Caring for Patients With Severe and Enduring Eating Disorders (SEED): Certification, Harm Reduction, Palliative Care, and the Question of Futility.

Anorexia nervosa is a serious mental illness with a high mortality rate. The body image distortion inherent to this disorder and the impaired judgment and cognition due to malnutrition frequently result in patients refusing treatment. Treatment is most effective if patients are treated early in the course of their illness and undergo a full course of treatment. Involuntary treatment may therefore be both life-saving and critical to recovery. Between April 2012 and March 2016, 109 patients (5.2% of patients admitted to the Eating Recovery Center in Denver, CO) were certified, 39% of whom were t

Development of a text message intervention aimed at reducing alcohol-related harm in patients admitted to hospital as a result of injury.

BACKGROUND: Screening for alcohol misuse and brief interventions (BIs) for harm in trauma care settings are known to reduce alcohol intake and injury recidivism, but are rarely implemented. We created the content for a mobile phone text message BI service to reduce harmful drinking among patients admitted to hospital following an injury who screen positive for hazardous alcohol use. The aim of this study was to pre-test and refine the text message content using a robust contextualisation process ahead of its formal evaluation in a randomised controlled trial. METHODS: Pre-testing was conducte

Comorbid mental disorders in extensively hospitalized patients due to severe self-harm: the association with intellectual developmental disorders.

PURPOSE: Self-harm is a frequent feature of borderline personality disorder. In severe conditions comorbidity across mental disorders is common. Intellectual developmental disorder (IDD) may be an underlying condition with high frequent self-harming behavior. Based on extensively hospitalized severe self-harm patients, we aimed to 1) explore associations between possible IDD (identified by Hayes Ability Screening Index, HASI), and mental health disorders, and 2) investigate the ability to identify possible IDD with a low-effort HASI subscale. We hypothesized that possible IDD was significantly

Clinical harm from failure to deploy personalized medicine for patients with metastatic non-small cell lung cancer.

BACKGROUND: Among the estimated 234,580 patients diagnosed with lung or bronchus cancer in 2024, an estimated 66%, or 154,823 patients, were diagnosed with advanced or metastatic non-small cell lung cancer (mNSCLC). More than half of patients have a genomic variant that can be treated with targeted therapy. Despite widespread evidence supporting the survival benefits of biomarker-driven management of patients with mNSCLC, real-world implementation of precision oncology has not kept pace with recommendations. OBJECTIVE: To quantify the potential survival deficit from underutilization of precis

Frequency of Antibiotic Overtreatment and Associated Harms in Patients Presenting With Suspected Sepsis to the Emergency Department: A Retrospective Cohort Study.

BACKGROUND: Treatment guidelines recommend rapidly treating all patients with suspected sepsis with broad-spectrum antibiotics. This may contribute to antibiotic overuse. We quantified the incidence of antibiotic overtreatment and possible antibiotic-associated harms among patients with suspected sepsis. METHODS: We reviewed the medical records of 600 adults treated for suspected sepsis with anti-methicillin-resistant Staphylococcus aureus and/or antipseudomonal β-lactam antibiotics in the emergency departments of 7 hospitals, 2019-2022, to assess their post hoc likelihood of infection, wheth

No Harmful Effect of Endovascular Treatment before Decompressive Surgery-Implications for Handling Patients with Space-Occupying Brain Infarction.

This study explored short- and mid-term functional outcomes in patients undergoing decompressive hemicraniectomy (DHC) due to space-occupying cerebral infarction and asked whether there is a potentially harmful effect of a priorly performed endovascular treatment (EVT). Medical records were screened for patients requiring DHC due to space-occupying cerebral infarction between January 2016 and July 2021. Functional outcomes at hospital discharge and at 3 months were assessed by the modified Rankin Scale (mRS). Out of 65 patients with DHC, 39 underwent EVT before DHC. Both groups, i.e., EVT + DH

Perspectives on Self-Harm and Suicidal Ideation in Nigeria: A Mixed-Methods Study of Patients, Family Caregivers, Clinicians, and the Public.

BACKGROUND: Suicide and self-harm are global disease burden that contributes significantly to years of lost life and mortality. Despite the increasing rates of suicide and self-harm in Nigeria, this topic is understudied. METHODS: A mixed-methods design was adopted. Study 1 interviewed n = 18 participants (n = 11 clinicians; n = 5 patients with a history of self-harm and suicide ideation; and n = 2 caregivers). All interviews were audio-recorded, transcribed verbatim, and analyzed using IPA. Study 2 surveyed n = 562 non-clinical sample about their tolerance toward self-harm and the data was a

What Should Clinicians and Patients Know About the Clinical Gaze, Disability, and Iatrogenic Harm When Making Decisions?

While clinicians, ethicists, and policymakers are increasingly aware that race, ethnicity, sexuality, gender, and class biases interfere with care provision, disability is not always considered as a confounding factor. This article explores the way embodiment affects personal and professional values. When patients who live with bodies others might not fully comprehend or embrace refuse-or challenge-clinical interventions, they offer real opportunities for clinicians to grasp the central role that embodied experience plays in how patients make health decisions and thereby avoid harming patients

Hepatocellular Carcinoma Risk Scores Predict Patients Under Surveillance at Low Risk of Benefit and High Risk of Harm.

