Reversal of paraparesis after thoracic aneurysm repair by cerebrospinal fluid drainage.

PURPOSE: To describe a case of reversal of delayed paraparesis, after an elective type I thoracoabdominal aortic aneurysm (TAAA) repair, via cerebrospinal fluid (CSF) drainage. CLINICAL FEATURES: A 75-yr-old woman developed paraparesis 13 hr after type I TAAA repair. The patient had been given combined regional and general anesthesia. There was no cerebrospinal fluid drain inserted at the time of surgery. The patient was hemodynamically stable throughout the procedure and was transported to the intensive care unit with trachea intubated and lungs ventilated. She demonstrated some initial lowe

Tropical spastic paraparesis/HTLV-I associated myelopathy. Etiology and clinical spectrum.

In 1985 we had the first indication that human T-cell lymphotropic virus (HTLV-I) was the possible etiological agent of a chronic myelopathy that seemed to be peculiar to the tropics and that is now known as endemic tropical spastic paraparesis (TSP). IgG antibodies to HTLV-I were found in serum and cerebrospinal fluid of patients from Jamaica, Colombia, Martinique, and shortly after in southern Japan, where the disease is called HTLV-I-associated myelopathy (HAM). The HTLV-I seropositivity was first determined by enzyme-linked immunoassay and confirmed by western immunoblot and in the cerebro

A new inherited prion disease (PrP-P105L mutation) showing spastic paraparesis.

We report the clinicopathological findings of 5 patients with an inherited prion disease with a codon 105 (Pro to Leu) mutation. All of the patients had a spastic gait disturbance and progressive dementia without either cerebellar signs, myoclonus, or periodic synchronous discharges. Autopsy of 3 patients revealed numerous amyloid plaques in the cerebral cortex, especially in the motor cortex and the frontal lobe where neuronal loss and severe gliosis were observed in the absence of spongiform changes. The cerebellum was preserved histologically except for only a few amyloid plaques. The pyram

Evoked potential abnormalities in progressive spastic paraparesis associated to HTLV-1.

The electrophysiological features of progressive spastic paraparesis (PSP) associated with HTLV-1 in Chile, a non-tropical country, are presented. Twenty-two of 45 PSP subjects were positive for HTLV-1 antibodies. Trimodal evoked potentials were all normal in only 2 of these cases (9.1%). Somatosensory evoked potentials (SSEPs) were abnormal in 19 patients (86.3%) with a mean amplitude of 1.31 uV SD 0.92. Visual evoked potentials (VEPs) and brainstem auditory evoked potentials (BAEPs) were normal in all patients except four. Peripheral nerve conduction was normal in all but one who showed disc

Flaccid paraparesis following obstetrical epidural anesthesia: possible role of benzyl alcohol.

Severe flaccid paraplegia accompanied by extensive sensory loss after epidural anesthesia administered for labor and delivery occurred in a previously healthy 24-year-old primagravida. Recovery was virtually complete after 16 months. The cause of the neurologic deficits may have been the 1.5 percent benzyl alcohol preservative contained in an 0.9 percent saline solution used for postdelivery epidural injection.

Ovarian Lymphoma and Bone Metastasis Presenting With Paraparesis in an Adolescent: A Case Report.

BACKGROUND Lymphoma comprises a heterogeneous group of lymphoid malignancies broadly categorized as Hodgkin lymphoma and non-Hodgkin lymphoma. Ovarian involvement is exceedingly rare, particularly in adolescents, and is most commonly associated with diffuse large B-cell lymphoma (DLBCL) as part of systemic disease. Its clinical and radiological resemblance to primary ovarian malignancies often leads to diagnostic challenges. CASE REPORT We describe the case of a 17-year-old nulligravid female adolescent who presented with a 2-month history of progressive bilateral lower extremity weakness acco

Preventable Burn Injury in a Person With Paraparesis Leading to Amputation: A Case Report.

Spinal cord injury (SCI) often results in prolonged recovery and complications, as seen in the case of a 35-year-old mason who sustained an SCI after a 10-foot fall in June 2021. Initially presenting with lower limb paresis and loss of bladder and bowel control, he underwent D12-L2 fixation with decompression and showed a gradual recovery of motor and sensory function above the knees. However, due to sensory loss and inadequate awareness regarding pressure injury prevention, he developed sacral pressure sores, which were healing with appropriate wound care and pressure offloading. During this

A rare case of adult-onset spastic paraparesis associated with Klinefelter syndrome.

