Cross-border access to clinical trials: participation of pediatric patients and language inclusion.

BACKGROUND: Cross-border access to clinical trials in Europe lacks specific regulation. Language diversity in Europe, with 24 official languages, is a key factor that must be considered when including international pediatric patients in multi-country studies. Providing translation enables patients to participate in clinical trials across borders. Specific consideration requires pediatric rare disease clinical trials for conditions with no approved treatment, where only a few countries participate, making cross-border access even more critical. METHODS: We analyzed retrospective data (2011-202

SDF-1/CXCR4 axis participants in the pathophysiology of adult patients with moyamoya disease.

BACKGROUND: Moyamoya disease (MMD) is characterized by an abundance of moyamoya vessels; however, the precise mechanism driving the spontaneous angiogenesis of these compensatory vessels remains unclear. Previous research has established a link between the stromal cell-derived factor-1 (SDF-1)/ CXC receptor 4 (CXCR4) axis and angiogenesis under hypoxic conditions. Nevertheless, the alterations in this axis within the cerebrospinal fluid, arachnoid membranes and vascular tissue of MMD patients have not been fully investigated. METHODS: Our study enrolled 66 adult MMD patients and 61 patients w

Factors influencing participation in psychosocial programming among orthopaedic trauma patients with PTSD.

INTRODUCTION: Post Traumatic Stress Disorder (PTSD) commonly occurs following acute trauma. Post-injury outcomes are negatively impacted by PTSD. Trauma Recovery Services (TRS) programming was developed at our institution in 2013 to provide psychosocial programming that increases patient satisfaction with care and ability to return to work and decreases PTSD symptoms. We sought to identify factors that influence patients' decision to participate in programming. METHODS: Over a 3-year period at a single, urban level 1 trauma center, 172 patients over the age of 18 screened positive for PTSD on

COVID-19 vaccine guidance for patients with cancer participating in oncology clinical trials.

Emerging efficacy data have led to the emergency use authorization or approval of COVID-19 vaccines in several countries worldwide. Most trials of COVID-19 vaccines excluded patients with active malignancies, and thus data on the safety, tolerability and efficacy of the vaccines in patients with cancer are currently limited. Given the risk posed by the COVID-19 pandemic, decisions regarding the use of vaccines against COVID-19 in patients participating in trials of investigational anticancer therapies need to be addressed promptly. Patients should not have to choose between enrolling on oncolo

Participation of Arachidonic Acid Metabolism in the Aortic Aneurysm Formation in Patients with Marfan Syndrome.

Marfan syndrome (MFS) is a pleiotropic genetic disease involving the cardiovascular system where a fibrillin-1 mutation is present. This mutation is associated with accelerated activation of transforming growth factor β (TGFβ1) which contributes to the formation of aneurysms in the root of the aorta. There is an imbalance in the synthesis of thromboxane A2 (TXA2) and prostacyclin, that is a consequence of a differential protein expression of the isoforms of cyclooxygenases (COXs), suggesting an alteration of arachidonic acid (AA) metabolism. The aim of this study was to analyze the participati

Financial incentives to promote cardiac rehabilitation participation and adherence among Medicaid patients.

PURPOSE: Cardiac rehabilitation (CR) improves medical outcomes after myocardial infarction or coronary revascularization. Lower socioeconomic status (SES) patients are less likely to participate in and complete CR. The aim of this study was to test whether financial incentives may increase participation and adherence to CR among lower-SES patients. METHODS: Patients eligible to participate in CR with Medicaid insurance coverage were approached for inclusion. Patients were placed on an escalating incentive schedule of financial incentives contingent upon CR attendance. CR participation was com

Extinction of Fear Generalization: A Comparison Between Fibromyalgia Patients and Healthy Control Participants.

UNLABELLED: Fear learning deficiencies might contribute to the development and maintenance of chronic pain disability. Fear is often not restricted to movements (conditioned stimulus [CS+]) originally associated with pain (unconditioned stimulus), but expands to similar movements (generalization stimuli [GSs]). This spreading of fear becomes dysfunctional when overgeneralization to safe stimuli occurs. More importantly, persistence of pain-related fear to GSs despite corrective feedback might even be more debilitating and maintain long-term chronic pain disability. Yet, research on this topic

[The effect of spouse participation in cardiac rehabilitation program on patients' compliance and exercise level].

BACKGROUND: Exercise-based cardiac rehabilitation programs have proven to be associated with decreased morbidity and mortality after cardiac events. Unfortunately, attendance is often poor, and long term compliance is low. This study examined the effect of the active participation of spouses in the exercise program on the compliance and physical fitness level of their partners. METHODS: During the years 1998-2001, 24 couples joined our cardiac rehabilitation program. The couples were divided into 2 groups on the basis of their medical status. In Group 1--Rehabilitation program (n = 14); one o
