Scoping Review of Obesity-Related Nutrition Educational Interventions for Medical Students, Residents and Fellows - a Handful of Paradoxes?

BACKGROUND: To our knowledge, no study assessed the published obesity-related nutrition educational interventions (ONEI) delivered to medical students, residents or fellows. The current scoping review undertook this task. METHODS: We searched PubMed, Web of Science and EMBASE databases. Original articles of ONEI delivered to medical students, residents or fellows were included. The ONEIs' goals, content, teaching and learning characteristics, health professionals involved, duration of the intervention, trainees, target populations, evaluation methods, and outcomes were scrutinized. Extracted

The outcomes of the obesity paradox in pulmonary embolism: a study of the national inpatient sample database from 2016 to 2020.

The "obesity paradox" suggests that, despite a higher baseline risk for adverse health outcomes, obese patients can experience a lower complication and mortality rate in conditions such as pulmonary embolisms (PE). This study aims to examine the association between obesity and inpatient outcomes of PE patients, utilizing the data from the National Inpatient Sample (NIS) database. We conducted a retrospective study analysis of obese adult PE patients (aged ≥ 18) using the NIS database from 2016 to 2020. Patients were categorized as either obese (body mass index (BMI) ≥ 30) or non-obese, excludi

NKX2-2 Mutation Causes Congenital Diabetes and Infantile Obesity With Paradoxical Glucose-Induced Ghrelin Secretion.

CONTEXT: NKX2-2 is a crucial transcription factor that enables specific β-cell gene expression. Nkx2-2(-/-) mice manifest with severe neonatal diabetes and changes in β-cell progenitor fate into ghrelin-producing cells. In humans, recessive NKX2-2 gene mutations have been recently reported as a novel etiology for neonatal diabetes, with only 3 cases known worldwide. This study describes the genetic analysis, distinctive clinical features, the therapeutic challenges, and the unique pathophysiology causing neonatal diabetes in human NKX2-2 dysfunction. CASE DESCRIPTION: An infant with very low

Changes in body weight and blood pressure: paradoxical outcome events in overweight and obese subjects with cardiovascular disease.

BACKGROUND/OBJECTIVES: The Sibutramine Cardiovascular OUTcomes (SCOUT) trial showed a significantly increased relative risk of nonfatal cardiovascular events, but not mortality, in overweight and obese subjects receiving long-term sibutramine treatment with diet and exercise. We examined the relationship between early changes (both increases and decreases) in body weight and blood pressure, and the impact of these changes on subsequent cardiovascular outcome events. SUBJECTS/METHODS: A total of 9804 male and female subjects, aged 55 years or older, with a body mass index of 27-45 kg m(-2) wer

Subcutaneous Fat Is Associated with Improved Survival in Patients with Non-Metastatic Clear Cell Renal Cell Carcinoma: Dissecting the Obesity Paradox.

Introduction: The "obesity paradox" describes the observed association between improved cancer-specific (CSS) and overall (OS) survival observed among obese (BMI ≥ 30 kg/m2) patients with RCC. Prior studies have reported lower stage/grade tumors among obese patients to explain this. This study aimed to evaluate subcutaneous (SFA) and visceral fat area (VFA) associations with CSS, OS, tumor stage and grade among patients with non-metastatic clear cell RCC. Methods: Following IRB approval, patients undergoing nephrectomy for clear cell RCC between 2000 and 2023 were screened for inclusion. Eligi

Impact of the obesity paradox on 28-day mortality in elderly patients critically ill with cardiogenic shock: a retrospective cohort study.

BACKGROUND: Previous studies have shown that the obesity paradox exists in cardiovascular disease (CVD), giving patients a survival advantage, but controversy remains as to whether it applies to patients with cardiogenic shock (CS), especially in the elderly. We therefore aimed to determine whether obesity affects 28-day prognosis in elderly patients with CS. METHODS: We used clinical data from the Medical Information Market in Critical Care IV (MIMIC-IV) database. Critical patients with CS were categorized into two groups based on age; age < 65 years and ≥ 65 years were classified as young a

Body mass index and clinical outcome of severe COVID-19 patients with acute hypoxic respiratory failure: Unravelling the "obesity paradox" phenomenon.

BACKGROUND AND AIMS: Although obesity have been generally shown to be an independent risk factor for poor outcomes in COVID-19 infection, some studies demonstrate a paradoxical protective effect ("obesity paradox"). This study examines the influence of obesity categories on clinical outcomes of severe COVID-19 patients admitted to an intensive care unit with acute hypoxic respiratory failure requiring either non-invasive or invasive mechanical ventilation. METHODS: This is a single centre, retrospective study of consecutive COVID-19 patients admitted to the intensive care unit between 03/2020

Obesity paradox in prostate cancer: increased body mass index was associated with decreased risk of metastases after surgery in 13,667 patients.

INTRODUCTION: Obesity might negatively affect prostate cancer (PCa) outcomes. However, evidence according to the associations between obesity and metastases-free survival after radical prostatectomy (RP) is still inconsistent. METHODS: We relied on PCa patients treated with RP at the Martini-Klinik Prostate Cancer Center between 2004 and 2015. First, multivariable Cox regression analyses examined the impact of obesity on metastases after RP. Last, in a propensity score matched cohort, Kaplan-Meier analyses assessed metastases-free survival according to body mass index (kg/m2) (BMI) strata (≥

Inverse relationship between the body mass index and severity of carotid siphon calcifications (another obesity paradox): Results from the Atahualpa Project.

