Inferior vena cava filter use and patient safety: legacy or science?

There has been a dramatic increase in vena cava filter (VCF) use over the past 20 years in the absence of evidence that filters provide a net patient benefit or are required in most cases. This increase is largely attributable to the availability of retrievable filters and expanded indications, particularly as primary prophylaxis in patients thought to be at high risk of pulmonary embolism. Substantial variability in VCF use, unrelated to patient clinical factors, has been shown between hospitals, from region to region, and among various countries. Despite the lack of direct evidence for the b

Patient safety in plastic surgery: identifying areas for quality improvement efforts.

Improving quality of health care is a global priority. Before quality benchmarks are established, we first must understand rates of adverse events (AEs). This project assessed risk-adjusted rates of inpatient AEs for soft tissue reconstructive procedures.Patients receiving soft tissue reconstructive procedures from 2005 to 2010 were extracted from the Nationwide Inpatient Sample. Inpatient AEs were identified using patient safety indicators (PSIs), established measures developed by Agency for Healthcare Research and Quality. We identified 409,991 patients with soft tissue reconstruction and 16,

Patient safety considerations concerning the scheduling of emergency-off system tests.

Emergency-off systems (EOS) are essential to the safe operation of medical accelerators and other high-risk equipment. To assure reliable functioning, some states require weekly tests; others permit monthly, tri-monthly or even six-monthly tests, while some do not specify test intervals. We investigate the relative safety of the various test schedules by computing the fraction of time during which a nonfunctional state of the EOS may remain undetected. Special attention is given to the effect of flexibility (i.e., to regulations that specify the number of tests that have to be done in any give

Patient-Reported Outcome Measures in Safety Event Reporting: PROSPER Consortium guidance.

The Patient-Reported Outcomes Safety Event Reporting (PROSPER) Consortium was convened to improve safety reporting by better incorporating the perspective of the patient. PROSPER comprises industry, regulatory authority, academic, private sector and patient representatives who are interested in the area of patient-reported outcomes of adverse events (PRO-AEs). It has developed guidance on PRO-AE data, including the benefits of wider use and approaches for data capture and analysis. Patient-reported outcomes (PROs) encompass the full range of self-reporting, rather than only patient reports col

Kaiser Permanente implant registries benefit patient safety, quality improvement, cost-effectiveness.

BACKGROUND: In response to the increased volume, risk, and cost of medical devices, in 2001 Kaiser Permanente (KP) developed implant registries to enhance patient safety and quality, and to evaluate cost-effectiveness. METHODS: Using an integrated electronic health record system, administrative databases, and other institutional databases, orthopedic, cardiology, and vascular implant registries were developed in 2001, 2006, and 2011, respectively. These registries monitor patients, implants, clinical practices, and surgical outcomes for KP's 9 million members. Critical to registry success is

Enteral feeding misconnections in the NICU: a continuing patient safety threat.

Although the first case of tubing misconnection was reported in 1972, this patient safety issue received little national attention until The Joint Commission issued a Sentinel Event Alert in the spring of 2006. This was followed by a "Money and Policy" article in the New York Times in the summer of 2010 that initiated a public awareness outcry against the healthcare system's delayed responsiveness in creating fail safe solutions. Since that time, many manufacturers have devised oral syringes, feeding tubes, and feeding "systems" for patients in the neonatal intensive care unit, but these devic

Emergency bedside cesarean delivery: lessons learned in teamwork and patient safety.

BACKGROUND: Maternal cardiovascular and pulmonary events during labor and delivery may result in adverse maternal and fetal outcome. Potential etiologies include primary cardiac events, pulmonary embolism, eclampsia, maternal hemorrhage, and adverse medication events. Remifentanil patient-controlled analgesia is an alternative when conventional neuraxial analgesia for labor is contraindicated. Although remifentanil is a commonly used analgesic, its use for labor analgesia is not clearly defined. CASE PRESENTATION: We present an unexpected and unique case of remifentanil toxicity resulting in

Improving the culture of patient safety through the Magnet® journey.

This article outlines how one academic medical center's nursing service has developed programs to improve patient safety and quality outcomes through the use of the Magnet Re-Designation Accreditation Process(R) and a shared governance model. Successful programs have been implemented across the continuum of care. These programs include educational initiatives that increased both the number of nurses with BSN degrees and specialty certifications and also the number of patient-focused initiatives, such as a reduction in central line infection rates, an increase in hand-washing compliance, and a

Redesigning a continence prescription service to improve patient safety and experience.

A review of prescribing practice for continence related products revealed shortcomings in the service that patients were receiving. This led to it being radically redesigned, with the continence service being responsible for prescriptions. The new service model also includes telephone triage before prescriptions are issued. Patient feedback on the new model has been extremely positive. This project won the 2009 Nursing Times continence award.

Impact of a comprehensive patient safety strategy on obstetric adverse events.

OBJECTIVE: We implemented a comprehensive strategy to track and reduce adverse events. STUDY DESIGN: We incrementally introduced multiple patient safety interventions from September 2004 through November 2006 at a university-based obstetrics service. This initiative included outside expert review, protocol standardization, the creation of a patient safety nurse position and patient safety committee, and training in team skills and fetal heart monitoring interpretation. We prospectively tracked 10 obstetrics-specific outcome. The Adverse Outcome Index, an expression of the number of deliveries

Patient safety issues in magnetic resonance imaging: state of the art.

The presence of a static magnetic field (Bo), a radiofrequency field (RF), a dynamic gradient which varies in time and loud noises during an MR examination could increase patient risk. Specifically, a magnetic field could interfere with ferromagnetic material leading to one of the following five dangerous interactions: 1) projectile effect, 2) twisting, 3) burning, 4) artefacts and 5) device malfunction. The projectile effect is when an object is attracted by the magnet with the risk, as reported in literature, of hitting the patient, operators and/or the instrument. Objects which typically ca

New standards to improve catheter stabilization and patient and worker safety.

The 2006 version of the Infusion Nursing Society (INS) Standards of Practice contains important changes to the society's recommendations for catheter stabilization. Some previously recommended stabilization methods are no longer endorsed, and one method -- manufactured catheter stabilization devices -- has emerged as the preferred method over all others. The literature cited in the document suggests that implementation of the revised standards should be an effective risk management strategy where catheter stabilization is concerned.

Erythema multiforme after meningitis vaccine: patient safety concerns with repeat immunization.

A 20-year-old college student developed an immunologic hypersensitivity reaction, erythema multiforme minor, 1-2 weeks after receiving a meningococcal conjugate vaccine. He had no history of erythema multiforme, nor had he received any other vaccine or drug therapy. The temporal relationship between the development of erythema multiforme and the vaccination suggests that the meningitis vaccine probably was the causal agent. The occurrence of this distinct cutaneous reaction, with the potential for a serious complication such as erythema multiforme major or Stevens-Johnson syndrome on rechallen

National analysis of adverse patient safety for events in bariatric surgery.

Identifying risk factors for adverse events after bariatric surgery (BaS) can help define high-risk groups to improve patient safety. We calculated cumulative incidence of adverse events and identified risk factors for these events using validated surgical patient safety indicators (PSIs) developed by the Agency for Healthcare Research and Quality. BaS patients > or =18 years old were identified using the 2002 Nationwide Inpatient Sample. Cumulative incidence at discharge was calculated for accidental puncture or laceration (APL), pulmonary embolus or deep venous thrombosis (PE/DVT), and posto
