[Rhinogenous inflammatory diseases of the orbit].

Patients of young age predominated in the group of 113 patients (69 males, 44 females) with orbital rhinogenic inflammation observed by the authors. 57.5% of the cases arose due to acute sinusitis, 42.5%-due to exacerbation of chronic sinusitis. The inflammation focus developed more frequently in the frontal sinuses and ethmoid labyrinth. Periostitis was diagnosed in 17 patients, subperiosteal orbital abscess in 9 patients. Inflammation of the orbital soft tissues presented as eyelid and orbital fat edema in 66 patients, as retrobulbar abscess in 11 and phlegmon in 10 patients. The treatment p

Primary extramedullary plasmacytoma of the orbit.

BACKGROUND: We report a case of extramedullary plasmacytoma of the orbit. This is a rare tumour which has uncommonly presented in the orbit. CASE REPORT: A 58-year-old woman presented with painless proptosis, upper lid swelling and ptosis. Incisional biopsy was performed. Clinical findings and investigations were correlated to establish the diagnosis. Treatment consisted of 35 Gy external beam radiotherapy. CONCLUSIONS: This condition is considered to be of low grade malignancy and as such can be regarded as a separate entity to multiple myeloma involving the orbit or solitary myeloma of bon

Color Doppler US of the orbit.

Color Doppler imaging of the orbit shows promise in evaluation of several types of orbital abnormalities. The most promising use is for evaluation of vascular disorders. Screening for orbital varices and cavernous-carotid fistulas is easily accomplished. Color Doppler imaging can also be useful in diagnosis of ocular ischemia, central retinal vessel occlusion, and anterior ischemic optic neuropathy. In cases of inflammatory disease, it may be helpful in searching for complications of infection and in monitoring the size of fluid collections. In cases of tumors, color Doppler imaging can be use

Magnetic resonance imaging of the orbit.

Magnetic resonance (MR) imaging plays an important role in evaluation of ocular and orbital lesions. Ocular tumors can be distinguished and delineated from adjacent fluid collections. Demonstration of internal septa within a well-defined mass leads to the diagnosis of cavernous hemangioma, whereas vascularity and internal lobulations are the hallmark of capillary hemangiomas. MR is the procedure of choice for evaluation of the optic nerve sheath complex. It precisely delineates and characterizes optic neuritis, optic gliomas, and perioptic meningiomas.

Inflammatory sinus diseases affecting the orbit.

Paranasal infections and inflammatory processes involving the orbits have been described, as have primary orbital conditions that may mimic acute sinus disease. Most common are bacterial and fungal sinus infections with secondary orbital spread, diseases for which the otolaryngologist-head and neck surgeon must be skilled in diagnosis and management in order to minimize sequelae. Optic neuritis arising from such infections requires prompt recognition and aggressive treatment if vision is to be preserved. The most common causes of unilateral proptosis, Graves' orbitopathy, and pseudotumor may i

Primary malignant ectomesenchymoma of the orbit.

Malignant ectomesenchymoma (MEM) is a rare soft tissue tumour believed to arise from a pluripotential migratory neural crest cell and composed of both a mesenchymal element (most often rhabdomyosarcoma) and a neuroectodermal element (often neuroblastoma). Reported sites of origin are the abdomen, perineum or scrotum, the extremities, the middle ear, nasopharynx, face, and neck. We report the first case of an orbital MEM, with a review of the 17 cases previously reported from other sites.

Mycosis fungoides metastatic to the orbit.

We report a case of mycosis fungoides metastatic to the anterior orbit in an 83-year-old woman. The patient had a history of several years' duration of well-controlled primary mycosis fungoides with localized dermatologic T-cell lymphomas, mainly on the lower extremities. She developed gradual proptosis, hyperophthalmos, and vertical diplopia. Computed tomography demonstrated a solid, extraconal mass in the right lower anterior orbit. Biopsy revealed a gristly, yellowish, infiltrating mass at the level of the inferior orbital septum. Pathologic examination revealed anaplastic T lymphocytes wit

Maxillary swing approach to the orbit.

A new surgical approach to the inferior half of the orbit is described and 2 cases are presented. The approach involves releasing the roof and the anterior wall of the maxillary antrum by osteotomies, keeping their blood supply via the attached masseter muscle, and swinging the osteoplastic flap away from the orbit. With the wide exposure gained, the retrobulbar tumor or large orbital floor tumor is dealt with, and the mobilized bone is replaced and wired into position. There is no postoperative disturbance of cosmetic appearance.

