be higher (p less than 0.025). These changes in the response dynamics are attributed to habituation. Student pilots were additionally tested with conflictin

Eye-movement disorders in brain-stem and cerebellar stroke.

Vertebrobasilar strokes can yield varied disturbances of eye movements, by affecting specific centers and pathways contained in the brain stem and cerebellum. Unique disorders combining supranuclear, nuclear, and infranuclear syndromes may occur. Some eye-movement abnormalities are useful localizing signs (eg, gaze palsies, rotatory nystagmus, and ipsilateral saccadic bias), but many others are not. The use of techniques such as magnetic resonance imaging may provide new insights in clinicotopographic correlations in patients with good recovery, in the absence of pathologic verification.

Measuring visual and quality of life outcomes in nystagmus: a scoping review.

INTRODUCTION: Pathological nystagmus of all aetiologies can cause visual disability and reduced quality of life. Recent research has characterised the dynamic nature of the condition and questioned the validity of widely used outcome measures, such as visual acuity (VA), as an accurate measure of visual function in nystagmus. VA only describes the spatial resolving capacity of the visual system for high contrast stimuli, whereas good visual function requires numerous components including contrast sensitivity, colour vision, depth perception and oculomotor functions. For these reasons, the lite
