Optimal Position and Orientation of an Ossicular Accelerometer for Human Auditory Prostheses.

In this study, a method for determining the optimal location and orientation of an implantable piezoelectric accelerometer on the short process of the incus is presented. The accelerometer is intended to be used as a replacement for an external microphone to enable totally implantable auditory prostheses. The optimal orientation of the sensor and the best attachment point are determined based on two criteria-maximum pressure sensitivity sum and minimum loudness level sum. The best location is determined to be near the incudomalleolar joint. We find that the angular orientation of the sensor is

Ossiculoplasty in Congenital Aural Atresia.

The need for ossiculoplasty in the repair of congenital aural atresia (CAA) is rare, occurring approximately 5% to 8% of patients. Predicting the need for ossiculoplasty in CAA repair based on high resolution computed tomography imaging is challenging. The technique is similar to traditional ossiculoplasty methods, with a prosthesis placed onto a mobile stapes bone extending up to a reconstructed tympanic membrane. Hearing results can be variable, but are, in general, inferior to reconstruction with the patient's native ossicular chain. This article describes the indications, technique, and su

From the laboratory concept to clinical trials: the journey of Otoimplant so far.

Nowadays, the role of alloplastic prostheses has become more important in ossicular chain reconstruction. Especially, bioactive medical devices may be an alternative for those currently used on the market. Areas covered: Medical devices which are implanted for over 30 days in the body are required to fulfill strict requirements. It is crucial to ensure the proper level of biocompatibility, biological stability and lack of cytotoxicity of the implanted material. If a medical device is expected to play an additional role, e.g. act as a bioactive or bactericidal agent, supplementary tests should

Incudostapedial rebridging ossiculoplasty with glass ionomer cement: a case report.

A 31-year-old woman presented with a complaint of hearing loss of a two-year history. Otoscopic examination revealed a central ear drum perforation without discharge. The air-bone gap was 35 dB at 0.5, 1, 2, and 3 kHz. Under general anesthesia, the patient underwent tympanoplasty for chronic otitis media. At surgery, the ossicular chain was intact except for discontinuity by a 2-mm gap between the long process of the incus and the head of the stapes. The gap was reconstructed with the use of a fluoroplastic prosthesis. Myringoplasty was successful, but the patient complained of hearing loss at

Clinical significance of stapedioplasty biomechanics: swimming, diving, flying after stapes surgery.

A piston prosthesis in stapedioplasty significantly modifies the function of the normal ossicular chain. Due to the fact that the ear works as a pressure receptor, a piston prosthesis will be displaced at ambient air pressure changes in a different way than the normal stapes. Our ear is constantly exposed to these pressure changes in daily live, for example during swallowing, with tubal opening, with wind gusts at the external ear, during flying, or diving. Temporal bone experiments showed that elevated static pressures, like in tympanometry, can displace a piston up to 0.5mm in the vestibule.

Characterization of mechanical behaviour of healthy and injured human incus by eigenfrequency evaluation.

The usage of finite element method techniques gives a possibility to replace time-consuming experiments or imitate physical process in the ear by numerical simulation. Especially, the research of spatial motion of ossicular chain in the middle ear is of high interest for the oto-surgeons and engineers. It is known that the most affected bone from the ossicular chain is the incus. After the cholesteatoma operation and tympanoplasty, the affected incus is removed or sacrificed; thus, the possibility of transducing noise lays on the stapes, new titanium or other material prosthesis. In this case,

[High resolution computerized tomography in the study of traumatic pathology of the temporal bone].

Thirty-five patients with temporal bone fractures were examined; the fractures were sometimes associated with dislocation of the ossicular chain caused by road fatalities. Computed Tomography (CT) was performed either because of the presence of clinical symptoms associated with trauma of the temporal bone, or because of a hemotympanum discovered during a CT scan of the brain. Thirty-three fractures were detected: 19 longitudinal, 6 transverse, and 8 complex. An incudostapedial dislocation was also detected, together with a displacement of a stapedial prosthesis from the lenticular process of i
