As health care spending continues to climb, government and industry, as the two major purchasers of health care services, are intensifying their scrutiny over health care delivery in an attempt to reduce their health care burden. The first round of utilization controls and reimbursement restrictions focused on necessity of admission and efficiency of care, causing a profound effect on hospital-based services. Declining occupancy rates, reduced inpatient reimbursements, and mounting contractual losses have pushed many hospitals to the point of financial disaster. The second round of controls ha

BACKGROUND: High burnout is reported among health professionals providing in-patient care to patients with coronavirus disease 2019 (COVID-19). Data are lacking on job stressors and burnout among health providers working in dedicated outpatient facilities for patients with suspected or confirmed COVID-19.
METHODS: This cross-sectional study, using a parallel mixed-methods design, was carried out in 2021-2022 among 22 nurses and 22 primary-care physicians working at a COVID Outpatient Respiratory Center (CORC) (100% participation). Work conditions were assessed via the nurse- and physician-spe

OBJECTIVES: Occupational medicine lies at the interface between work and health. Not only do workplace hazards impact health, but our state of health influences our ability to get to work, to perform work, to tolerate work, and to gain a measure of satisfaction from the work we do. Comprehensive occupational medicine requires familiarity with the work that patients do; knowledge of the workplace itself and its hazards; appreciation of the social forces that shape work; and understanding of how chemical, physical, biologic, mechanical, and psychosocial agents influence health. Many practitioner

Generalized anxiety and mixed anxiety-depression have received less attention than the major mood and anxiety disorders ith respect to their possible effects in increasing disability and health care utilization. A review of recent studies, however, indicates that these conditions are prevalent in primary care medical settings and are associated with significant social and occupational disability. Generalized anxiety disorder is also one of the most common diagnoses seen in patients presenting with medically unexplained somatic complaints such as chest pain, irritable bowel symptoms, and hyperv

BACKGROUND: Cardiac electrophysiologists and electrophysiology (EP) laboratory professionals face underexplored occupational hazards, including direct and indirect effects of radiation exposure, orthopedic strain, and pregnancy-related concerns. Despite advances in radiation-reduction strategies, cumulative exposure continues to merit attention.
OBJECTIVE: This study aimed to better understand these occupational risks.
METHODS: A 20-question global (multiple-choice and open-ended) survey was distributed to Heart Rhythm Society members, including EP physicians and nonphysician members (nurses

BACKGROUND: Italian Law 81/08 (so-called "Unified Text of Laws on Health and Safety at Work"), came into force on 15 May 2008 and incorporates provisions related to medical surveillance of drug and alcohol dependency at the workplace.
OBJECTIVES: Occupational health traditionally addresses the issue of protection of worker from occupational hazards. The issue of protection of third parties from behaviour of workers resulting from drug and alcohol dependency implies an original methodological approach, involving full cooperation of employer, employees, and health and safety consultants.

A systematic assessment was made of the occurrence of renal cancer among men by industrial and occupational classification using the Cancer-Environment Registry, which links cancer incidence (1961-79) and census data (1960) with industry and occupation for all employed individuals in Sweden. Data were analysed separately for cancers of the renal parenchyma and pelvis. Significantly increased risks for renal cell cancer were observed for several professional and white collar occupations, including physicians and others in the health care industry. By contrast, the risks for renal pelvis cancer

BACKGROUND: It has been shown that a recently defined stressor, 'illegitimate tasks', has negative effects on employees' work motivation and health. Better understanding of the illegitimate tasks undertaken by physicians might contribute to a more resource-efficient division of labour within the health care system, with beneficial effects on organisational economics and employee performance. We aimed to investigate the prevalence of unreasonable illegitimate tasks, their associations with workplace variables and their impact on health, in particular sickness presenteeism.

Background: Healthcare workers (HCWs) have experienced sustained occupational exposure to SARS-CoV-2 throughout the COVID-19 pandemic. While infection prevention and control (IPC) practices have been widely implemented, limited prospective data exist on how occupational exposure risk and adherence to protective practices evolve over time, particularly beyond the acute phases of the pandemic. This study aimed to prospectively assess occupational and community exposure risk to COVID-19 among HCWs and to evaluate temporal changes in adherence to IPC practices during routine care and aerosol-gener

Tuberculosis (TB) is the most common cause of fatal infections worldwide. Recent TB figures in Europe indicate that 30 people were infected with tuberculosis each hour in 2017. Healthcare workers are at particular risk of being infected through patient contact. TB is the second most common occupational infectious disease among German healthcare workers. Routine data from the German Social Accident Insurance were used to examine trends in occupational TB diseases. We analyzed annual cross-sectional data for the years 2002 to 2017. The data underwent descriptive analysis. A total of 4653 TB case

OBJECTIVES: This study aimed to evaluate the process of implementation of a peer coaching intervention program for occupational physicians (OPs) to improve the execution of preventive tasks. Specifically, the evaluation seeks to: (1) describe the reach and uptake of the intervention program; (2) determine the extent to which the program was implemented as intended; (3) provide insights into experiences of OPs, and (4) identify factors influencing the implementation.
METHODS: This study employed a mixed-methods design. To address the four research aims, seven process indicators were used: acce

Good physical and mental health is a prerequisite for anyone planning to scuba dive. A certificate of fitness to dive for those willing to enter a scuba diving course as well as for active divers, either amateur or occupational, can only be issued if there are no medical contraindications to dive. It is usually within the competence of a diving instructor, a manager of underwater work or a physician to assess a person's mental and physical health and grant them permission to stay under hyperbaric conditions. The legal requirements for issuing a certificate of fitness to dive are different for

A bus driver came to our observation after an occupational traffic accident due to a syncopal event. The positive result of the tilt testing demonstrated the neurally-mediated nature of the syncope. The accident involved approximately 40 people (all the bus passengers), fortunately without severe injuries or deaths. The described episode indicates the need for a procedural algorithm, commonly approved, applicable in the field of prevention, for those occupational categories with severe accident risk. Indeed, the possibility exists to identify at least a part of the subjects predisposed to neur

Chronic ill-health is a major driver of premature exit from employment. While the annual incidence of ill-health-related job loss in the French workforce is estimated at 7.8 per 1,000 workers, the specific burden among cleaners remains unknown. This study aimed to quantify the incidence of medical unfitness among cleaners in France and to identify the principal diseases and demographic factors associated with job loss. A 4-year follow-up study (2016-2019) was conducted within the occupational health service of Montpellier, France. All employees deemed medically fit at baseline were included. D