REPORT: The rare association of Klinefelter syndrome and the clinical presentation of a late onset chronic progressive spastic paresis. CLINICAL PRESENTATION AND GENETICS: An infertile, 61-year-old man, presented with late adult onset of gait problems, deep muscle pain, and bladder problems. He presented for the first time, years after onset with a spastic paraparesis with high arched feet. His parents had already died, but the patient described high arched feet with his mother. There is no further certain information about the parents. After thorough investigation, an additional X chromosome

Intramedullary tuberculoma of the thoracic spine presented as paraparesis: A rare case report.

Tuberculosis (TB) is a bacterial infection with Mycobacterium tuberculosis which primarily affects the lungs; however, it can affect any organ in the body. Brain tuberculous infection usually comes via hematogenous spread from a pulmonary focus and rarely via direct spread from ear, cranial vault or spine. It can present in various conditions including the following: tuberculous meningitis (TBM), brain or spinal cord tuberculoma, miliary TB, tuberculous abscess, tuberculous encephalopathy, and even intracranial hemorrhage. Here, we report an extremely rare case of intramedullary spinal cord tu

Spirituality as Strategies for Coping With Tropical Spastic Paraparesis: Results of Focus Group.

In the logic of integrality in health, one of the aspects less addressed by assistance services is the question of spirituality. This study utilized qualitative analysis from focus groups to identify whether spirituality can contribute to coping with problems arising from the HTLV-1 myelopathy associated or tropical spastic paraparesis (HAM/TSP). The testimonies were recorded and then transcribed. The information was then systematized by the analysis of thematic-categorical content. When giving voice to people who suffer from HAM/TSP, there is clear evidence that spirituality, understood broad

Posterior mediastinal neuroblastoma masked as flaccid paraparesis in a 3 year child.

Neuroblastoma is an embryonic tumor of the sympathetic nervous system originating from neural crest cell remnants, which may be found in the adrenal glands or sympathetic chain. We report a case of a three-year-old girl who presented with flaccid paraparesis, and acute respiratory distress that required mechanical ventilation after a multiple-day history of gradual-onset dyspnea and right upper-lung opacity that was initially misdiagnosed as pneumonia. Chest and spinal magnetic resonance imaging (MRI) revealed a tumor in the posterior mediastinum with intra-spinal canal extension. The patient

PARAPARESIS IN A PATIENT WITH ADVANCED HIV DISEASE WITHOUT MANIFESTATIONS OF OPPORTUNISTIC INFECTION.

HIV infection had been associated with many symptoms and signs but the least expected is paraparesis in the absence of constitutional clinical features. This case presentation is to highlight the unusual presentation of a 43 year old who presented with difficulty in walking due to gradual weakness in his lower limbs (more on right than left) and difficulty in passing urine of 8 months duration. All the common symptoms and signs associated with advanced HIV infection were absent. Urethroscopy and prostate biopsy were done and showed normal findings. A case of multilevel percutaneous vertebroplasty for vertebral metastases resulting in temporary paraparesis.

Percutaneous vertebroplasty (PVP) is a minimally invasive treatment that has been widely used for the treatment of osteoporotic vertebral compression fractures and vertebral tumors. However, the maximum number of vertebral segments treated in a single PVP remains controversial. Furthermore, PVP may cause complications, including cement leakage, pulmonary embolism, bone cement toxicity, and spinal nerve-puncture injury. We report the rare case of a patient who underwent multilevel PVP for vertebral metastases, with no bone cement leakage or spinal cord injury, but who developed temporary parapa

Histopathological proof of the pathogenicity of a rare GFAP mutation in a patient with flaccid paraparesis.

Infantile Alexander disease is a rare progressive leukodystrophy caused by autosomal dominant mutations in the (GFAP) gene typically presenting with psychomotor retardation, progressive macrocephaly and refractory epilepsy. Neuroradiological hallmarks are extensive white matter lesions with frontal preponderance as well as signal intensity changes of basal ganglia and medulla oblongata with variable contrast enhancement. Here, we report an atypical manifestation in a 21-month-old boy presenting with flaccid paraparesis and areflexia. Cognitive, visual as well as fine motor skills and muscular
