OBJECTIVE: The operative correction of scoliosis requires multiple intraoperative techniques and tools to achieve an adequate result. Frequently, multiple methods are used to accomplish this, such as rod cantilever techniques, in situ bending, Smith-Petersen and pedicle subtraction osteotomies, closed reduction methods, and rod derotation techniques. Rod derotation techniques will be reviewed and discussed in this article.
METHODS: A review of the available literature on anterior and posterior rod derotation is performed with a case example of the authors' experience utilizing this technique.

UNLABELLED: Osteochondritis Dissecans (OCD) is a condition for which the aetiology remains unknown. It affects subchondral bone and secondarily its overlying cartilage and is mostly found in the knee. It can occur in adults, but is generally identified when growth remains, when it is referred to as juvenile OCD. As the condition progresses, the affected subchondral bone separates from adjacent healthy bone, and can lead to demarcation and separation of its associated articular cartilage. Any symptoms which arise relate to the stage of the disease. Early disease without separation of the lesion

Patients with primary sarcomas of bone and soft tissue are generally treated on protocol by a multidisciplinary team at tertiary referral centers. Conversely, patients with metastatic disease to the skeleton are often evaluated by general orthopaedic surgeons in the community. Skeletal metastases affect a growing percentage of the elderly population and can contribute to a marked decrease in their quality of life. Treatment includes stabilization with the latest prosthetic implants and new devices. Advances in the field of radiation oncology and bisphosphonate use in patients with bone metasta

Gordon's Syndrome is described as an autosomal dominant condition with the characteristics of short stature, a stiff spine, camptodactyly (89%), cleft palate (27%) and club feet (73%). The authors present a case report of a patient with this rare entity complicated by an unusual complex spinal deformity. There are no prior reports in the literature concerning operative or nonoperative management of deformity in this patient population. Scoliosis in Gordon's Syndrome shares the characteristics of an arthrogrypotic neuromuscular curve and demands extensive soft tissue release for optimal correct

Navigation procedures in orthopedic surgery require fixation of reference markers to the anatomic region of interest. Inadequate fixation might lead to micromotion or loosening of the reference marker, consequently causing registration failures or errors in navigation. Osseous rigid fixation is usually achieved by minimally invasive Schanz screws or pins. The goal of this study was to evaluate a non invasive external fixation device, a headband so far used in cranial navigation, as an alternative invasive fixation technique to reference markers in the femur. A common navigation system with an

No consensus had been reached about the optimal treatment for syndesmotic fixation. We performed a review of randomized controlled trials (RCTs) to assist in clarifying many of the controversies that were debated for years despite new technology and options. PubMed, Embase, and the Cochrane Library were searched through specific terms and limits. Only RCTs were selected for final inclusion. Study screening and data extraction were performed independently by 2 reviewers. Thirteen RCTs, including 828 cases, met the inclusion criteria and are summarized in this review. Pooled results demonstrated

STUDY DESIGN: Systematic review.
OBJECTIVE: To identify and describe reconstruction methods for the treatment of transverse sacral fracture (TSF) and to evaluate outcomes based on treatment interventions.
SUMMARY OF BACKGROUND DATA: A variety of surgical interventions for stabilization of TSFs exist, yet the optimal management remains unclear. Although there are many individual case reports and series describing techniques to stabilize TSF, prior reviews fail to provide a comprehensive summary of current and past surgical techniques and their individual outcomes.
METHODS: Our systematic rev

The results of treatment are compared in three groups of patients treated by various methods of osteosynthesis. In group 1 (n = 203) osteosynthesis was carried out using wire suture. In group 2 (n = 231) titanic miniplates and screws were used. In group 3 (n = 21) combined methods were used (simultaneous fixation by wire sutures and titanium miniplates). Indications to the use of different osteosynthesis methods are defined, errors and complications in osteosynthesis in the upper and middle zones of the face are analyzed.

Osteosynthesis with fixateur externe is indicated in unstable, infected fractures, infected pseudarthroses and defect pseudarthroses, second-degree and third-degree open fractures as well as closed or unstable old fractures and pseudarthroses with marked damage of the soft parts, as well as in all arthrodeses of the knee joint or ankle joint (foot arthrodeses). The main field of application is the shaft of the lower leg with its adjacent joints. To achieve satisfactory stability, mounting should always be three-dimensional and numerous additional details must be taken care of, all of which are

Besides the methods of screwing of plates and medullary nailing, the fixateur externe (F.E.) is another method which can be considered for treating bone fractures at the femur, F.E. being an exceptional indication. In fresh fractures of the femur, open fractures of the second and third grade as well as debris and multiple-plane fractures are typical indications for F.E. Over and above, F.E. is almost always indicated as an emergency measure in case of initial inoperability (polytrauma, shock, loss of blood, craniocerebral trauma) or continuing general inoperability. F.E. has been found useful

Percutaneous K-wire fixation is still a useful technique for closed oblique phalangeal and meta-carpal fractures when an adequate closed reduction can be achieved. Lag screw fixation may be the best choice for open fixation of long oblique phalangeal and metacarpal fractures. For short oblique fractures, plating or tension band wiring is recommended. Plating provide rigid fixation to allow early mobilization; however, one may encounter frequent complications such as extensor lag, stiffness, or joint contracture when plating technique is used in phalangeal fractures. Tension band wiring techniq

Patients with common hand fractures are likely to present in a wide variety of outpatient orthopedic practices. Successful rehabilitation of hand fractures addresses the need to (1) maintain fracture stability for bone healing, (2) introduce soft tissue mobilization for soft tissue integrity, and (3) remodel any restrictive scar from injury or surgery. It is important to recognize the intimate relationship of these 3 tissues (bone, soft tissue, and scar) when treating hand fractures. Fracture terminology precisely defines fracture type, location, and management strategy for hand fractures. The

STUDY DESIGN: To provide a technical report on a new device. Early outcome assessment of patients undergoing anterior cervical decompression and stabilization with the described technique.
OBJECTIVES: To detail the operative and technical aspects of the new anterior cervical instrumentation and to identify its suitability for an extensive clinical study.
SUMMARY OF BACKGROUND DATA: The controversy over the need for fusion in the surgery of cervical disc disease is in part fueled by the absence of an ideal technique. However, the design of the integrated anterior cervical plate and cage devic

Unilateral configurations with stiffness characteristics similar to those of the most rigid one- and two-plane bilateral constructs are easily built. Mechanically, most effective were two-plane unilateral frames with a short bone-rod distance. But single- and double-bar anterior unilateral frames had the best combination of clinical and mechanical features. To put gradual frame builddown on a rational basis, a sequence of stepwise frame reduction was developed. This sequence leads from the most rigid unilateral constructs to frames that allow almost complete force transmission across the fract

Since closed treatment is effective in 80% to 85% of distal femoral fractures, operative intervention is not recommended. Unnecessary surgery produces unwarranted complications. The cast-brace technique should be used in the first week or two after injury to prevent deformities commonly associated with distal femoral fractures. The thigh cast should be carefully molded well over the femoral condyles. Distal femoral traction is preferred to proximal tibial traction to correct fracture deformities. If the closed reduction technique is not satisfactory in the early period after injury, operative

Surgery in metastatic bone disease is currently the most frequent type of surgery in orthopedic oncology. Improved survival rates and an increasing incidence of bone metastasis have led to an increase in complications caused by metastatic disease, such as pathological fractures or hardware failure after operative treatment. Although surgery of metastatic lesions remains a palliative therapy concept, because of sufficient therapy for the primary carcinomas, tumor-specific-oriented follow-up protocols and a variation in the prognosis for the individual entities, an individually adapted treatment
