Off-label use of muscular VSD device for closure of a rare congenital portosystemic shunt.

BACKGROUND: Congenital portosystemic shunt (CPSS) is a vascular malformation in which portal blood drains toward the systemic circulation, leading to pulmonary hypertension. CASE PRESENTATION: A 10-year-old patient was brought for evaluation because of dyspnea on exertion. Echocardiography revealed a pulmonary hypertension of 75 mmHg, and multi-slice CT angiography revealed the presence of a CPSS. Closure was finally implemented using a muscular ventricular septal defect device. Follow-up of the patient revealed a gradual decline in pulmonary hypertension. CONCLUSIONS: CPSS is an overlooked

[Off-label use of recombinant factor VII (rFVIIa) in teaching hospitals in Paris in 2010].

INTRODUCTION: Recombinant activated factor VII (rFVIIa) (Novoseven(®)) was initially developed as a substitutive treatment in haemophiliacs but has then been used in situations of major haemorrhage in non-haemophiliacs (off-label use). The goal of the present study was to assess the practice patterns when rFVIIa is used in off-label indications in major teaching hospitals of Paris in 2010. METHODS: We retrospectively identified files of patients in whom rFVIIa had been used. Physicians in charge of these patients (or the most proxy physician available) were contacted and files analysed with o

Off-label use of antipsychotic medications in the department of Veterans Affairs health care system.

OBJECTIVE: This study aimed to determine the prevalence of prescribing antipsychotics to adults without schizophrenia or bipolar disorder and to identify factors associated with such off-label use. METHODS: Patients with at least one prescription for an antipsychotic medication from the Department of Veterans Affairs (VA) during fiscal year (FY) 2007 were identified in national VA administrative databases. Rates of off-label antipsychotic use were determined along with average doses. Multivariate logistic regression models identified sociodemographic and clinical characteristics associated wi

Unlicensed and off label drug use in acute lymphoblastic leukaemia and other malignancies in children.

BACKGROUND: The need to use unlicensed and off label drugs in children due to the lack of suitable, licensed formulations with appropriate prescribing information creates many problems on an everyday basis for healthcare professionals, carers and children. PATIENTS AND METHODS: This prospective study was designed to examine the incidence and nature of unlicensed and off label prescribing, in paediatric oncology patients with acute lymphoblastic leukaemia and other malignancies. Inpatient and outpatient prescriptions were analysed for a 4-week period. RESULTS: All patients received at least o

Associations between off-label feed additives and farm size, veterinary consultant use, and animal age.

Data from the United States National Swine Survey collected by the National Animal Health Monitoring System were used to describe the use of feed additives in swine feeds. Data were collected from 710 farms. The concentration of feed additives expressed in grams per ton of complete feed was described by stage of production, and the use of feed additives above the labeled treatment levels (i.e. off-label) was identified. Of the 3328 feeds, about 79% contained feed additives used in the labeled manner. For all classes of pigs, the prevalence of labeled feed additive use was greater than 75%. Pen

A physician's prerogative to prescribe drugs for off-label uses during pregnancy.

Physicians frequently prescribe drugs for indications other than those on the product label. Reasons for such off-label use during pregnancy include: prevention of repetitive abortion, inhibition of premature labor, reduction of fetal or neonatal infection, reduction in development of preeclampsia and its complications, and ripening of the cervix or induction of labor. A physician has a legal right to prescribe for off-label indications despite regulatory, manufacturer, and cost constraints. Such prescribing habits would not be considered experimental if based on sound scientific evidence. Ade

Out of the spasticity box: Off-label uses of botulinum toxin in children.

Botulinum toxin is frequently used as a therapeutic for a variety of non-FDA approved indications in children. This narrative literature review explores three off-label uses of botulinum toxin in pediatric conditions including congenital muscular torticollis, spastic trismus, and neonatal brachial plexus palsy. While more research is needed to establish treatment, dosing and localization guidelines for the use of botulinum toxin in these commonly treated conditions, available evidence is discussed.

Prescribed medicine use and extent of off-label use according to age in a nationwide sample of Australian children.

BACKGROUND: Medicine prescribing for children is impacted by a lack of paediatric-specific dosing, efficacy and safety data for many medicines. OBJECTIVES: To estimate the prevalence of medicine use among children and the rate of 'off-label' prescribing according to age at dispensing. METHODS: We used population-wide primarily outpatient dispensing claims data for 15% of Australian children (0-17 years), 2013-2017 (n = 840,190). We estimated prescribed medicine use and 'off-label' medicine use according to the child's age (<1 year, 1-5 years, 6-11 years, 12-17 years) defined as medicines wit

Comparison between on-label versus off-label use of drug-eluting coronary stents in clinical practice: results from the German DES.DE-Registry.

BACKGROUND: Observational studies from the USA have demonstrated that off-label use of drug-eluting stents (DES) is common. Data on off-label use in Western Europe are limited. METHODS: We analyzed the data of consecutive patients receiving DES prospectively enrolled in the multicenter German DES.DE registry (Deutsches Drug-Eluting Stent Register) between October 2005 and October 2006. Off-label use was defined in the presence of one of the following criteria: ST-elevation myocardial infarction, in-stent stenosis, chronic total occlusion, lesions in a bypass graft, in bifurcation or left main

Off-label use of dapagliflozin in a dog with recurrent calcium oxalate urolithiasis: a case report.

BACKGROUND: Calcium oxalate (CaOx) urolithiasis is a common and recurrent condition in dogs, typically managed with dietary modification, thiazide diuretics, and potassium citrate. In human medicine, sodium-glucose cotransporter 2 (SGLT2) inhibitors such as dapagliflozin are increasingly used to manage diabetes mellitus and chronic kidney disease and have been shown to alter urinary factors associated with nephrolithiasis. Their effects in veterinary patients, however, remain uncharacterized. This case report describes the novel off-label use of dapagliflozin in a dog with refractory CaOx cyst

The Off-Label Use of Selective Serotonin Reuptake Inhibitors for Sexual Behavior Management: Risks and Considerations.

BACKGROUND: Selective serotonin reuptake inhibitors (SSRIs) are one of the most frequently used medication classes in psychiatry, with many approved and off-label uses. One common side effect of SSRIs is sexual dysfunction, leading to the off-label use of SSRIs to manage inappropriate sexual behaviors in psychiatric settings. However, no official guidelines exist for this off-label use of SSRIs, so a review of this use is warranted. METHODS: This review was conducted using the PubMed and Google Scholar databases. Grey literature was considered for inclusion in this review, but only one report

Off-label use of alemtuzumab, belatacept, and rapamycin in kidney transplantation-Long-term real-world experience.

Conventional immunosuppressive regimens in kidney transplantation continue to pose significant challenges, largely due to their reliance on calcineurin inhibitors (CNIs) and corticosteroids. These challenges have driven the search for alternative regimens to minimize toxicity and preserve long-term graft function. In this study, we present real-world data utilizing a CNI-free regimen that makes use of the synergistic effects of costimulation blockade and mTOR inhibition. We retrospectively analyzed 106 kidney recipients from 2016 to 2024 who received alemtuzumab induction, followed by de novo
