After a stroke, patients frequently show compromised swallowing, mastication, and speech, as well as unfavorable motion and deviation of the tongue and mandible. The dentist can improve the oral rehabilitation of a patient with deteriorated facial and oral muscles after a stroke by incorporating orofacial myofunctional therapy. This report describes a method for tongue exercises and correction of mandible deviation in an edentulous patient after a stroke by using a pearl on a wire in the anteriomedian palatal part of the maxillary denture.

We describe the sleep evolution of two cases of segmental myoclonus. The first patient had symptomatic palatal myoclonus which, as in most reported cases, persisted during sleep with a slight but significant reduction in frequency. The second patient presented apparently essential spinal myoclonus, which disappeared on falling asleep and recurred for short periods during arousals. This patient also had nocturnal myoclonus involving the legs, as well as those muscles affected by spinal myoclonus. The physiopathological significance of this unusual association is discussed.

Cleft palate (with or without cleft lip) occurs in about 1: 750-1: 2000 births in different societies in the world. Cleft palate individuals have a greater incidence of hearing loss than the general population. The primary cause of the ear problem in cleft patients is eustachian tube dysfunction. The cause for which is abnormal insertion of levator veli palatini and tensor veli palatini muscles into the posterior margin of the hard palate and the palatal aponeurosis and associated muscular hypoplasia.

In an attempt to improve speech results following palate repair while allowing adequate maxillary growth, a palatoplasty using two opposing Z-plasties of the soft palate, one of the oral and one of the nasal layers, has been used in 22 infants. Eight patients had unilateral cleft lip and palate, eight had bilateral cleft lip and palate, and six had cleft palate. The Z-plasties facilitate effective dissection and redirection of the palatal muscles to produce an overlapping muscle sling and lengthen the velum without using tissue from the hard palate, which permits hard palate closure without pu

OBJECTIVE: Cleft palates are strongly associated with the development of otitis media due to the anatomic and functional defect of the soft palate musculature and the associated alterations of velopharyngeal muscle insertion on tubal cartilage, or even intrinsic alterations of the cartilage, which affects eustachian tube function. This study will assess velopharyngeal muscle adequacy after palatoplasty through videonasoendoscopy and verify if there is a correlation with otologic status.
DESIGN: Transversal study.
SETTING: Otorhinolaryngology and cleft palate outpatient service of the Hospita

Fibrosis and defective muscle regeneration can hamper the functional recovery of the soft palate muscles after cleft palate repair. This causes persistent problems in speech, swallowing, and sucking. In vitro culture systems that allow the study of satellite cells (myogenic stem cells) from head muscles are crucial to develop new therapies based on tissue engineering to promote muscle regeneration after surgery. These systems will offer new perspectives for the treatment of cleft palate patients. A protocol for the isolation, culture and differentiation of satellite cells from head muscles is

Contrary to the advocated linear approximation of the muscles in closure of the soft palate we have developed a different method which finds its anatomical confirmation in a publication by Kriens. Since we normally have intermingling vertical and horizontal muscle fibres within the soft palate it seemed logical to copy nature by the surgical technique. We believe that we are better able to reestablish normal anatomical findings by two opposing Z-plastics compared with the methods previously described. With this procedure we are also able to shift the centre of muscle function further dorsally,

We reported a case of 89-year-old woman showing rhythmic skeletal myoclonus mainly on the right upper limb. This myoclonus appeared five days after the cerebral infarction. It was seen constantly both at rest and in posture, and decreased during voluntary movement. When the patient was under emotional stress, it spread to the submandibular, neck and trunks of upper limb. During sleep, this movement completely disappeared. There was no myoclonus in palato-pharyngo-laryngo-oculo-diaphragmatic muscle group. In the examination of the surface electromyography, the movement was not reciprocal betwee

From January of 1992, we applied a combined method to repair cleft palate in 20 patients and received satisfactory results. The method is characterized by pushing back the mucosal flap of the hard palate, a Z-plasty on the nasal mucosa, repositioning the levator muscle to lengthen the palate, circumferential pharyng oplasty using denervated extensor hallucis brevis muscle, without making relaxing incisions and elevating the mucoperiosteal flap, avoiding interference to the greater and lesser palatine vessels and nerves, without relaxing palatal aponeurosis. The advantages of this method are pr

OBJECTIVES/HYPOTHESIS: The soft palate is composed of several muscles. So far, these muscles have been analyzed separately and the whole structure of the soft palate has been described very vaguely. The purpose of this study was to describe the structure of soft palate collectively.
STUDY DESIGN: Human cadaver dissection.
METHODS: Anatomical dissection was performed on 20 Korean, formalin-fixed cadavers (13 men and 7 women). The heads were randomly assigned to either dissection groups, medial-to-lateral (10 heads) or posterior-to-anterior (10 heads).
RESULTS: Fibro-fatty layer was located i

