Does the scutum of the external auditory canal have a role in the stapedotomy operation? A radio-clinical evaluation.

OBJECTIVE: This study aimed to assess the impact of the scutum on oval window accessibility during stapedotomy surgery. Radio-clinical correlation was applied to determine the impact. METHODS: This was a prospective observational case-series study that included 127 candidates for stapedotomy surgery. The oval fossa was classified into two types according to the accessibility of the stapes footplate. RESULTS: Both study groups showed a statistically significant difference in pre-operative radiological length as the p value was less than 0.001. The Spearman correlation coefficient revealed tha

[Oval window canal and CT-diagnosis of peripheral pareses of the facial nerve].

Computed tomography (CT) has examined 200 patients (400 temporal bones) at the age of 0-74 with unaffected facial nerve and 28 patients with symptoms of peripheral paresis or paralysis of the facial nerve of different genesis. Polyposition CT of the temporal bone is an intravital noninvasive method of the Fallopius' canal visualization. The canal is visualized on the tomograms in 100% cases irrespective of the patients' age. The following causes of n. facialis paresis were revealed: neurinoma of the facial nerve, sarcoma of the temporal bone, destruction of the tympanic canal wall in chronic o

[Idiopathic perilymph fistula].

This article presents 6 cases with idiopathic perilymph fistulae. The etiology of idiopathic perilymph fistula may be congenital malformation of the otic capule. Diagnosis of idiopathic perilymph fistula primarily depends on history and exploratory tympanotomy, but ENG-fistula test and positional audiometry are worth of diagnosing. During microsurgical procedure, attention must be paid to the presence of membrane of the round window niche. If a good repair of fistula is not followed by a good result, other lesions such as double labyrinth membrane rupture should be considered.

Prosthesis size in stapedectomy.

Primary stapedectomies were performed on 60 patients with bilateral otosclerosis. Every patient had a 4 mm long Robinson prosthesis with a 0.4 mm wide shaft placed in one ear and a 4 mm long Robinson prosthesis with 0.6 mm wide shaft placed in the opposite ear. With the 0.4 mm wide prosthesis, 54 patients overclosed the air bone gap and 6 were within 10 dB of closing. With the 0.6 mm wide prosthesis, which was placed in the opposite ear, 51 patients overclosed their air-bone gap and 8 were within 10 dB of closing. We conclude that there is no statistical difference in hearing results between t

Salivary gland choristoma of the middle ear: report of a case and review of the literature.

Salivary gland choristoma (heterotopic salivary gland tissue) is a rare condition that occurs at various locations within the head and neck. We present a 10-year-old boy with a salivary gland choristoma of the middle ear and compare findings with the 15 similar cases published in the English and German languages. Patients typically have a long-standing conductive hearing loss and visible middle ear mass. Operative findings include a lobulated middle ear mass of histologically normal salivary gland tissue attached posteriorly in the region of the oval window, together with absent or malformed o

Stapedectomy in osteogenesis imperfecta.

Operative findings and results of stapedectomy in 32 patients (43 ears) with osteogenesis imperfecta are presented. The stapes footplates were in many ears thick and of a very soft consistency. In all the operated ears we found the stapes footplates fixated in the oval window. Brittle and atrophic stapes crura were a common finding, but in only 5 ears fractures of the crura were observed. A thick and vascular middle ear mucosa was seen in several ears. Although the middle ear proved quite different from that of otosclerosis, no major difficulties and no serious complications occurred during st

Chronic perilymph fistula in the guinea pig with implications in the human.

To improve understanding of the pathophysiology of perilymph fistulas, a predictable animal model of a chronic fistula was developed. Our findings suggest that guinea pig fistulas do not remain patent for prolonged periods. By extrapolating these findings to humans, we postulate that the symptoms and signs of perilymph fistula are possibly due not to one prolonged constant fistula, but rather a series of "blowouts" from an inherent congenital or posttraumatic weak spot in either the round or oval window. We feel that a diagnosis of perilymph fistula must be considered in any patient presenting

Mucormycosis of the temporal bone.

Mucormycosis of the temporal bone is described in a 60-year-old female with uncontrolled diabetes mellitus whose symptoms related to cranial nerve palsies and hearing loss, following spread of infection from the nasopharynx. The infection spread along the eustachian tube and tensor tympani muscle to the base of the skull, involving the internal carotid artery with mycotic thrombosis and rupture. Subsequent spread occurred from this area predominantly along nerve pathways and as mycotic emboli in blood vessels of the labyrinth and middle ear. Infection also spread from the anterior middle ear w

Temporal bone histopathology of atresia auris congenita with chromosome aberration.