BACKGROUND AND AIMS: The importance of the obesity paradox in the intracranial vasculature has not been explored. We assessed whether the body mass index (BMI) correlates with severity of carotid siphon calcifications (CSC) in community-dwelling adults. METHODS: Using a population-based study design, Atahualpa residents aged ≥40 years underwent head computed tomography (CT) for assessment of CSC. We evaluated the association between BMI and severity of CSC (dependent variable) using regression models adjusted for demographics and cardiovascular risk factors. RESULTS: Of 830 people enrolled i

Muscle Strength as a Key Independent Predictor of Arterial Stiffness and Metabolic Syndrome in Aging Mexicans: Unveiling the Sarcopenic Obesity Paradox.

Background/Objectives: The cardiometabolic consequences of sarcopenic obesity (SO) remain poorly characterized in Latin America. We hypothesized that SO uniquely concentrates aortic stiffness, metabolic syndrome, and vitamin D deficiency in older Mexican adults, and that handgrip strength exhibits a dose-response relationship with cardiometabolic risk independent of adiposity. Methods: We conducted a cross-sectional analysis of 2087 adults ≥ 50 years from the Mexican Health and Aging Study (MHAS) 2012. Four phenotypes were defined using EWGSOP2 criteria: lean-fit, lean-sarcopenic, obese-non-sa

Association of body mass index and long-term mortality in patients from nationwide LIPIDOGRAM 2004-2015 cohort studies: no obesity paradox?

BACKGROUND: An obesity paradox has been described in relation to adverse clinical outcomes (e.g., mortality) with lower body mass index (BMI). AIMS: We sought to evaluate the association between BMI and weight loss with long-term all-cause mortality in adult populations under the care of family physicians. METHODS: LIPIDOGRAM studies were conducted in primary care in Poland in 2004, 2006, and 2015 and enrolled a total of 45,615 patients. The LIPIDOGRAM Plus study included 1627 patients recruited in the LIPIDOGRAM 2004 and repeated measurements in 2006 edition. Patients were classified by BMI

Influence of BMI on inducible ventricular tachycardia and mortality in patients with myocardial infarction and left ventricular dysfunction: The obesity paradox.

BACKGROUND: There is little known about the influence of obesity on ventricular electrical remodelling after myocardial infarction. The aim of our study was to assess the relationship between body mass index (BMI) and the primary outcome of inducible-VT and the secondary outcome of all-cause mortality in consecutive patients who presented with ST elevation myocardial infarction (STEMI) and LV-dysfunction (LVEF ≤ 40%). METHODS AND RESULTS: Consecutive patients (n = 380) with STEMI and LV-dysfunction (LVEF ≤ 40%) underwent electrophysiological (EP) studies for risk-stratification. Inducible-VT

Unpacking the Obesity Paradox in Lung Cancer: A Nationwide Analysis of In-Hospital Mortality From the US National Inpatient Sample (2020-2022).

Background Lung cancer remains the leading cause of cancer-related mortality worldwide. Although obesity is an established risk factor for multiple malignancies, numerous studies have reported an "obesity paradox," wherein patients with obesity demonstrate improved survival compared with normal-weight or underweight individuals. Whether this survival advantage reflects a true protective biological mechanism or methodological artifacts such as reverse causation driven by cancer cachexia and selection bias remains controversial, particularly within the acute inpatient setting. We evaluated the i

The obesity paradox in bone health: high prevalence of osteoporosis and its non-linear association with BMI among urban elderly in China.

Osteoporosis is a major public health concern in aging populations. Although data from rural Chinese populations are available, large-scale epidemiological studies on osteoporosis prevalence and its risk factors, particularly the complex relationship with body mass index (BMI), remain limited among urban community-dwelling elderly in China. A community-based cross-sectional study was conducted from April to August 2025 among elderly residents (aged ≥ 60 years) in Jiangling Sub-district, Suzhou. Participants were derived from a government-sponsored, non-mandatory health examination program thro

Body mass index and mortality: The "Obesity Paradox" in critically ill patients with intra-abdominal infection or sepsis - An international cohort study.

OBJECTIVE: To assess the relationship between body mass index (BMI, i.e., kg/m2) and mortality in ICU patients with intra-abdominal infection or sepsis. METHODS: This is a secondary analysis from AbSeS, an international, observational cohort study including patients with intra-abdominal infection (n = 2588). Patients were classified as underweight (BMI < 18.5), normal weight (BMI 18.5-24.9), overweight (BMI 25-29.9), or obese (BMI ≥ 30). Independent relationships with mortality were assessed by logistic and Cox regression. Results reported as odds ratios (OR) or hazard ratios (HR) 95 % and co

The impact of body composition on mortality of COVID-19 hospitalized patients: A prospective study on abdominal fat, obesity paradox and sarcopenia.

BACKGROUND & AIMS: Obesity has been described as a predisposing risk factor to severe forms of COVID-19, but conflicting results are emerging on its real impact on the mortality of COVID-19. We aimed to compare clinical outcomes and mortality among COVID-19 patients according to obesity, metabolic syndrome and adiposity distribution. METHODS: We conducted a prospective observational study of all consecutive adult patients with a confirmed diagnosis of SARS-CoV-2 infection admitted to the Infectious Diseases Clinic at Udine Hospital, Italy, from January 2021 to February 2021.