T-cell lymphoma of the orbit.

In a 52-year-old man with drooping of the right upper eyelid and a palpable mass in the superior temporal quadrant of the right orbit, a biopsy specimen revealed diffuse mixed-type lymphoma with immunostaining evidence of a helper T-cell phenotype. No other foci of lymphoma were found. The orbital lesion was treated with irradiation (total dose, 4,200 cGy administered in 20 treatments). At 4 1/2 years after treatment, the patient had had no recurrence.

Blow-in fractures of the orbit.

A blow-in fracture is an inwardly displaced fracture of the orbital rim or wall resulting in decreased orbital volume. The purpose of this study is to classify orbital blow-in fractures, describe the distinguishing clinical and radiologic features, and review the result of treatment. The series consists of 41 patients with blow-in fractures (34 males and 7 females). The mean age of the patients was 36 years. All were treated between 1979 and December of 1986 at Sunnybrook Medical Centre in Toronto. Clinical features of blow-in fractures were primarily related to the decrease in volume of the o

Myxoma of the skull and orbit.

An unusual case of myxoma of the skull extending into the orbital cavity is reported. Myxoma is a benign tumour of the soft tissues and maxillary bones, which often presents infiltrating growth and recurrences. Histologically it is characterized by abundant myxoid stroma and is often confused with other tumours which can present mucoid degeneration. Although diagnosis is impossible before histological examination, the possibility of a myxoma must be considered in the presence of extradural osteolytic poorly vascularized lesions.

Aneurysmal bone cyst of the orbit.

Proptosis of the left eye had developed in a 42-year-old woman for 2 months' duration. Roentgenogram studies showed a mass in the roof of the left orbit and bone erosion. An excisional biopsy confirmed the presence of an aneurysmal bone cyst. To the authors' knowledge, the patient is the oldest reported patient who has this uncommon orbital lesion.

Breast carcinoma metastatic to the orbit.

Breast carcinoma metastatic to the orbit presented in four patients as a diffuse mass lesion. Clinical findings included restricted ocular motility, palpable mass, enophthalmos, and ptosis. The delay in diagnosis in three cases was due to subtle early findings, the long time interval from the primary breast lesion, lack of other metastases, and, in each of the four cases, the patient's reluctance to disclose any history of breast disease. Pathologic examination of the orbital breast metastases revealed two types: an adenocarcinomatous pattern with nests of pleomorphic malignant appearing cells

Intraosseous hemophilic pseudotumor of the orbit.

Intraosseous hemophilic pseudotumor involving the orbit has not been described previously. A 14-year-old white male presented with a rapidly progressive, expansile right orbital mass consisting of hematoma contained within bony cortex. Computerized tomography revealed findings consistent with intraosseous hemophilic pseudotumor. The patient responded to conservative therapy of factor VIII replacement.

Cemento-ossifying fibroma of the orbit.

Cemento-ossifying fibroma presents with ophthalmic symptoms and signs rarely. We report the clinical and pathological findings in a case of cemento-ossifying fibroma of the right maxilla with extension into the orbital floor causing intermittent vertical diplopia, proptosis, and upward displacement of the globe. Compression of the nasolacrimal duct produced epiphora early in the course of the disease. Fibrous dysplasia has often been diagnosed in other cases of benign monostotic fibro-osseous conditions. Ossifying fibroma is easily confused with fibrous dysplasia. The histopathological differe

Posttraumatic vascular malformations of the orbit.

The close association of the external and internal carotid circulations in the orbital cavity is well recognized. Two cases are presented in which local trauma to the orbit apparently resulted in the formation of an arteriovenous fistula. These lesions progressively expanded, producing classical features of proptosis, chemosis, and bruit. The investigation by selective angiograms and the surgical management of these malformations are discussed. A further case is presented in which an early lesion was noted in angiography after a head injury, with subsequent disappearance of the lesion some wee

Amyloidosis of the orbit and adnexae.

Five patients representing a spectrum of orbital and adnexal amyloidosis are presented. One patient with conjunctival amyloidosis did not have antecedent infection nor systemic disease (primary localized amyloidosis), while another patient with conjunctival amyloidosis had a long history of repeated attacks of bacterial conjunctivitis (secondary localized amyloidosis). The third patient presented with serial bilateral lacrimal gland amyloid without systemic disease (primary localized orbital amyloidosis). Another patient had intraorbital amyloid associated with metastatic deposits or immunoglo