AIMS: Surveillance for hepatocellular carcinoma (HCC) is recommended for patients with cirrhosis. Multiple risk scores aim to stratify HCC risk, potentially allowing individualized surveillance strategies. We sought to validate four risk scores and quantify the consequences of surveillance via the calculation of numbers needed to benefit (NNB) and harm (NNH) according to classification by risk score strata. METHODS: Data were collected on 482 patients with cirrhosis during 2013-2014, with follow-up until 31/12/2019. Risk scores (aMAP, Toronto risk index, ADRESS HCC, HCC risk score) were deriv

Do fluoroquinolone agents produce therapeutic benefits or harmful effects in patients with periodontitis? A systematic review and meta-analysis.

The adjunctive use of fluoroquinolone (FQ) agents in patients with periodontitis produces contradictory results. There has been no meta-analysis performed based on the evaluations of FQ use that would enable making appropriate clinical decisions. Our study aimed to evaluate, via a systematic review and metaanalysis conducted in accordance with the PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) guidelines, the clinical benefits, antimicrobial effects and safety profiles of the FQ agents administered to periodontitis patients under a conventional treatment regime. A harm reduction model to assess the impact of new treatments for pain over standard of care among patients with osteoarthritis.

BACKGROUND: Osteoarthritis (OA) affects millions of adults in the United States and can result in substantial pain, functional impairment, and significant clinical and economic burden. To manage chronic pain associated with OA, treatment guidelines recommend a variety of pharmacologic treatments, including traditional oral nonsteroidal anti-inflammatory drugs (NSAIDs), cyclooxygenase-2 inhibitors (COX-2s), and opioids. While these drug treatments can be effective at pain management, they are also associated with significant clinical and economic burden. New treatments for chronic pain among pa

Personalising add-on treatment with inhaled corticosteroids in patients with chronic obstructive pulmonary disease: a benefit-harm modelling study.

BACKGROUND: Since the benefit-harm balance of adding inhaled corticosteroids to long-acting β2-agonists (LABA) and long-acting muscarinic antagonists (LAMA) for patients with chronic obstructive pulmonary disease is unclear, we evaluated this addition for a range of patient profiles. METHODS: Analyses considered the effects of low-to-moderate doses of inhaled corticosteroids, LABA, and LAMA compared with LABA and LAMA alone, outcome incidences, and preference weights assigned to averted moderate-to-severe exacerbations (benefit) and severe pneumonia, candidiasis, and dysphonia (harm). An investigation of different types of eyewear and face shields in protecting patients and operators from the harmful effects of ultraviolet radiation.

BACKGROUND: Phototherapy and photochemotherapy are commonly used treatment modalities for a range of chronic skin conditions. Whilst undergoing ultraviolet radiation exposure, patients can wear personal protective equipment. Face shields in combination with other forms of skin cover may be worn to provide both skin and eye protection. There are a number of different types of eyewear and face shields available for purchase. Some of these have clear identifications that show the levels of protection provided whilst others may not have any. METHODS: A series of similar experiments has been under

An everyday phrase may harm your patients: the influence of negative words on pain during venous blood sampling.

OBJECTIVE: Venous blood sampling is one of the most common diagnostic medical procedures performed in clinical practice. It has been shown that negatively loaded words may result in negative affective reactions and, consequently, in an increased perception of pain. We aimed to evaluate whether common warnings before venous blood sampling might induce unnecessary pain. METHODS: We included 100 healthy participants (50 females, 50 males) who were randomized to one of the 2 study groups ("sting" vs. "beware"). Directly before insertion of the needle, the participants were warned with either the

Functional consistency across two behavioural modalities: fire-setting and self-harm in female special hospital patients.

BACKGROUND: Fire-setting and self-harm behaviours among women in high security special hospitals may be understood using Shye's Action System Theory (AST) in which four functional modes are recognized: 'adaptive', 'expressive', 'integrative', and 'conservative'. AIMS: To test for relationships between different forms of fire-setting and self-harm behaviours and AST modes among women in special hospital, and for consistency within modes across the two behaviours. METHOD: Clinical case files evidencing both fire-setting and self-harm behaviours (n = 50) were analysed for content, focusing on i

Which Harms to Patients Affect Family-Building Decision-Making in States in Which Legislatures Ban or Restrict Abortion Care?

AbstractAbortion bans and restrictions harm patients and are not evidence based. Bans express poor understanding of the clinical complexity of many patients' need for early intervention and need to avoid mortality or morbidity. Restrictions delay clinically indicated care and avoidable complications and contribute to poor physical and mental health outcomes; they also intrude in family decision-making and contribute to poor neonatal outcomes. This commentary on a case considers how bans and restrictions ignore patients' views about their and their family's quality of life and ignore patients'
