Traumatic posterior atlantoaxial dislocation without related fracture of the odontoid process is very rare, and only ten cases have been previously reported. The objective of this paper was to describe a case of traumatic posterior atlantoaxial dislocation without related fracture of the odontoid process, and its management with atlantoaxial transarticular screw fixation and bony fusion through an anterior retropharyngeal approach, and to review the relevant literature. The patient's medical and radiographic history is reviewed as well as the relevant medical literature. Posterior atlantoaxial

There are five types of odontoid anomalies: os odontoideum, ossiculum terminale, hypoplasia of the odontoid, aplasia of the odontoid base and aplasia of the odontoid. All five anomalies may lead to atlantoaxial instability. Their clinical signs, symptoms and treatments are identical. Only radiographic findings are distinctive. The incidence of odontoid anomalies is uncertain. A 15-year-old boy was hospitalized with progressive numbness of his limbs on the right side for one month. Os odontoideum was confirmed by cervical radiography and magnetic resonance imaging. The lesion was stabilized wit

OBJECTIVE: This study aimed to analyze a clinical series of odontoid fractures treated with true percutaneous anterior odontoid screws. The technique we used is different from the traditional approach of using blunt dissection to arrive at the anterior surface of the vertebral body.
METHODS: Between 2004 and 2021, forty-two patients underwent anterior screw fixation of the odontoid process using the true percutaneous technique without dissection among the fascial planes. Instead, we pressed from the medial border of the sternocleidomastoideus muscle with our second and third fingers to feel t

The classical operative approaches to the cervical spine include the posterior one and the anterior exposure along the sternomastoid muscle. However, neither of these are helpful in exposing the upper cervical vertebrae, especially the odontoid process, atlas, and axis. We have used the transoral-transpharyngeal exposure for lesions of the odontoid process and upper (first to third) cervical vertebrae in six patients. The pathologic processes included rheumatoid disease and fracture of the cervical vertebrae, suspected tumor with compression of the spinal cord, basilar invagination, and compre

BACKGROUND: Unstable osteolytic lesions of the occipitocervical junction are rare and may occur in hematological malignancy or vertebral hemangioma, among others. Different case reports have been published about vertebroplasty for treatment of spinal metastases of the upper cervical spine. Only few cases concern balloon kyphoplasty of C2. We present a consecutive case series including four patients with an osteolytic lesion of the dens axis and describe a technical note for balloon kyphoplasty of C2 and an additional anterior odontoid screw fixation.
METHODS: Four consecutive patients with an

Cervical spine involvement by rheumatoid arthritis is common; brain-stem compression secondary to vertical subluxation of the odontoid in patients with rheumatoid arthritis is rare. Vertical subluxation results from 1) destruction of the transverse atlantal, apical, and alar ligaments of the atlas and odontoid, and 2) bone resorption in the occipital condyles, lateral masses of the atlas, and basilar processes of the skull. Neurological symptoms result from direct compression of the brain stem or from ischemia secondary to compression of vertebral arteries, anterior spinal arteries, or small p

One patient with the chief complaint of acute torticollis was diagnosed as atlantoaxial rotatory instability secondary to hypoplasia of odontoid process. The diagnosis was confirmed by cervical radiography and computerized tomography. The attack of wryneck was first misdiagnosed as cervical muscle strain. Detailed physical examination revealed the tenderness to be located not at the sternocleidomastoid muscle. The fixed and painful rotatory deformity strongly indicated a cervical spinal lesion. Plain radiographic tests of the cervical spine revealed atlantoaxial rotational subluxation. Hypopla

A gentle traction is often sufficient to repose unstable lesions of the cervical spine, when a retention by means of conservative methods is not possible. Therefore a ventral, intracorporal fusion with autologous, corticospongious chip material combined with a plate fixation is an adequate solution to obtain a stable, movable blocking of the lower six cervical vertebrae. In case of a completely ruptured dorsal ligamentous apparatus, a ventral bone-chip blocking is generally insufficient. Ventral spondylodesis should be combined with a dorsal traction belt in these cases. Fresh, instable fractu

