The effectiveness of a human patient simulator in the ATLS shock skills station.

BACKGROUND: The use of simulation as a teaching adjunct in Advanced Trauma Life Support (ATLS) has not undergone rigorous psychometric testing. We hypothesized that an advanced, computer-controlled human patient simulator (HPS) would be a useful adjunct to the ATLS shock skills station. MATERIALS AND METHODS: Forty-four PGY-1 residents enrolled in ATLS courses were randomized into control (CTL) and experimental (EXP) groups. All students took a shock-specific pre- and post-test multiple choice question examination (MCQE). The EXP group used the HPS in the shock skills station; the CTL group w

Small group teaching: clinical correlation with a human patient simulator.

The popularity of the problem-based learning paradigm has stimulated new interest in small group, interactive teaching techniques. Medical educators of physiology have long recognized the value of such methods, using animal-based laboratories to demonstrate difficult physiological principles. Due to ethical and other concerns, a replacement of this teaching tool has been sought. Here, the author describes the use of a full-scale human patient simulator for such a workshop. The simulator is a life-size mannequin with physical findings (palpable pulses, breath/heart sounds, blinking eyes, etc.)

Effect of simulated altitude on the visual fields of glaucoma patients and the elderly.

We investigated whether mild hypoxia, such as may be encountered in civilian aircraft, causes temporary visual field defects in elderly persons or temporarily increases pre-existing defects in persons with glaucoma. The central 24-2 program on the Humphrey automated perimeter was used to test visual fields in 3 groups of subjects: 6 with glaucoma; 12 age-matched controls, all of whom were over 44 years of age; and 6 normal subjects under 36 years of age. Visual fields were tested at ground level and at simulated 3000 m (10,000 ft) in an alternating sequence. A repeated measures design of multi

Diagnosis of alcoholism in a simulated patient encounter by primary care physicians.

Although early detection and treatment of alcoholism have been shown to be efficacious, it is widely accepted that primary care physicians often fail to diagnose alcoholism. In this study, a computerized, simulated encounter with an alcoholic patient was used to assess the performance of a randomly selected sample of primary care physicians in diagnosing alcoholism. Of 95 physicians in this study, only 32 percent diagnosed alcoholism with maximal certainty. There was great variability among physicians in the threshold of information needed to diagnose alcoholism. One third of subjects misinter

Excessive Bladder Filling at Simulation Correlates with Longer Treatment Times, More Interruptions, and Inferior Patient Experiences During Prostate Cancer Radiotherapy.

PURPOSE: Bladder filling during prostate radiotherapy (RT) helps displace bowel and reduce bladder dose but can be uncomfortable for patients. We hypothesized that overly full bladders at simulation (sim) are associated with longer treatments, more delays, and worse patient experience. METHODS: We retrospectively analyzed prostate cancer patients receiving fractionated RT over 9 months across 3 treatment centers within an academic network. Bladder diameter at sim was measured from dome to trigone on sagittal CT. We recorded average treatment duration, number of times patients were taken off t

Improving patient and clinician safety during COVID-19 through rapidly adaptive simulation and a randomised controlled trial: a study protocol.

INTRODUCTION: COVID-19 required healthcare systems to iteratively adapt for safe and up-to-date care as knowledge of the disease rapidly evolved. Rates of COVID-19 infections continue to fluctuate and patients without COVID-19 increasingly return to the emergency department (ED) for care. This leads to new challenges and threats to patient and clinician safety as suspected patients with COVID-19 need to be quickly detected and isolated among other patients with non-COVID-19-related illnesses. At the front lines, emergency physicians also face continued personal safety concerns and increased wo

Cone-beam computed tomography-based diagnosis and treatment simulation for a patient with a protrusive profile and a gummy smile.

For patients with bimaxillary protrusion, significant retraction and intrusion of the anterior teeth are sometimes essential to improve the facial profile. However, severe root resorption of the maxillary incisors occasionally occurs after treatment because of various factors. For instance, it has been reported that approximation or invasion of the incisive canal by the anterior tooth roots during retraction may cause apical root damage. Thus, determination of the position of the maxillary incisors is key for orthodontic diagnosis and treatment planning in such cases. Cone-beam computed tomogr

Strengthening Patient-Safety in ERAS Pathways: An Evidence-Informed Framework for Simulation-Based Nursing Practice Development in Acute Surgical Care.

Background: Enhanced Recovery After Surgery (ERAS) pathways depend on nursing-led safety behaviours such as early mobilisation, opioid-sparing analgesia, device minimisation, and reliable discharge teaching to prevent immobility-related, opioid-related, and device-related harms. However, pre-licensure medical-surgical preparation inconsistently embeds these competencies, leaving ERAS delivery and patient-safety vulnerable to variation. Objective: To develop an evidence-informed, practice-development framework that translates ERAS principles into measurable nursing competencies and management p
