[Orthodontic and orthophonic neonatal guidance in facial cleft].

Babbling, which prepares pronunciation, the first articulatory movements and the most important 'imitation-interchange mechanism' with the parents, appears as early as age 3 months. According to the method that we proposed in Toronto in 1977, the soft palate is closed at age 3 months, whereas the hard palate remains open till 6 months of age. We think now that the fact of allowing the timely development of these primordial functions by provisional closure of the hard palate with a thin palatal plate and by parental guidance as early as 3 months of age should bring about additional progress.

Use of a palatal stabilizing device in prevention of palatal grooves in premature infants.

A prospective, randomized study using an acrylic palatal stabilizing device (PSD) was conducted to evaluate the effectiveness of this device in preventing disruptions in palatal architecture. A total of 26 premature infants with birth weights of 540 to 1740 g, and intubated orally for a period varying from 7 to 109 days were randomized to control and experimental groups. All neonates in the control group developed palatal grooving ranging from 2 to 5 mm in depth, whereas those in the experimental group (with a PSD) showed no evidence of palatal grooving. We conclude that a PSD is an effective

Early prosthetic management of cleft palate. Articulation development prosthesis: a preliminary report.

This study evaluated the effects of early prosthetic intervention for achieving normal articulation development in cleft palate patients. Early surgery is the preferred method of treatment to permit normal articulation development. Prosthetic management provides an alternative treatment to promote more normal articulation development when early surgery is not planned. It can be concluded that a prosthesis must be placed prior to development of meaningful speech if improved articulation development is to be achieved. Close cooperation among surgeon, speech pathologist, and prosthodontist is nec

[Therapy problems in sphenoethmoidal meningoceles].

After a brief survey of the forms of cranial meningoencephaloceles, the current opinions on their development, symptoms and surgical therapy, a report is given on the case of a five-year-old boy suffering from a sphenoethmoidal meningocele of the size 3 x 3 x 4 cm. There was at the same time a wide median palate cleft, obviously a consequence of the meningoencephalocele. A further finding presented by the boy was a neuroradiologically diagnosed agenesis of the Corpus callosum and a median cheiloschisis. This suggests a central development disturbance as a primary cause. The attempt to perform

Maxillary Surgical Obturator Following Resection of an Odontogenic Myxoma in a Pediatric Patient.

The treatment of odontogenic myxoma (OM) may result in several oral complications. Although oral and esthetic rehabilitation following extensive resections is crucial, it remains a significant challenge. A 14-year-old Brazilian male patient presented with an OM in the left hemimaxilla, which was associated with bone expansion and facial asymmetry. Oral rehabilitation was planned prior to surgical resection, and the proposed treatment was carried out in two stages. In the first stage, an interim obturator prosthesis (IOP) was fabricated. The IOP played a crucial role in tamponading the oronasal

Use of Immediate Obturator Prosthesis as a Rehabilitation Option in Cases of Squamous Cell Carcinoma.

Maxillary defects caused by surgical resection generate abnormal communication between the nasal and oral cavities, hindering proper eating and phonetics, and causing aesthetic damage. Prosthetic intervention is necessary to rehabilitate these patients, in order to block communication and allow correct speech and swallowing functions. This article reports the manufacture of an immediate palatal obturator prosthesis after hemimaxillectomy due to the surgical removal of an intraoral squamous cell carcinoma, which guaranteed better conditions for postoperative recovery, as well as better adaptati

Secondary Reconstruction of Premaxilla With a π-Shaped Fibula Osteocutaneous Flap.

Midface deformities due to oncologic bony defects are often difficult to secondarily correct. The authors herein report 2 cases of secondary reconstruction of an oncological premaxillary defect using a π-shaped fibula osteocutaneous flap. The authors divided the fibula into 3 pieces and made it π-shaped to reconstruct the curvature of the premaxilla. The palatal mucosal defect was closed with a skin paddle. The vascular pedicle was hooked up to the neck vessels either directly or by interposition of vessel grafts. Reconstruction was successful in both cases. The facial appearance was greatly i

Palato-Antral Involvement of a Primary Extracranial Sinonasal Meningioma.

Primary extracranial sinonasal meningiomas are one of the rarest tumours involving the head and neck region. Very few reports exist with additional involvement of the palato-antral region. A 45-year-old male presented with frequent episodes of epistaxis, complaints of nasal blockage and nasal regurgitation for 8 months in 2021 at our tertiary care government hospital in East Godavari district, India. The patient was diagnosed with a primary extracranial sinonasal meningioma with palatal involvement and surgical excision via intra-oral and endoscopic approaches were performed. Postoperative hea

[Value of nasopharyngofiberoscope, spectrography and computer in diagnosis and treatment of pathologic speech after cleft palate operation].

