Idiopathic scoliosis (IS) is a multifactorial disease with epigenetic modifications. Tissue dependent and differentially methylated regions (T-DMRs) may regulate tissue-specific expression of the estrogen receptor 1 gene (ESR1). This study aimed to analyze methylation levels within T-DMR1 and T-DMR2 and its concatenation with ESR1 expression of IS patients. The study involved 87 tissue samples (deep paravertebral muscles, both on the convex and the concave side of the curve, and from back superficial muscles) from 29 girls who underwent an operation due to IS. Patient subgroups were analyzed a

BACKGROUND: The function of the paraspinal muscles and especially the psoas muscle in maintaining an upright posture is not fully understood. While usually considered solely as a hip flexor, the psoas muscle and its complex anatomy suggest that the muscle has other functions involved in stabilizing the lumbar spine. The aim of this study is to determine how the psoas muscle and the posterior paraspinal muscles (PPM; erector spinae and multifidus) interact with each other.
METHODS: A retrospective review including patients undergoing posterior lumbar fusion surgery between 2014 and 2021 at a t

STUDY DESIGN: Basic science, experimental animal study.
OBJECTIVE: To determine the effects of Botulinum toxin type A (BTX-A) injections on the mechanical properties of skinned muscle fibers (cells) of rabbit paraspinal muscles.
SUMMARY OF BACKGROUND DATA: BTX-A has been widely used in the treatment of disorders of muscle hyperactivity, such as spasticity, dystonia, and back pain. However, BTX-A injection has been shown to cause muscle atrophy, fat infiltration, and decreased force output in target muscles, but its potential effects on the contractile machinery and force production on the ce

OBJECTIVE: While paraspinal muscle degeneration is closely linked to lumbar disc herniation (LDH), the relationship between radicular compression and paraspinal muscle degeneration remains controversial, and evidence regarding the effect of percutaneous endoscopic lumbar discectomy (PELD) on paraspinal muscle degeneration is limited. This study aimed to investigate the correlation between radicular compression and paraspinal muscle degeneration in LDH patients and to evaluate postoperative changes in paraspinal muscles following PELD.
METHODS: A total of 185 patients with single-level L4-5 LD

BACKGROUND: Aortic abdominal calcification (AAC) is associated with spine-related conditions, such as lower back pain and reduced bone mineral density. Similar to peripheral vascular disease, AAC possibly reduces blood flow to the lumbar posterior paraspinal muscles (PPM) which may lead to atrophy and increased fatty infiltration.
METHODS: Imaging of patients with lower back pain was analyzed. AAC was assessed on lateral lumbar radiographs according to the Kauppila classification. The cross-sectional area of the PPM was measured on a T2-weighted axial MRI sequence and the functional cross-sec

The Wiltse transforaminal lumbar interbody fusion (W-TLIF) modifies the traditional open posterior lumbar fusion to treat the same degenerative pathologies through a smaller, muscle-sparing incision that accesses the lumbar spine through the paramedian corridor between the multifidus and longissimus muscles, preserving the posterior musculature and limiting iatrogenic soft tissue trauma. This protocol provides a reproducible, step-by-step guide to performing the muscle-sparing W-TLIF for lumbar decompression and interbody fusion. After prone positioning and fluoroscopic level localization, a p

OBJECTIVE: Paraspinal muscle flaps have been used for complex wound reconstruction. Given the higher rates of wound complications associated with closure following posterior cervical decompression and fusion (PCDF), this study evaluated the use of paraspinal muscle flap advancement to mitigate this recognized risk.
METHODS: A retrospective cohort of patients who underwent PCDF with conventional closure at an academic quaternary center between 2011 and 2022 was identified. Postoperative outcomes were compared with those of a prospectively maintained cohort of patients who underwent PCDF with p

OBJECTIVE: Evidence that individuals with osteoporosis have an increased susceptibility to and frequency of sarcopenia has led to the creation of the term "osteosarcopenia." Given the limitations of dual-energy X-ray absorptiometry in spine bone mineral density assessment, novel techniques such as MRI-based vertebral bone quality (VBQ) are increasingly being utilized to quantify site-specific bone quality in the spine. The relationship between cervical paraspinal muscle morphology and cervical bone quality has not been investigated. This study aimed to explore the relationship between cervical

Evoking muscle responses by electrical vestibular stimulation (EVS) may help to understand the contribution of the vestibular system to postural control. Although paraspinal muscles play a role in postural stability, the vestibulo-muscular coupling of these muscles during walking has rarely been studied. This study aimed to investigate how vestibular signals affect paraspinal muscle activity at different vertebral levels during walking with preferred and narrow step width. Sixteen healthy participants were recruited. Participants walked on a treadmill for 8 min at 78 steps/min and 2.8 km/h, at

PURPOSE: Spinal and lower extremity degeneration often causes pain and disability. Lower extremity osteoarthritis, eventually leading to total knee- (TKA) and -hip arthroplasty (THA), can alter posture through compensatory mechanisms, potentially causing spinal misalignment and paraspinal muscle (PM) atrophy. This study aims to evaluate the association between prior THA or TKA and PM-degeneration in patients undergoing elective lumbar surgery for degenerative conditions.
METHODS: A retrospective analysis of patients undergoing lumbar surgery for degenerative conditions was conducted. Patients

BACKGROUND: Delayed meningeal cyst (DMC) is a rare complication after spinal surgery that is often ignored by clinicians, and its in-depth research is infrequent. In particular, no consensus has been reached about its surgical treatment. We found that patients with a DMC failed after conservative treatment, epidural blood patch, subarachnoid drainage, and initial attempts to eliminate the meningeal cyst. Therefore, we introduce the application of a paraspinal muscle flap repair under a microscope for the treatment of DMC.
METHODS: In this study, 13 patients who were repaired by a paraspinal m

BACKGROUND CONTEXT: Most osteoporotic vertebral compression fractures (OVCFs) are treated conservatively; however, in some patients, progressive vertebral body collapse leads to spinal deformity and cord compression. These complications are strongly associated with impaired performance activities of daily living and a poor quality of life.
PURPOSE: To identify the role of the paraspinal muscle as a risk factor for progressive vertebral body collapse in patients with OVCF.
STUDY DESIGN: This was a retrospective observational study.
