Deep Brain Stimulation for Dystonia.

Dystonia refers to movement disorders characterized by sustained muscle contractions that produce abnormal postures, twisting movements, and other abnormal involuntary movements. A spectrum of etiologies underlies the various dystonia syndromes, ranging from genetic conditions to brain injury. First-line therapy for dystonia consists of pharmacologic agents of several classes and, particularly for focal dystonia, chemodenervation therapy with botulinum toxin. Many patients with dystonia realize an inadequate response to those treatments, and for such patients whose symptoms are sufficiently tr

Destructive stereotactic surgery for treatment of dystonia.

BACKGROUND: This study is a retrospective review of the results of stereotactic destructive surgery in selected cases of drug-resistant dystonia. METHODS: Fifty-eight patients with drug-resistant dystonia were treated with stereotactic surgery between 1991 and 1999 in our institution. These patients' charts were retrospectively analyzed. The timing of the conducted evaluations was as follows: preoperatively, postoperatively, in the postoperative 1st week, 6th month, 12th month, and also thereafter every year. RESULTS: Symptoms of dystonia occurred before the age of 10 years in 30 patients (5

Comparison of different surgical strategies for cervical dystonia: Evidence from Bayesian network analysis.

BACKGROUND AND PURPOSE: Several surgical techniques have been used to treat cervical dystonia (CD), however, to date, the optimal surgical technique for CD remains controversial. We therefore conducted the first network meta-analysis to compare different surgical strategies for CD to inform clinical practice. METHODS: Electronic databases were searched for surgical strategies for treating CD. The primary outcome was improvement in total Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) score. Subgroup analysis was performed to compare short-term (< 1 year) and long-term (≥ 1 year) o

Cytotoxicity of ventricular cerebrospinal fluid from Parkinson patients: correlation with clinical profiles and neurochemistry.

Other investigators have reported that the cerebrospinal fluid (CSF) from patients with Parkinson's disease (PD) might contain endogenous dystrophic factors. Using CSF samples drawn from individual PD patients during surgery, we investigated the toxic effect of ventricular CSF (vCSF) on the growth of PC12 cells and the correlation between the clinical profiles of the patients and CSF neurochemistry. Ventricular CSF samples from 28 patients with PD or essential tremor (ET) were collected during ventriculography for stereotactic pallidotomy or thalamotomy. PC12 cells were incubated with 20% vCSF

Acute complications of movement disorders surgery: effects of age and comorbidities.

The most common indication for movement disorder surgery is Parkinson's disease (PD), and the incidence of PD increases with age. The analysis reported here was undertaken with the primary goal of examining whether there is a relationship between peri-operative complications and age. The Nationwide Inpatient Sample (Agency for Healthcare Research and Quality, Rockville, MD, USA) was queried for 10 years beginning in 1999 for patients undergoing deep brain stimulator insertion, pallidotomy, and thalamotomy for treatment of PD, essential tremor, and dystonia. Inpatient complications, including d

Mechanisms underlying the onset and expression of levodopa-induced dyskinesia and their pharmacological manipulation.

A significant proportion of patients with Parkinson's disease (PD) receiving dopamine replacement therapy in the form of levodopa develop dyskinesia that becomes a major complicating factor in treatment. Dyskinesia can only be effectively treated by a reduction in drug dose, which limits efficacy, by co-administration of the weak NMDA antagonist amantadine or by surgical treatment (pallidotomy, DBS). This raises the important question of why dyskinesia occurs in PD and how it can be avoided or suppressed by pharmacological treatment. This review assesses some of the mechanisms that underlie dy

Dystonia: clinical features, genetics, and treatment.

PURPOSE OF REVIEW: The present review covers recent advances in dystonia research related to dystonia genetics and treatment. These have led to the discovery of novel dystonia genes and loci, to changing classification schemes, and to the introduction of improved and new treatment options. RECENT FINDINGS: Currently 13 different forms of dystonia can be distinguished on a genetic basis (dystonia types 1-13). Recently, a novel gene locus (DYT13) was detected in a family with segmental dystonia, and the gene causing myoclonus-dystonia was identified (SGCE). Furthermore, a novel mutation in the

Dystonia.

Therapy for most people with dystonia is symptomatic, directed at lessening the intensity of the dystonic contractions. For a small minority of patients (eg, those with dopa-responsive dystonia, Wilson's disease, or psychogenic dystonia), specific therapy directed at one of the many causes of dystonia is available. Before initiating treatment, clinicians need to decide if a patient has a form of dystonia amenable to such therapy. The most sensitive and least costly method to diagnose DRD is a therapeutic trial of levodopa. It is, therefore, recommended to treat all those with dystonia beginnin

Cranial procedures without hair removal.

OBJECTIVE: In 1992, Winston published the first large series of patients undergoing cranial neurosurgery without hair removal (Winston KR: Hair and neurosurgery. Neurosurgery 31:320-329, 1992). Prompted by this report, the senior author began a prospective trial in 1992 of cranial neurosurgery without hair removal. METHODS: All patients undergoing elective cranial surgery were offered the opportunity to undergo surgery without hair removal. The protocol advocated by Winston was strictly followed in the first 100 patients but has subsequently been modified. Patients having only cranial procedur

Movement Disorders in Children: Treatment Algorithm and Surgical Options.

