INTRODUCTION: Acute aortic syndrome (AAS) is a group of acute and critical conditions, including acute aortic dissection (AAD), acute intramural haematoma and penetrating aortic ulcer. High mortality and morbidity rates result in a poor patient prognosis. Prompt diagnoses and timely interventions are paramount for saving patients' lives. In recent years, risk models for AAD have been established worldwide; however, a risk evaluation system for AAS is still lacking in China. Therefore, this study aims to develop an early warning and risk scoring system in combination with the novel potential bi

ImportanceThe role of anticoagulant therapy as an adjuvant treatment for sepsis remains controversial. Although observational studies have often suggested benefits, large-scale randomized controlled trials have yielded conflicting results, and the risk-to-benefit profile across different anticoagulant agents remains unclear.ObjectiveTo evaluate the efficacy and safety of anticoagulant therapy and its effect on all-cause mortality and major bleeding events in adult patients with sepsis as well as explore differences between study designs and anticoagulant types.Data sources: A systematic search

Randomized controlled clinical trials and real-world observational studies provide complementary information but with different validity. Some clinical questions (disease behavior, prognosis, validation of outcome measures, comparative effectiveness, and long-term safety of therapies) are often better addressed using real-world data reflecting larger, more representative populations. Integration of disease history, clinician-reported outcomes, performance tests, and patient-reported outcome measures during patient encounters; imaging and biospecimen analyses; and data from wearable devices inc

Hyperkalemia is a common electrolyte disorder, especially in chronic kidney disease, diabetes mellitus, or heart failure. Hyperkalemia can lead to potentially fatal cardiac dysrhythmias, and it is associated with increased mortality. Determining whether emergency therapy is warranted is largely based on subjective clinical judgment. The Investigator Network Initiative Cardiovascular and Renal Clinical Trialists (INI-CRCT) aimed to evaluate the current knowledge pertaining to the emergency treatment of hyperkalemia. The INI-CRCT developed a treatment algorithm reflecting expert opinion of best

BACKGROUND: Until 2023, recommendations for prescribing protein to critically ill patients were largely based on observational studies and non-randomized interventions. Several large, multicentre non-randomized studies of intervention conducted between 2009 and 2017 consistently reported that higher protein intake during the acute phase of critical illness was associated with lower mortality. However, major randomized controlled trials published in recent years (the EFFORT Protein trial in 2023, the PRECISe trial in 2024, and the TARGET Protein trial in 2025) have found no clinical benefit of

Most clinical questions in neurology are inherently causal, yet observational studies have long been interpreted in terms of associations rather than causal effects of well-defined interventions. This limitation has contributed to persistent biases in neurology research, including inappropriate covariate adjustment, temporal misalignment, immortal-time bias, and susceptible depletion. Recent developments in causal inference offer a formal framework to overcome these issues through counterfactual reasoning, causal diagrams, and g-methods. The target trial emulation approach translates these con

The issue of assessing the effectiveness of health technologies (drugs, devices, etc.) through observational studies is becoming increasingly important as registration and market access agencies consider them in their evaluation process. In this context, observational studies must be able to provide real demonstrations of a level of reliability comparable to those produced by the conventional randomized controlled trial (RCT) approach. The objective of the roundtable was to establish the acceptability criteria for an observational study (non-randomized, non-interventional study) to be able to

The pathophysiology of migraine remains poorly understood. Like most migraine preventive therapies, patent foramen ovale (PFO) closure was never intended for the treatment of migraine. After closure of PFO for other reasons, migraine symptom reduction/elimination was noted in some patients. Subsequent small trials failed to prove its benefit. There is significant evidence suggesting a platelet-mediated mechanism linking migraines to PFO. The GORE RELIEF Clinical Study is a randomized, blinded, placebo- and sham-controlled trial, currently enrolling. The study design is meant to optimize patien

Introduction: Xpert MTB/RIF is a rapid diagnostic instrument for pulmonary tuberculosis (TB). However, studies reported varied accuracy of Xpert MTB/RIF in detecting Mycobacterium tuberculosis in pericardial effusion. Methods: We performed a systematic review of literature in PubMed, published up to February 1, 2020, according to PRISMA guidelines. We screened cross-sectional studies, observational cohort studies, and randomized control trials that evaluated the accuracy of Xpert MTB/RIF in diagnosing TB pericarditis. OBJECTIVE: To evaluate the causes of death and risk factors of pulmonary thromboembolism.
METHODS: Pubmed, English Medical Current Contents, Chinese Conference Data and Chinese Biomedical Database were searched from January 1995 up to May 2011. And the references of these studies were also examined. Observational studies were assessed according to suggestion of quality assessment with references. Randomized control trials (RCT) were assessed with Jadad scale. Software RevMan 5.1 was used to examin the heterogeneity of trials. The fixed or random effect model was employed to pool the risk rati

