Deep brain stimulation in obsessive-compulsive disorder.

The use of deep brain stimulation in psychiatric disorders has received great interest owing to the small risk of the operation, the reversible nature of the technique, and the possibility of optimizing treatment postoperatively. Currently, deep brain stimulation in psychiatry is investigated for obsessive-compulsive disorder, Gilles de la Tourette's syndrome, and major depression. This chapter reviews the application of deep brain stimulation in obsessive-compulsive disorder. Preliminary results suggest that deep brain stimulation in obsessive-compulsive disorder can effectuate a decrease of

Brain imaging in pediatric obsessive-compulsive disorder.

OBJECTIVE: To review progress in understanding pediatric obsessive-compulsive disorder (OCD). The focus is on the frontal-striatal-thalamic model of OCD, neurobiological and genetic studies of the disorder, and their influence on recent advances in treatment. METHOD: Computerized literature searches were conducted with the key words "obsessive-compulsive disorder" in conjunction with "pediatric," "genetics," and "imaging." RESULTS: Neuroimaging studies find evidence to support the frontal-striatal-thalamic model. Genetic and neurochemical studies also implicate glutamate in the pathological

Neurocognitive profile analysis in obsessive-compulsive disorder.

Although neurocognitive deficits have been identified in obsessive-compulsive disorder (OCD), little research has focused on whether these deficits are generalized or specific to a given cognitive domain. We assessed the relative strengths and weaknesses of 26 adult patients with OCD compared to 38 age- and sex-matched healthy volunteers in domains of motor, verbal memory, visual memory, reasoning/problem solving, processing speed processing, and language. Profile analysis revealed an overall neurocognitive deficit of 1/2 standard deviation in OCD patients versus healthy volunteers, with relat

Response inhibition deficits in obsessive-compulsive disorder.

Difficulty inhibiting irrelevant information may play a central role in the aetiology of obsessive-compulsive disorder (OCD). The aim of the present study was to determine whether OCD subjects (n=20) exhibit deficits in behavioural and cognitive inhibition compared with a clinical control group diagnosed with panic disorder (n=20). All subjects were administered a Go/Nogo task (a measure of behavioural inhibition) and a Stroop test (a measure of cognitive inhibition). OCD subjects made more commission errors on the Go/Nogo task, and they made more errors and displayed longer reaction times on

Altered cortical excitability in obsessive-compulsive disorder.

OBJECTIVE: To assess cortical inhibitory and excitatory mechanisms in obsessive-compulsive disorder (OCD). BACKGROUND: Transcranial magnetic stimulation (TMS) studies have found decreased neuronal inhibition and a reduced cortical silent period in the primary motor area in Tourette's syndrome, focal dystonia, and other disorders believed to involve dysfunction of subcortical structures, including the basal ganglia. Dysfunction of the basal ganglia and linked regions also has been implicated in OCD, which has significant clinical and familial overlap with tic disorders. METHODS: We applied th

Obsessive-compulsive disorder in post-streptococcal infection.

OBJECTIVE: We describe the sudden onset of obsessive-compulsive symptoms following a peritoneal infection with alpha-haemolytic streptococci. CLINICAL PICTURE: A 35-year-old woman with no past history or family history of obsessions or compulsions developed these symptoms 2 weeks after a peritoneal infection. TREATMENT: The patient received 80 mg fluoxetine daily. OUTCOME: She responded to treatment with a progressive reduction in symptoms. CONCLUSIONS: It is suggested that these obsessions and compulsions may be related to an autoimmune response to the streptococcal infection.

Gender and psychopathology in obsessive compulsive disorder.

The gender difference in OCD was studied in 52 patients attending the psychiatric OPD and it was found that 35 (67.30%) patients were males compared to 17 (32.70%) females, which constituted 0.72 % and 1.03% of the male and female patients attending the OPD during the study period, respectively. Hence, inspite of the male preponderance in the study sample, it might not reflect the true prevalence of the disorder in the community. The age of onset for males was lower than that for females, but it did not reach statistical significance. The mean duration of illness for the entire sample was 7.48±

EEG frequency analysis in obsessive-compulsive disorder.

The cerebral basis of obsessions and compulsions has attracted increasing attention in neuropsychology and neuropsychiatry. Clinical and imaging studies have suggested frontal lobe dysfunction in some cases of obsessive-compulsive disorder and Tourette syndrome with obsessional symptoms. We compared EEG spectral measures in 20 such patients not taking medications and 12 neurologically intact unmedicated controls. EEG was recorded from O1-A1+A2, O2-A1+A2, Fz-A1+A2, F7-C3, F8-C4, T5-O1 and T6-O2. One-minute epochs of artifact-free EEG were used for compressed spectral array and calculation of ti

Antidepressant-induced hypomania in obsessive-compulsive disorder.

Out of ten consecutive patients with DSM-III-R obsessive-compulsive disorder without any previous history of bipolarity, three patients showed antidepressant-induced hypomania (clomipramine, one patient; fluvoxamine, two patients) within the first 5 to 8 weeks of the drug treatment. These data support the previous results on a strong association between obsessive-compulsive disorder and bipolar affective illness.

