A 46-year old lady was under regular follow up for aortic valve replacement done in 2002 for aortic stenosis. The valve was a Carbomedics 19 mm bi-leaflet aortic valve prosthesis for which she had adequate anticoagulation since implantation. She had a past history of end stage renal failure, type 2 diabetes, hypertension, cerebrovascular disease and systemic lupus erythematosus. She was asymptomatic and had a routine transthoracic echocardiogram performed which revealed that her aortic valve prosthesis was well seated with an elevated velocity across the valve; the aortic valve leaflets were p

THE CONCLUSIONS WHICH MAY BE DRAWN FROM THE RESULTS OF THE EXPERIMENTS HERE PRESENTED ARE: 1. The cornea of the rabbit is highly sensitive to the action of various injected bacteria. The lesions vary from insignificant, transient changes to severe, destructive panophthalmitis, with fine gradations from the mildest to the violent form of inflammation. Moreover, animals that receive the same organisms show like changes. 2. The varying degree of inflammatory reaction is related to the pathogenicity of the special culture employed; as, for example, is shown by the reactions to Type I pneumococci a

Identification of thrombus-related mechanical prosthetic valve dysfunction (MPVD) has important therapeutic implications. We sought to develop an algorithm, combining clinical and echocardiographic parameters, for prediction of thrombus-related MPVD in a series of 53 patients (24 men, age 52 +/- 16 years) who had intraoperative diagnosis of thrombus or pannus from 1992 to 1997. Clinical and echocardiographic parameters were analyzed to identify predictors of thrombus and pannus. Prevalence of thrombus and diagnostic yields relative to the number of predictors were determined. There were 22 pat

Rheumatoid arthritis (RA) is a systemic disorder characterized by synovial inflammation and subsequent destruction and deformity of synovial joints. The articular lesions start with synovitis, focal erosion of unmineralized cartilage, and then culminate in the destruction of subarticular bone by pannus tissue. Periarticular osteopenia and systemic osteoporosis follow as late complications of RA. Osteoclasts, specialized cells that resorb bone, play a central role in developing these osteolytic lesions. To elucidate the mechanism of osteoclastogenesis and bone destruction in autoimmune arthriti

A 5-week-old boy with Peters' anomaly was found to have an interstitial deletion of the long arm of chromosome 11; no developmental delays or dysmorphic features were evident. His right cornea was enlarged and opaque with extensive pannus formation; the anterior chamber, iris, and lens were not visible. The left eye showed a central opacity and a superficial pannus; the optic disk and macula could not be visualized. He underwent bilateral corneal transplantation. Histologic examination of the corneal buttons confirmed the diagnosis. The potential genetic mechanisms in this case included a gene

PURPOSE: Aniridia is a rare corneal disease that is often associated with aniridia-associated keratopathy (AAK). In AAK, the conjunctival tissue crosses the limbal border, forming a corneal pannus that extends into the corneal center. With increasing AAK severity, corneal pannus formation, vascularization, and ocular surface inflammation increase. The purpose of this study was to investigate inflammation-related mRNA expression in conjunctival epithelial cells in AAK and its relationship with AAK severity.
METHODS: Using impression cytology, bulbar conjunctival cells were sampled from 20 subj

OBJECTIVE: Aggressive fibroblast-like synoviocytes (FLS) drive synovial hyperplasia, extracellular matrix (ECM) remodeling, and pannus formation in rheumatoid arthritis (RA). However, the pathogenic FLS subpopulations and their upstream regulators remain incompletely defined. This study aimed to (i) identify FLS subpopulation(s) responsible for synovial hyperplasia, ECM remodeling, and pannus development, (ii) elucidate the molecular mechanisms regulating their pathogenic activation, and (iii) explore therapeutic strategies targeting FLS activation and pannus-associated neovascularization in R

PURPOSE: A classification-based segmentation method is proposed to quantify synovium in rheumatoid arthritis (RA) patients using a deep learning (DL) method based on time-intensity curve (TIC) analysis in dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI).
MATERIALS AND METHODS: This retrospective study analyzed a hand MR dataset of 28 RA patients (six males, mean age 53.7 years). A researcher, under expert guidance, used in-house software to delineate regions of interest (ROIs) for hand muscles, bones, and synovitis, generating a dataset with 27,255 pixels with corresponding TICs

Thirteen patients with atlantoaxial dislocation(s) had posterior stabilization and were evaluated clinically with magnetic resonance (MR) imaging and conventional radiography both pre- and postoperatively. Cases with anterior instability had large periodontoid granulomatous pannus formation, but this was lacking in cases with fixed cranial settling. The pannus caused cord compression in four cases and occupied the entire anterior subarachnoid space in four other cases. After stabilization, the granuloma decreased in size in all patients and in four cases it vanished completely. Major signs of

