Short lateral posterior condyle is associated with trochlea dysplasia and patellar dislocation.

PURPOSE: Surgeons mainly consider the anterior anatomy of the distal femur in the treatment of patellar instability (PI) with trochlear dysplasia (TD). Through this research, the idea was to analyse the posterior femoral condyle length in TD. The research team posited the presence of morphological differences in the posterior part of the femoral epiphysis in TD compared to a control group. They also postulated that the posterior bicondylar angle (PCA), in the axial plane, was increased in TD. METHODS: This is a single-centre morphological study of 100 patients who had a computed tomography (C

Spontaneous superior patellar dislocation in young age: case report and reduction technique.

Superior patellar dislocation is a very rare pathology, which happens in middle age. We report a case of spontaneous superior patellar dislocation, which occurred at 30 years of age. Differential diagnosis is patellar tendon rupture, which could be reliably excluded by clinical examination and radiographs. Reduction manoeuvres should be tried with diluted intra-articular local anaesthetic for hydrodilatation to gain mechanical advantage. Splinting the knee in gentle flexion is recommended if general anaesthetic is employed. Recurrent dislocation or osteochondral fractures warrants surgical tre

Avulsion fracture of an ossified pes anserinus tendon post-lateral patellar dislocation.

The pes anserinus is a common tendon comprising the tendinous insertions of the sartorius, gracilis, and semitendinosus muscles. It inserts at the anteromedial aspect of the tibia and plays a significant role in stabilization of the medial side of the knee joint. The current article presents a case with recurrent lateral patellar dislocations causing chronic stress along the medial knee stabilizers and consecutive enthesophyte formation at the insertion of the pes anserinus tendon that showed a transverse fracture upon a subsequent incident of traumatic lateral patellar dislocation. Avulsion i

Total knee arthroplasty in a pseudoachondroplastic dwarfism patient with bilateral patellar dislocation.

Late presentation of congenital patellar dislocation with advanced osteoarthritis is rare. This article presents a case of a 59-year-old man with underlying pseudoachondroplastic dwarfism. Advanced osteoarthritis due to bilateral neglected congenital patellar dislocation was treated with total knee arthroplasty without patella relocation surgery. Two years later, the patient had an improvement in Knee Society scores, painless function, and stability.

Recurrent patellar dislocation in adolescents: medial retinaculum plication versus vastus medialis plasty.

BACKGROUND: The clinical outcome of arthroscopic medial retinaculum plication (MRP) compared with vastus medialis plasty (VMP) for recurrent patellar dislocation in adolescents is unknown. HYPOTHESIS: Arthroscopic MRP can yield similar results to open VMP for recurrent patellar dislocation in adolescents. STUDY DESIGN: Randomized controlled clinical trial; Level of evidence, 2. METHODS: Sixty adolescent patients were randomly divided into 2 groups to receive arthroscopic MRP and open VMP respectively. The patients were followed up at 3, 6, 12, and 24 months postoperatively and computed tomo

The "sliver sign": a specific radiographic sign of acute lateral patellar dislocation.

OBJECTIVE: The objective of this work is to assess the prevalence of the sliver sign, defined as an intraarticular linear or curvilinear ossific density, in association with knee effusion in patients with acute knee trauma, as a predictor of recent lateral patellar dislocation (LPD). MATERIALS AND METHODS: A retrospective radiology database search for the term 'patellar dislocation' on MRI knee exams performed at our institution over a 7-year period identified 216 studies. Of these, 142 exams met true positive gold standard diagnostic criteria for LPD. Imaging findings of both the retrospecti

Patellar dislocation in a 16-year-old athlete with femoral trochlear dysplasia.