The speech of 45 cases with velopharyngeal incompetence (VPI) were evaluated by combining application of nasopharyngofiberoscope (NPF) and computer-spectrographic analysis system (CSAS) after cleft palate operation and conservative speech therapy with using cleft palate obturators. The results showed: Improving rate of velopharyngeal incompetence (IRVPI) was close to 33.33% after 2.5-3 years therapy. The spectrographic analysis showed a clearer, and darken frequency band, normal resonance peaks (F2,F3) and seldom appearance of negative value of voice origination time (VOT). The above indicated

Nasopharyngeal Polyp in a Patient With Submucous Cleft Palate.

Encountering a nasopharyngeal polyp in a patient with submucous cleft palate (SMCP) is a difficult problem, as the lesion could support the weak palate. Removal of this lesion may unmask the SMCP with consequent worsening of speech nasality. Nasal septal polyp protruding to the nasopharynx in a patient with SMCP has not been reported before in the literature. This report describes a septal polyp arising from the posterior border of the nasal septum and protruding in the nasopharynx in a 16-year-old girl with submucous cleft palate. The polyp appeared to support the weak palate, and they acted

Prosthodontic treatment of the edentulous adult cleft palate patient.

Clefts of the upper lip and plate are relatively common, yet dental treatment of these patients is still very poor and many grow up suffering dental neglect. Dental practitioners should become involved in the treatment team as dental needs are present from birth to death. Adult cleft patients often need tooth replacement with obturation of any residual clefts. They are best treated with tooth-supported removable appliances including partial and complete overdentures, thus preservation of their natural dentition is desirable. Edentulous cleft palate patients present with restorative difficultie

Early non-surgical closure of postoperative palatal fistulae.

Ten of 11 patients who developed oronasal fistulae subsequent to palatoplasty and/or pharyngeal flap operation obtained complete physiological closure of the fistula by using a vinyl palatal appliance for a two to three month period. All these fistulae were diagnosed within 10 days to two weeks after the surgery, and the appliance therapy was started immediately. We believe this treatment to be an effective, uncomplicated method for non-surgical closure of recent postoperative oronasal fistulae.

Pterygoid implants for maxillofacial rehabilitation of a patient with a bilateral maxillectomy defect.

Bilateral maxillectomy is known to have serious esthetic and functional consequences. The retention and support of a maxillary obturator prosthesis in these patients is particularly challenging. Surgical placement of implants is also challenging because of the lack of available bone. Therefore, implant placement into remote sites such as zygoma has been advocated. Very few articles in the literature have discussed the use of pterygoid/pterygomaxillary implants in patients undergoing maxillectomy. This case report describes the maxillofacial rehabilitation of an elderly man who underwent a bila

Selective Retreatment and Sinus Lift: An Alternative Approach to Surgically Manage the Palatal Roots of Maxillary Molars.

Endodontic microsurgery on the palatal root of maxillary molars presents a clinical challenge because of the root position and approximation from the maxillary sinus floor. Attempting a buccal or a palatal approach to address the root is associated with limited accessibility and visibility as well as the risk of injury to the maxillary sinus membrane and/or the greater palatine nerves and vessels. If all the maxillary molar roots require surgical intervention, two flaps may even be needed, which can make the procedure technically more difficult and lengthier. This case report presents 2 clinic

Nonsurgical Management of Horizontal Root Fracture with Necrotic Apical Segment and Coronal Cervical Abfraction using Mineral Trioxide Aggregate: Case Report.

Horizontal root fractures are relatively rare dental findings and often pose diagnostic and treatment challenges. In addition to a horizontal root fracture, this case report also demonstrates a finding of a noncarious cervical abfraction lesion, which involves the loss of dental hard tissue from the cervical areas of teeth through processes unrelated to caries. A 22 -year-old healthy female patient was complaining of the maxillary left first premolar area with a history of dental trauma. Clinically, the tooth presented with a noncarious cervical abfraction lesion. Endodontic testing showed a n

Delayed hard palate repair and speech deficiencies: a cautionary report.

In the management of patients with complete palatal clefts early repair of the soft palate (before 1 year of age) and delayed repair of the hard palate (after five or six years of age) has been advocated on the basis that good speech will develop following soft palate closure and that avoidance of trauma to the hard palate will obviate maxillary growth disturbance. In addition, it is said that many of the remaining hard palate fistulas will close spontaneously and that residual hard palate openings will be easy to close. Thirty-two cases treated in this way are reviewed, and a decade of experi