While normal human body temperature is often cited as 36.1-37.2 °C, there is high-quality evidence demonstrating that older adults typically have lower baseline body temperatures. In contrast to hypothermia (< 35 °C) which is well recognized as a medical emergency, there is emerging evidence that low body temperature (35.0-36.0 °C) may predict poor outcomes in the geriatric population. This narrative review synthesizes the current literature on the association between low body temperature in adults aged ≥ 65 years and common geriatric co-morbidities. Across observational and cohort studies, lo

INTRODUCTION: Cholecystitis is a highly prevalent disease that imposes a substantial burden on the healthcare system. Despite strong underlying evidence, guideline adherence in the treatment of cholecystitis remains low. Moreover, important gaps in knowledge persist that must be addressed to optimise existing guidelines. The primary aim is to assess the nationwide variation in cholecystitis treatment and identify opportunities to improve guideline adherence. Secondary aims include determining the best cystic duct closure method; the best model to predict concomitant choledocholithiasis; the op

BACKGROUND: Patients at the end-of-life may experience refractory symptoms of which pain, delirium, vomiting and dyspnea are the most frequent. Palliative sedation can be considered a last resort option to alleviate one or more refractory symptoms. There are only a limited number of (qualitative) studies exploring the experiences of relatives of sedated patients and their health care professionals (HCPs). The aims of this study protocol are: 1) to elicit the experiences of bereaved relatives and health care professionals of patients treated with palliative sedation and 2) to explore the unders

Severe viral respiratory infections have gained attention in recent years. One aspect is the cognitive impairment that many patients experience either during or following infections, such as COVID-19. The aim of this Journal club is to present and discuss different studies on this topic in the hope of proposing future research questions. The four studies discussed all investigate COVID-19 patients in observational cohorts and the possible association between severe viral respiratory infection and cognitive impairment. Needham et al. and Born et al. investigate the immunological signature of CO

BACKGROUND: Suicide is a leading cause of death in children and youth, with suicidal thoughts and suicide attempts (referred to as non-fatal suicidal behaviors (NFSB)) being among its strongest predictors. Positive parenting (e.g., warmth, responsiveness), negative parenting (e.g., control, hostility), and parent-child relationship quality (e.g., trust, communication) have been reported to be associated with differences in NFSB in this population. To date, no comprehensive systematic review has considered together the wide range of parenting factors studied in relation to NFSB, and no meta-ana

Organic contaminants have long been known to have negative consequences for wildlife health. For avian carnivores, particular concern surrounds the use of compounds such as pesticides and pharmaceutical drugs, derived from global agricultural and veterinary practices. Previous cases of avian poisoning in North America, Europe and Asia have received great attention, owing to significant declines in avian populations and wider consequences. By contrast, the Neotropics, comprised of Latin America and the Caribbean, has received relatively little attention, despite an extensive agricultural sector

BACKGROUND: In the study on triglyceride-induced pancreatitis (TG-IAP), a core clinical dataset using the Jandhyala method was developed to collect the minimum amount of information for each patient presenting with TG-IAP globally. This approach offered a unified framework for observing multiple populations of TG-IAP patients using the same set of indicators, resulting in a considerably larger and uniform real-world population. It was understood that when this core dataset is implemented in a patient registry it could address the issue of missing data in observational studies and produce highe

INTRODUCTION: Delayed cerebral ischaemia (DCI) caused by aneurysmal subarachnoid haemorrhage (aSAH) is the most frequent complication and typically contributes to poor neurological outcome or deterioration of patients' condition. Therefore, early accurate and effective prediction of DCI is urgently needed. This study aims to construct a dynamic nomogram for precisely calculating the risk of DCI in patients with aSAH. Internal validation of this tool is conducted using the training cohort, and independent external validation is completed by using other medical centre datasets.

BACKGROUND: Specific surgical procedures, such as upper abdominal and thoracic surgery, are connected to an increased predisposition of postoperative pulmonary complications (PPCs). The incidence of PPCs could vary approximately between 20-90% with upper abdominal surgery, which can be minimized by using treatment procedures that increase lung capacity and encourage inspiration. This review aims to examine the effectiveness of already existing evidence-based interventions that promote lung expansion, thereby preventing PPCs.
METHOD: We mainly focused on the existing evidence of preoperative e