Prosthetic valve thrombosis (PVT) is a frequent complication with a mechanical valve that presents with symptoms of heart failure or thromboembolic episodes. A 45-year-old lady with antiphospholipid syndrome (APS) complicated by a previous history of native mitral valve thrombus and mechanical mitral valve replacement maintained on warfarin presented with complaints of chest pain and shortness of breath (NYHA class 2). The initial lab showed a subtherapeutic international normalized ratio (INR) of 1.8. Transthoracic echo (TTE) showed severe mitral stenosis with a normal ejection fraction of 65

Specific major histocompatibility complex (MHC) class II genes result in a high susceptibility to rheumatoid arthritis (RA), with co-stimulatory molecules working together with MHC class II during the progression of the disease. To elucidate the involvement of the B7.1 co-stimulatory molecule in RA, we analyzed the phenotype of B7.1 transgenic (named D1BC) mice and the sequential differentiation of synovial fibroblasts (SFs) by studying the expression of chondrogenic and osteogenic lineage markers together with lineage tracing experiment using B7.1 transgene in vivo. The B7.1 transgene was dri

Peroxidase-anti-peroxidase (PAP) staining and specific antibodies against cathepsin G and elastase from polymorphonuclear leukocytes (PMN) were applied to pannus-free and microscopically intact superficial articular cartilage. Restricted local deposits containing cathepsin G and elastase were found in three of ten patients with seropositive rheumatoid arthritis (RA), in one of three patients with seronegative RA and in one patient with juvenile chronic arthritis (JCA). Similarly, localized deposits of IgG and C3 were found in the patients with seropositive RA and JCA, but not in the patient wi

BACKGROUND: Bioprosthetic heart valves are a treatment option for patients with significant valvular disease. We present the first morphologic analysis of a series of explanted Mitroflow pericardial valves (model A12) and discuss their modes of failure.
METHODS: Between 2001 and 2010, 12 Mitroflow (A12) valves (Sorin Group Inc, Vancouver, Canada) were examined, grossly and microscopically, for postimplantation changes. Cusp tears and thickening, calcification (at radiography), pannus formation, inflammation, and thrombus deposition were noted. Microscopically, tissue degeneration including th

Rheumatoid arthritis (RA) is one of the most common autoimmune diseases. Erosive arthritis and pannus are key clinical features of RA, and the cervical spine (CS) involvement in RA is ranked 3rd in frequency after hand and foot joint involvement. Neurological complications in patients with the CS involvement significantly aggravate the course of RA and can cause an adverse outcome. Two clinical cases of RA patients with subluxation in the atlanto-occipital (AOJ) and atlanto-axial (AAJ) joints, pannus, and fatal outcome are presented. It was found that one of the first symptoms of AOJ and AAJ s

Dysfunction of any mechanical prosthesis due to thrombus or pannus requires prompt definite diagnosis and therapy. A 55-year-old woman had received a prosthetic valve for aortic regurgitation 3 years before and was admitted for intermittent chest pain. Electrocardiography showed remarkable ST depression in broad leads during chest pain. Coronary angiography and cinefluoroscopy performed for definite diagnosis at the time of the third attack revealed no stenotic lesion in the coronary artery, but an artificial valve stuck in the completely open position. The diagnosis was acute aortic regurgita

Spinal cord injury (SCI), particularly of the traumatic variety, is a relatively common condition that disproportionately affects the elderly. Cases of SCI with nontraumatic etiologies in the geriatric population have increased over the last 20 years, however. Pannus formation, resulting from chronic inflammation of the spine, is one such etiology that may progress to SCI and potentially result in rapid neurological degeneration. Here we describe a case of an elderly woman who presented with a sudden onset of quadriplegia without a history of trauma. Radiography revealed upper cervical instabi

We describe the case of a 67-year-old Asian female patient suffering from severe systemic lupus erythematosus (SLE), including biopsy-proven glomerulonephritis, since the age of 40 who was admitted for tetraparesis. Neurological examination confirmed proximal muscular weakness, hypoesthesia and diminished tendon reflexes. The patient suffered from extremely severe Jaccoud's arthropathy. Magnetic resonance imaging (MRI) demonstrated severe narrowing of the upper spinal canal due to a soft tissue mass surrounding the odontoid process, assumed to be a synovial pannus, causing myelopathy. The pati

To study the vascular changes in inflammatory diseases of the cornea 22 patients with various corneal inflammations were examined by means of anterior segment fluorescein angiography. Simple avascular central and marginal corneal ulcers stained with fluorescein in the late phase of angiography. An inflamed limbus and an early microscopic pannus adjacent to the ulcer were seeen in simple corneal ulcers. Progressive pannus with pronounced fluorescein leakage was observed in chronic corneal ulcer, disciform keratitis, Mooren's ulcer, and complicated acute keratoconus. In sclerokeratouveitis and i
