Peripheral Nerve Stimulation Implant for Chronic Posttraumatic Hip and Pelvic Pain: A Case Report.

BACKGROUND: Chronic hip and pelvic pain following trauma can be challenging to treat, particularly when pain originates from multiple nerve distributions. Traditional treatments often provide limited relief; peripheral nerve stimulation is a minimally invasive alternative. CASE REPORT: A 68-year-old woman with persistent pelvic and hip pain following a right iliac wing fracture reported minimal response to medications and injections. Diagnostic nerve blocks targeting the obturator nerve, femoral sensory branches, and middle cluneal nerve, provided significant but short-term pain relief. A per

A stepwise approach to 'groin pain': a common symptom, an uncommon cause.

A 70-year-old man was admitted with stinging pain in his right groin radiating towards his inner thigh. In a few days, the pain increased in intensity, he experienced diminished strength in his right leg and got dependent on morphine. Conventional investigations did not show any pathology. A CT-scan revealed a mass in the obturator foramen, indicative of a possible obturator hernia or neuroma of the obturator nerve. However, an additional MRI showed a well-circumscribed cystic mass in the obturator foramen, indicative of a ganglion cyst. This cyst was percutaneously punctured under the CT-guid

Juxtaneural ganglia arising from the hip joint: focus on magnetic resonance imaging findings and clinical manifestations.

OBJECTIVE: To present cases of juxtaneural ganglia arising from the hip with a discussion of the magnetic resonance imaging (MRI) findings, presenting symptoms, and possible treatment option. MATERIALS AND METHODS: Two radiologists performed a consensus review of MRI scans obtained between January 2013 and March 2021 to identify patients with juxtaneural ganglia around the hip. A total of 11 patients with 11 juxtaneural ganglia were identified. Medical records and MRI findings were retrospectively reviewed. RESULTS: Eight patients had lesions involving the sciatic nerve, and three patients h

Perineural dextrose injections in the treatment of lower urinary tract symptoms and dyspareunia induced by obturator neuralgia.

Obturator neuralgia is commonly diagnosed and treated in orthopedics. It produces groin pain, sensory alteration in the medial thigh (dysesthesia, sensory loss, or pain), adductor muscle weakness and pain/restriction of hip movements. Basically, the clinical diagnosis of obturator neuralgia is made by producing pain during internal rotation of the hip against resistance ("obturator sign") or by extension and lateral leg movements. We postulate obturator neuralgia can induce lower urinary tract symptoms and dyspareunia and be diagnosed by using three clinical signs characterizing neuralgia ever

Obturator hernia revisited: surgical anatomy, embryology, diagnosis, and technique of repair.

Obturator hernia is the protrusion of intraperitoneal or extraperitoneal organs or tissues through the obturator canal. The first case was published by de Ronsil in 1724. Obturator hernia is more common in older malnourished women due to loss of supporting connective tissue and the wider female pelvis. The hernia sac usually contains small bowel, especially ileum. It may follow the anterior or posterior division of the obturator nerve. In most cases, obturator hernia presents with intestinal obstruction of unknown cause. It may present with obturator neuralgia, as a palpable mass or, in cases

Double-powered free gracilis muscle transfer for smile reanimation: A longitudinal optoelectronic study.

The choice of the motor donor nerve is a crucial point in free flap transfer algorithms. In the case of unilateral facial paralysis, the contralateral healthy facial nerve can provide coordinated smile animation and spontaneous emotional expression, but with unpredictable axonal ingrowth into the recipient muscle. Otherwise, the masseteric nerve ipsilateral to the paralysis can provide a powerful neural input, without a spontaneous trigger of the smile. Harvesting a bulky muscular free flap may enhance the quantity of contraction but esthetic results are unpleasant. Therefore, the logical solu

Bilateral obturator neuropathy caused by an intrapelvic fibrosarcoma with myofibroblastic features in a dog.

A nine-year-old female Rottweiler presented with a 6-week history of progressive impairment of hindlimb adduction. Clinical examination showed abduction of both hind legs when walking on a smooth surface, pain at the medial surface of the left thigh, and an intrarectal palpable mass at the pelvic floor. Electromyography demonstrated fibrillation potentials in the adductor muscles on both sides. Pelvic radiographs showed severe osteolysis of the ischium. Gross post-mortem examination following euthanasia disclosed a large retroperitoneal mass, invading the obturator foramina and compressing bot

Femoral nerve and lumbar plexus injury after minimally invasive lateral retroperitoneal transpsoas approach: electrodiagnostic prognostic indicators and a roadmap to recovery.