OBJECTIVE: This case report discusses the diagnosis and conservative management of a patient with femoral trochlear dysplasia (FTD) and subacute patellar dislocation. Femoral trochlear dysplasia is characterized by an abnormally shallow trochlear groove. Disengagement of the patella from the shallow femoral trochlea is common in FTD and is a predisposing risk factor to recurrent patellar dislocation and subsequent premature osteoarthrosis. Accurate diagnosis is necessary to prevent recurrent patellar dislocations and damage of the patellofemoral joint cartilage and supporting structures. CLIN

Knee replacement for osteoarthritis secondary to chronic patellar dislocation and trochlear dysplasia.

Fifteen knees in patients suffering from symptomatic osteoarthritis secondary to chronically dislocated extensor mechanisms have been treated by knee replacement. Five knees had permanently dislocated patellae while 10 knees (four of which had undergone patellectomies) had dislocations of the extensor mechanisms with knee flexion. In fourteen instances satisfactory realignment of the extensor mechanism was obtained. In twelve knees, a substantial functional improvement was achieved. Two knees had persistent pain and one had unexpected tibio-femoral instability.

Patellar dislocation. The long-term results of nonoperative management in 100 patients.

One hundred patients were treated nonoperatively for primary acute patellar dislocations, either by plaster cast (N = 60), by posterior splint (N = 17), or by patellar bandage or brace (N = 23). Follow-up examinations were performed at an average of 13 years later (range, 6 to 26 years). Overall, there were 0.17 redislocations per follow-up year; the redislocation frequencies per follow-up year for each patient group were 0.29, patellar bandages or braces; 0.12, plaster cast; and 0.08, posterior splint. In addition, there were fewer recurrences and subsequent problems (patellofemoral pain or s

Physiotherapy Rehabilitation Post Patellar Dislocation (PRePPeD)-protocol for an external pilot randomised controlled trial and qualitative study comparing supervised versus self-managed rehabilitation for people after acute patellar dislocation.

BACKGROUND: Patellar dislocations mainly affect adolescents and young adults. After this injury, patients are usually referred to physiotherapy for exercise-based rehabilitation. Currently, limited high-quality evidence exists to guide rehabilitation practice and treatment outcomes vary. A full-scale trial comparing different rehabilitation approaches would provide high-quality evidence to inform rehabilitation practice. Whether this full-scale trial is feasible is uncertain: the only previous trial that compared exercise-based programmes in this patient population had high loss to follow-up.

Medial Patellar Retinaculum Cable Transfer With Anchoring Suture Reinforcement and Medial Tightening to Correct Patellar Dislocation in Skeletally Immature Patients.

Surgical treatment of patellar dislocation poses a challenge in skeletally immature patients. The traditional approach of lateral release and medial imbrication carries a high probability of recurrence. Medial patellofemoral ligament reconstruction recently has become a popular procedure for patellofemoral instability, but its broad application in immature skeletal systems is limited by the risk of injury to epiphyseal plate. Herein, we describe an alternative surgical strategy whereby medial patellar retinaculum cable transfer is done in conjunction with anchoring suture reinforcement and med

The Number of Patellar Dislocation Events Is Associated With Increased Chondral Damage of the Trochlea.

BACKGROUND: Patellofemoral instability is associated with chondral injuries to the patella, trochlea, and lateral femoral condyle. Although studies have demonstrated an association between patellar dislocations and chondral injuries, the influence of the number of dislocations on chondrosis is not established. PURPOSE: To elucidate the precise association between the number of patellar dislocation events and the severity of chondral injuries in a multicenter cohort study at the time of patellar stabilization procedures. Neglected Locked Patellar Dislocation in an Adult Female: Report of a Rare Case.

INTRODUCTION: Dislocation of patella is a very common injury which usually reduces spontaneously or can be reduced easily using gentle manipulation. Irreducible patellar dislocations are rare and usually result due to either rotation of patella along the horizontal or vertical axis or due to bony impaction. Neglected locked patellar dislocations are extremely rare injuries presenting additional challenges. CASE REPORT: We are reporting a case of a 24-year female who presented to us 4 months after suffering a knee injury for which she received native treatment initially. On presentation, patie