The histologic and histochemical features of palatine muscles from 53 horses were studied; 25 of the horses were racehorses that had upper airway obstruction associated with dorsal displacement of the soft palate and 28 of the horses did not have any respiratory disorders and served as controls. Pathologic features observed included myonecrosis, phagocytosis, mononuclear cell infiltration of perimysial connective tissue, alkaline phosphatase-positive myofibers, and myofibers with cytoarchitectural changes that included irregular staining of the intermyofibrillar sarcoplasm and sarcoplasmic mas

OBJECTIVE: The palatal aponeurosis is a controversial structure, both in terms of its anatomy and its function. This article points out a pathologic finding in the cleft palate condition that has not been previously described.
DESIGN AND METHOD: By means of surgical dissections, this study demonstrates in detail that the palatal aponeurosis exists even in cleft palates, but it is disrupted, malpositioned, and folded in two layers.
PATIENTS: This dissection method has been performed on more than 150 patients with cleft of the hard and soft palate, with or without cleft of the lip and alveolus

The skeletal muscle tissue of the lip, tongue, soft palate and masticatory muscles have been morphometrically studied in 70 human fetuses (4-10-month-old). In the muscles mentioned decrease in the nuclear volume and in the nuclear-cytoplasmic ratio is observed. Increase of the muscle fibres in the area of transversal sections is especially intensive during last three months. The labial and glossal fibres reach the greatest thickness. On the 6th month and further, there is an essential difference in number of myons with nuclei in the transversal section of the tongue and lips, as compared with

The palatal muscle reconstruction has become an essential part of cleft palate repair. Sixty-one cases of various cleft palate and velopharyngeal incompetence after palatoplasty repair using the Furlow's double Z-plasty technique have been reported. A description of our surgical procedure along with the follow-up of 40 cases 1/2 to 1 year after operation demonstrate the advantages of the Furlow's palatoplasty. Our preliminary follow-up results show that the palatal competence rates are 90.5% in 21 cases of nasopharyngoscopic examination and 83.7% in 37 cases of still lateral X-ray radiograph.

BACKGROUND: Palatal tremor is involuntary, rhythmic and oscillatory movement of the soft palate. Palatal tremor can be classified into three subtypes; essential, symptomatic and palatal tremor associated with progressive ataxia.
METHODS: A thorough Pubmed search was conducted to look for the original articles, reviews, letters to editor, case reports, and teaching neuroimages, with the keywords "essential", "symptomatic palatal tremor", "myoclonus", "ataxia", "hypertrophic", "olivary" and "degeneration".
RESULTS: Essential palatal tremor is due to contraction of the tensor veli palatini musc

This study investigated the presence of orofacial myofunctional disorders in Filipino adults born with cleft palate with or without cleft lip (CP±L) who underwent delayed primary palatal (and lip) closure. Nineteen Filipino adults (mean age=22.4, SD=4.47 y) who underwent delayed primary palatal closure with (n=10) or without (n=9) early or delayed lip closure (CP±L) were included. Perceptual and instrumental orofacial myofunctional outcomes were compared with a control group (mean age=19.2, SD=0.98 y) consisting of individuals without a CP±L. In addition, the results of patients with delayed l

Tinnitus produced by synchronous repetitive contraction of the middle ear muscles (middle ear myoclonus) is a rare condition. We present six cases of middle ear myoclonus in whom different management regimes were successful. In two patients, the tinnitus was controlled by conservative measures. In one patient, whose tinnitus was associated with blepharospasm, significant improvement occurred following botulinum toxin injection into the ipsilateral orbicularis oculi. Three patients were cured by tympanotomy with stapedial and tensor tympani tendon section. The aetiology of this type of myoclonu

Acute Guillain-Barre syndrome (GBS) is characterized by an acute onset of limb weakness and areflexia. There are a few rare variants that have been described and one of them is the pharyngeal-cervical-brachial (PCB) variant (oropharynx, neck, and proximal upper limb muscles). However, in this patient, the only presentation was bulbar involvement with fast recovery within days. This is likely to be the milder form of PCB that has rarely been described before. A 19-year-old Malay lady presented with progressive dysphagia associated with nasal voice for one week duration. There was no limb weakne

OBJECTIVE: The aim of this study was to analyze the function of the palatal muscles in vivo by real-time wireless electromyography in rats. The effects of palatal wounding were also analyzed.
METHODS: Microelectrodes were implanted six rats; in the masseter muscle (two-rats) for comparison, in the unwounded soft palate (two-rats) and the soft palate that received a surgical wound (two-rats). Two weeks after implantation, a wound was made in the soft palate using a 1 mm biopsy-punch. Electromyographic measurements and video-recordings were taken weekly to monitor train-duration and peak-amplit