Paediatric movement disorders (PMDs) encompass a wide range of diseases resulting from countless aetiologies. These are distinct from their adult counterparts, with a far greater proportion of genetic mutations, perinatal injuries, metabolic disorders, and in many cases, undefined neurodevelopmental abnormalities. Given the complexity of both the diagnostic process and management pathway of PMDs, early identification is critical to guide appropriate intervention in a timely manner. Through early recognition and management, clinicians positively impact neurodevelopment, quality of life, and lon

Altered discharge pattern of basal ganglia output neurons in an animal model of idiopathic dystonia.

A decreased activity of basal ganglia output neurons is thought to underlie idiopathic dystonias and other hyperkinetic movement disorders. We found recently a reduced spontaneous discharge rate of entopeduncular neurons (internal globus pallidus in primates) in dt(sz) hamsters, an unique model for idiopathic paroxysmal dystonia in which stress-inducible attacks show an age-dependent severity. Otherwise, it has been suggested that an altered discharge pattern may be more important for the occurrence of dystonia than a reduced discharge rate. Based on qualitative and computerized quantitative e

Objective clinical registration of tremor, bradykinesia, and rigidity during awake stereotactic neurosurgery: a scoping review.

Tremor, bradykinesia, and rigidity are incapacitating motor symptoms that can be suppressed with stereotactic neurosurgical treatment like deep brain stimulation (DBS) and ablative surgery (e.g., thalamotomy, pallidotomy). Traditionally, clinicians rely on clinical rating scales for intraoperative evaluation of these motor symptoms during awake stereotactic neurosurgery. However, these clinical scales have a relatively high inter-rater variability and rely on experienced raters. Therefore, objective registration (e.g., using movement sensors) is a reasonable extension for intraoperative assess

Magnetic resonance-guided focused ultrasound for Parkinson's disease since ExAblate, 2016-2019: a systematic review.

INTRODUCTION: ExAblate received FDA approval for treatment of a range of movement disorders in 2016, including tremor-dominant Parkinson's disease (TDPD), dyskinetic PD, and essential tremor. This incisionless device allows for magnetic resonance-guided focused ultrasound (MRgFUS) for ablation of several regions of interest. Current studies should aim to measure pre- and post-operative neurocognitive functioning to better understand MRgFUS in PD and how it compares to deep brain stimulation, which has known cognitive risks among certain populations. METHODS: PubMed, CINAHL, PsycINFO, and Coch

Unilateral Stereotactic Radiofrequency Lesioning as a Surgical Treatment Option for Meige Syndrome.

Background Meige syndrome is a segmental dystonia affecting the head and neck, with bilateral blepharospasm as the primary symptom. First-line treatment typically involves Botox injections. For cases resistant to this treatment, bilateral deep brain stimulation of the globus pallidus internus (GPi) is considered. This study explores the efficacy of unilateral radiofrequency (RF) lesioning as an alternative surgical treatment for Meige syndrome. Methods We investigated six cases of medically refractory Meige syndrome treated with unilateral RF lesioning between October 2022 and August 2023. The

Robot-assisted procedures in pediatric neurosurgery.

OBJECTIVE During the last 3 decades, robotic technology has rapidly spread across several surgical fields due to the continuous evolution of its versatility, stability, dexterity, and haptic properties. Neurosurgery pioneered the development of robotics, with the aim of improving the quality of several procedures requiring a high degree of accuracy and safety. Moreover, robot-guided approaches are of special interest in pediatric patients, who often have altered anatomy and challenging relationships between the diseased and eloquent structures. Nevertheless, the use of robots has been rarely r

The emerging role of high-intensity focused ultrasound (HIFU) as a symptomatic treatment for Parkinson's disease and essential tremor: a narrative review.

The high-intensity focused ultrasound (HIFU) technique has become precise, incisionless, and ablative for treating essential tremor (ET) and Parkinson's disease (PD), driven by advances in magnetic resonance imaging (MRI) thermometry and tractography. The current review integrates the main clinical outcomes associated with established and emerging HIFU targets, synthesizing efficacy data, adverse effect (AE) patterns, and procedural characteristics relevant to clinical decision-making. In ET, ventral intermediate nucleus (VIM) thalamotomy results in tremor reductions ranging from 56 to 81%, wi

Visual evoked oscillatory responses of the human optic tract.

Optic tract oscillatory responses directly recorded during posteroventral pallidotomy were investigated to reveal their features with respect to extracranially recorded visual oscillations and to clarify their contributions to scalp-recorded or far-field visual evoked potentials. Oscillatory responses of the optic tract consisting of early and subsequent late oscillations were recorded in all patients. Early oscillations consist of five negative and positive peaks, and late oscillations consist of two to four negative and positive peaks. The frequency of the first peak of early oscillations (1