A family study of obsessive-compulsive disorder.

OBJECTIVE: The goal of this study was to determine 1) whether obsessive-compulsive disorder is familial, 2) whether there is a familial relationship between obsessive-compulsive disorder and Gilles de la Tourette's syndrome and chronic tics, and 3) whether different familial types of obsessive-compulsive disorder exist. METHOD: In this family study, all available first-degree relatives of 100 probands with obsessive-compulsive disorder were interviewed directly with structured interviews, and best-estimate diagnoses were assigned. In addition to the 466 first-degree relatives of the probands,

Childhood movement disorders and obsessive compulsive disorder.

Recent investigations of childhood-onset obsessive compulsive disorder (OCD) and pediatric movement disorders such as tics, Tourette's syndrome (TS), and Sydenham's chorea suggest that these disorders may be related. Although comorbid obsessive-compulsive symptoms have long been recognized in individuals with TS, more recent studies have demonstrated that tics and TS are surprisingly common in children with primary OCD, and further, that the two disorders seem to have a common genetic vulnerability. Obsessive-compulsive symptoms are also manifest in Sydenham's chorea, a neurologic variant of r

Onset of obsessive-compulsive disorder in pregnancy.

OBJECTIVE: Although the role of pregnancy and childbirth in postpartum psychosis and depression has been studied, the association between pregnancy and obsessive-compulsive disorder has not been specifically addressed. The authors evaluated the role of pregnancy in the onset of obsessive-compulsive disorder. METHOD: Female patients with obsessive-compulsive disorder (N = 106) completed a questionnaire assessing age at onset of symptoms, marital status, number of children, age at each pregnancy, and life events associated with the onset of obsessive-compulsive disorder. RESULTS: Of the 106 wo

[Obsessive compulsive disorder. Clinical and epidemiologic studies].

Until recently, Obsessive-Compulsive Disorder (OCD) was considered rare trouble with rather poor outcome. Currently progress in behavioral psychology, psychopharmacology and methodology of epidemiologic studies multiplying by 50 the traditional prevalence rates, give an impetus to the interest in this pathology. Recent clinical and epidemiologic data in OCD are reported in this paper. Multiple questions are evoked such as the issue of OCD homogeneity, the meaning of comorbidity with other psychological disorders: depression, panic attacks, schizophrenia, Gilles de la Tourette syndrome, the rea

Obsessive-compulsive disorder apparently related to abortion.

This case study presents a young woman who developed a severe obsessive-compulsive disorder after a routine medical procedure. It is suggested that this procedure brought back repressed guilt from three abortions and thus led to the onset of symptoms. The case is discussed in relationship to available research and theory.

Clinical features of obsessive-compulsive disorder among individuals who experience health-related obsessive-compulsive symptoms.

BACKGROUND: While some obsessive-compulsive disorder (OCD) phenotypes are well-established and better understood, it is unclear whether the presence of health-related obsessive-compulsive symptoms (OCS) is associated with specific clinical characteristics. We aimed to investigate whether OCS involving concerns with illness, diseases, body parts or aspects of appearance (i.e. health-related OCS) are associated with differences in demographics, experience of stressful life events, clinical severity, clinical course, endorsement of established OCD dimensions, and psychiatric comorbidities. Childhood emotional maltreatment, maladaptive coping and obsessive-compulsive symptoms in patients with obsessive-compulsive disorder.

Previous studies have reported childhood emotional maltreatment (CEM) to be associated with specific obsessive-compulsive (OC) symptoms, but maladaptive coping, which may be the underlying mechanism in this relationship, has not been evaluated yet. Thus, the present study aimed to examine the effects of CEM on the OC symptoms of responsibility for harm and unacceptable thoughts, as well as obsessive-compulsive disorder (OCD) severity, through maladaptive coping, including cognitive avoidance, experiential avoidance and emotional suppression in OCD patients (n = 360). The results showed that CE

Predictors of the 6-year outcome of obsessive-compulsive disorder: Findings from the Netherlands Obsessive-Compulsive Disorder Association study.

OBJECTIVE: Obsessive-compulsive disorder is characterized by a chronic course that can vary between patients. The knowledge on the naturalistic long-term outcome of obsessive-compulsive disorder and its predictors is surprisingly limited. The present research was designed to identify clinical and psychosocial predictors of the long-term outcome of obsessive-compulsive disorder. METHODS: We included 377 individuals with a current diagnosis of obsessive-compulsive disorder, who participated in the Netherlands Obsessive Compulsive Disorder Association study, a multicenter naturalistic cohort stu

[19 pandemic and obsessive compulsive disorder. How does the COVID-19 pandemic influence symptoms in obsessive compulsive disorder patients?].

The COVID-19 pandemic had a significant psychological impact on the mental state of both healthy and psychiatric patients. The authors involved 34 obsessive-compulsive patients in their study. The patient group included both patients with and without symptoms of contamination, patients with and without COVID-19 infection, and patients with and without continuous, available therapy. Changes in general condition as a result of the pandemic that had existed for nearly two years, adherence to health standards, and changes in compulsive symptoms were assessed. The results show that the non-specific
