A significant amount of variability exists between observers in designating cup/diac (C/D) ratios. Further, different methods are used for evaluating the size of the cup. On method uses a combination of contour and color, the other specifically measures contour and pallor separately. This study confirms that these methods yeild different numerical results. The observations of pallor, sweating, agitation and restlessness, at the time of an injured patient's admission to the resuscitation room, were compared with other more complex indices of injury severity and the ability of each observation to predict mortality was observed. The number of positive observations was significantly related to the Revised Trauma Score and the Injury Severity Score, and pallor was significantly related to mortality. The observation that an injured patient is pale is an important index of severity.

Here, we describe the emergence of an idea to quantify these optic disc color changes in multiple sclerosis patients. A cross-sectional study of 150 child labourers engaged in different occupations in a slum area of Calcutta was conducted to assess their health and nutritional status. Most (88%) of them had some type of morbidity. Female child workers (95.5%) suffered more than the males (84.7%). Thiamine deficiency classically manifests as the triad of Wernicke encephalopathy: acute confusional state, ataxic gait, and ocular motor dysfunction. However, most patients do no present with this classic triad. Optic neuropathy in thiamine deficiency is a rare manifestation and is usually associated with fundus appearances of optic disc swelling or optic disc pallor. We present 2 unique cases of thiamine deficiency where the fundus demonstrated peripapillary retinal nerve fiber layer thickening without florid disc swelling or pallor.

Although external genital cutaneous sarcoidosis is rare, it can closely resemble lichen sclerosus (LS) and delay diagnosis. We report a case of a 69-year-old woman with long-standing vulvar symptoms and LS-like pallor of the labia minora and clitoris. Biopsy showed non-caseating granulomas with negative infection stains, consistent with vulvar sarcoidosis. This case supports the need for early biopsy in cases of persistent or atypical vulvar pallor and highlights sarcoidosis as an uncommon but important LS-mimic.

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare hematological malignancy that derives from precursors of plasmacytoid dendritic cells and is characterized by disseminated, erythematous or bluish-livid plaques or nodi. Because of the disease's rarity the diagnosis and treatment still pose a significant challenge. We present a case of a patient with BPDCN and show clinical and diagnostic characteristics as well as potential treatment regimes.