Anterior screw fixation is a well-recognized technique that is used to stabilize Type IIB fractures of the odontoid process in the elderly. However, advanced age and osteoporosis are 2 risk factors for pseudarthrosis. Kyphoplasty has been described in the treatment of lytic lesions in C-2. The authors decided to combine these 2 techniques in the treatment of unstable fractures of the odontoid. Two approximately 90-year-old patients were treated for this type of fracture. Instability was demonstrated on dynamic radiography in one patient, and the fracture was seen on static radiography in the o

The study was undertaken to improve an approach to treating patients with fractures of the odontoid of the C2 vertebra. Forty-nine patients were admitted to hospital for fractures of the odontoid process on January 14, 1989 to January 3, 2004. Physical, target X-ray study of the cervical spine, computed tomography of the C1-2 vertebrae, magnetic resonance imaging of the neck were performed on admission. Type II odontoid fractures were found in 17 patients, Type III in 32, of them 5 patients were observed to have an axial fracture concurrent with Jefferson's fracture. Forty-one of the 49 patien

INTRODUCTION AND IMPORTANCE: Fractures of the second cervical vertebrae (C2) and its odontoid process account for one of the most frequent cervical spine injuries that cause significant mortality and morbidity. The goal of injury treatment is to restore atlantoaxial complex stability. This article reports a young male patient aged 19 years who underwent surgical treatment due to odontoid fractures type III.
CASE PRESENTATION: A 19-year-old male patient came with a chief complaint of weakness in the upper and lower extremities for 3 weeks before admission. The patient underwent a series of phy

BACKGROUND/AIM: Craniocervical junction (CCJ) can be involved in inflammatory arthritis. We aimed to define types of CCJ involvement in rheumatoid arthritis (RA), spondyloarthritis (SpA), and psoriatic arthritis (PsA) and compare them with patients without inflammatory arthritides.
MATERIALS AND METHODS: In this retrospective analysis, cervical CT or MRIs of patients with RA, SpA, or PsA, taken for any reason between 2010 and 2020, according to ICD-10 codes, were scanned. Demographic data of the patients were recorded. CCJ involvements (atlantoaxial, vertical, or subaxial subluxation, odontoi

OBJECT The endoscopic endonasal approach (EEA) has been established as an alternative approach to craniovertebral junction (CVJ) pathology in adults. The authors have previously described the nasoaxial line (NAxL) as an accurate predictor of the lower limit of the EEA to the CVJ in adults. The surgical anatomy limiting the EEA to the pediatric CVJ has not been well studied. Furthermore, predicting the lower limit of the EEA in various pediatric age groups is important in surgical planning. To better understand the anatomy affecting the EEA to the CVJ, the authors examined the skull base anatom

Associated fractures of the atlas and axis are frequent, particularly in the elderly patients following a simple low-energy fall. This injury can be easily misdiagnosed on initial plain radiographs, and therefore computed tomography scan is a useful adjunct in patients with a degenerative spine. There is still no consensus as to the optimal treatment of combined axis-atlas fractures, and the majority of authors propose a therapeutic strategy dependent on the odontoid fracture pattern. We describe a combined atlas and axis fracture in a 92-year-old patient who was managed with nonoperative trea

BACKGROUND: Retro-odontoid pseudotumors (ROPs) typically present as a single mass posterior to the odontoid ranging from isointense to hypointense relative on T1-weighted magnetic resonance (MR) imaging (MRI T1WI). Here, a patient with ROP exhibited the double-layer sign on the MRI T1WI characterized by an initial ventral layer posterior to the odontoid process followed by a secondary dorsal layer.
CASE DESCRIPTION: An 84-year-old male presented with cervical myelopathy attributed to ROP resulting in atlantoaxial instability on dynamic X-ray studies, and the double-layer sign on the T1 MR acc

BACKGROUND: We report the first case of retro-odontoid pseudotumor with an isolated symptom of C2-C3 dysesthesia triggered by a traumatic event.
CASE PRESENTATION: An 86-year-old man, who was a wood craftsman for more than 50 years, presented to the emergency room with sudden-onset severe posterior head and neck dysesthesia after accidentally falling backward. No neurological impairment of the extremities was noted. Computed tomography revealed a C2-C7 osteophyte formation, mainly in front of the vertebral bodies. Moreover, magnetic resonance imaging showed cervical spinal cord compression by