Injury to the lumbosacral (LS) plexus is a well-described complication after lateral retroperitoneal transpsoas approaches to the spine. The prognosis for functional recovery after lumbosacral plexopathy or femoral/obturator neuropathy is unclear. We designed a retrospective case-control study with patients undergoing one-level lateral retroperitoneal transpsoas lumbar interbody fusion (LLIF) between January 2011 and June 2016 to correlate electrodiagnostic assessments (EDX) to physiologic concepts of nerve injury and reinnervation, and attempt to build a timeline for patient evaluation and re

Nerve transfer for restoration of lower motor neuron-lesioned bladder and urethra function: establishment of a canine model and interim pilot study results.

OBJECTIVE: Previous patient surveys have shown that patients with spinal cord or cauda equina injuries prioritize recovery of bladder function. The authors sought to determine if nerve transfer after long-term decentralization restores bladder and sphincter function in canines. METHODS: Twenty-four female canines were included in this study. Transection of sacral roots and hypogastric nerves (S Dec) was performed in 6 animals, and 7 animals underwent this procedure with additional transection of the L7 dorsal roots (L7d+S Dec). Twelve months later, 3 L7d+S Dec animals underwent obturator-to-p

Temporospatial parameters of gait after obturator neurolysis in patients with spasticity.

OBJECTIVE: The management of spasticity should be implemented with the most appropriate pharmacologic agents. Ideally, these agents should provide functional improvement with minimal adverse effects. The aim of this study was to evaluate the width of the base of support and the velocity of gait before and after obturator nerve block. Blocks were performed with aqueous phenol solution in patients with unilateral hip adductor muscle overactivity that resulted in scissoring gait. The goal was to analyze functional improvement and quantify outcomes and to attempt to document adverse effects. DESI

Dual-Innervated Free Gracilis Muscle Transfer for Facial Reanimation in Children.

BACKGROUND: Facial palsy may have deleterious effects for pediatric patients. The most common reconstruction is 2-stage free gracilis muscle transfer (FGMT) after cross-face nerve graft (CFNG). This requires a prolonged period from time of surgery to smile. New techniques using both a CFNG and motor nerve to masseter (MNM) as dual power sources in a single-stage surgery have been described in adults. Here, we examine our experience with this technique in children. METHODS: A retrospective study was performed examining patients who underwent dual-innervated single-stage FGMT at 2 pediatric hos

Continuous Pericapsular Nerve Group Block for Hip Surgery: A Case Series.

The pericapsular nerve group (PENG) block targets articular branches of the obturator, accessory obturator, and femoral nerves. This block has mostly been used to reduce pain caused by hip fractures. A catheter inserted in the plane between the iliopsoas muscle and iliopubic eminence can extend the analgesic duration. We describe the use of this block for successful pain relief of 10 patients with hip fracture (intertrochanteric, subtrochanteric, neck of femur) scheduled for surgery and continuing analgesia in the postoperative period with a catheter inserted in the musculofascial plane as des

[Two cases of rare abdominal hernias].

Two cases of rare abdominal hernias were presented: sciatic and obturator. Both presented cases confirm diagnostic difficulties, often leading to delay in therapy. The best way to reduce mortality in these patients seems to be the increase of number of preoperative diagnoses. In obturator hernia's diagnosis the attention is payed to Romberg's symptom, although in sciatic--to symptoms of sciatic nerve irritation or anuria. All diagnostic doubts should be solved with the help of herniography or pelvic CT.

Surgical and anatomic study of calving paralysis.

Dissections of the bovine pelvic inlet region were made to determine which nerves were vulnerable to fetal-induced traumatic damage. Surgical sections of the obturator nerves and the lumbar roots of the sciatic (ischiatic) nerves were done to determine a possible cause of calving paralysis. Only 1 of 11 cattle in which bilateral obturator nerve section was done was unable to stand after the operation, and in this individual, secondary stifle injuries were found on necropsy. Subsequent surgical operations were performed on some of these cattle in order to section the lumbar root of the sciatic