Temporal bone findings in two infants demonstrating congenital aural atresia with microtia were presented focusing mainly on the external and middle ears. These two cases were proved to have chromosome aberration that is, 13-15 trisomy and extra chromosome in group G individually. Temporal bones showed the multiple anomalies through the external, middle and internal ears. The external canals were filled with connective tissue in one case and with bony plate in the other to form aural atresia. There were no development of the tubotympanic recess in a case of 13-15 trisomy. The oval window area

[Differential diagnostic problems in cerebellopontile angle tumors? A case report].

A patient, suffering from otosclerosis of the right ear, was stapedectomized in 1977. Due to bleeding into the oval window during surgery, a persistent deficit of inner ear performance occurred, causing vertigo and hearing loss of high frequencies, in the right ear. As vertigo increased over the years, the patient underwent extensive otological and neurological examinations without result. Because of anamnestic data all interest was focussed on the right ear, and even gas cisternography was performed on the right side only. Due to a left-side sudden hearing loss, the patient came to the Freibu

Suppressed sneezing as a cause of hearing loss and vertigo.

Two cases of inner ear injury caused by suppressed sneezing are described. One patient experienced vestibular symptoms in the form of reflexogenic vertigo that was relieved by surgical section of the tensor tympani tendon. The other patient had a sudden severe permanent sensorineural hearing loss. It is proposed that the aerodynamic pressure increase associated with suppressed sneezing is transmitted via the eustachian tube to cause an implosive fistula of either the round or oval window with injury to the membranous labyrinth.

Intracochlear Drug Injections through the Round Window Membrane: Measures to Improve Drug Retention.

The goal of this study was to develop an appropriate methodology to apply drugs quantitatively to the perilymph of the ear. Intratympanic applications of drugs to the inner ear often result in variable drug levels in the perilymph and can only be used for molecules that readily permeate the round window (RW) membrane. Direct intracochlear and intralabyrinthine application procedures for drugs, genes or cell-based therapies bypass the tight boundaries at the RW, oval window, otic capsule and the blood-labyrinth barrier. However, perforations can release inner ear pressure, allowing cerebrospina

Oval and round window vibroplasty: a comparison of hearing results, risks and failures.

The aim of this study was to compare oval and round window vibroplasty. Eighteen (18) patients implanted with Vibrant Soundbridge (VSB) were enrolled. Two groups were formed depending on FMT placement: on round window in ten cases (RW group) and on oval window in eight (OW group). Pre and postoperative audiological tests were performed both under headphones and free-field settings, VSB on and off. One (1) RW patient experienced sudden hearing loss at the operated side after 4 months from surgery and was excluded from the analysis. Both groups showed good hearing results. Significant difference

Long-term results of a new stapedotomy prosthesis.

The long-term results of a new, home-made stapedotomy prosthesis are presented. The innovations of this prosthesis concern both portions, design for attachment to the incus and shaft. Our series include a total number of 42 individuals with stapes fixation, operated using the new prosthesis, with a medical follow-up period between eighteen months and five years. Our innovations offer a proper and safe insertion of the prosthesis into the oval window associated with excellent manipulation and handling. These advantages could be of major importance for younger surgeons and trainees. Postoperativ

Craniometaphyseal dysplasia: operative findings and treatment.

Craniometaphyseal dysplasia (CMD) is a genetic bone disorder involving an abnormality of modeling of the long bones and sclerosis of the cranium. Both conductive and sensorineural hearing may be frequently associated with this syndrome. Conductive hearing loss is due to attic fixation of the lateral ossicular chain and hyperostosis formation with stapes ankylosis. Two patients with CMD were treated surgically. Two of three ears operated on had closure of the air-bone gap, whereas the third ear failed ossicular reconstruction because of hyperostosis of the promontory with deepening of the oval

Transtympanic gentamycin in the treatment of Ménière's disease.

Severe cases of Ménière's disease are successfully treated with ototoxic antibiotics. Gentamycin sulphate gives the best results. With the aid of a small syringe the drug is introduced into the external auditory canal 3-5 times per day. From there it is transported into the middle ear through a ventilating tube (Grommet) under slight pressure (Politzer bag). The liquid then penetrates the round and oval windows and influences inner-ear function. More than 90% of our patients in 25 years after therapy had no further attacks.

Anatomic Landmarks for Localization of the Vertical Segment of Facial Nerve on Multislice CT Multiplanar Reconstruction Images.

The facial nerve decompression via mastoid is simple and easy to expose the vertical segment of the facial nerve (VFN). The objective of this study was to find out the relationship between the VFN and anatomic landmarks to facilitate prediction of the location of VFN during facial nerve decompression. The different landmarks were cochlear window (CW), oval window (OW), promontorium tympani (PT), and mastoid antrum (MA). Parameters of 140 patients (280 observations) with healthy middle ears were measured on high-resolution spiral multislice computed tomographic multiplanar reconstruction (MPR)
