Apicoectomy on an incisor tooth of a Victorian koala (Phascolarctos cinereus victor).

An eight-year-old Victorian koala was presented with a discharging mandibular sinus of at least one month duration. On examination, a dental abscess of the right mandibular incisor tooth was found. During the course of endodontic treatment, a size 2 Gates Glidden bur separated from the shank and was lodged in the canal. Due to the anatomy of the tooth the bur could not be removed and an apicoectomy was performed. Following the apicoectomy (follow-up period two years), the periapical pathology resolved.

Nasal Fistula Associated With Dental Infection Following Severe Tooth Injury: A Case Study.

Dental pulp infection is predominantly initiated by anaerobic, gram-negative bacterial flora introduced through dental caries, operative dental procedures, or severe tooth injury, which can lead to pulp necrosis. A 39-year-old man developed an extra-nasal sinus tract on the right side of the nostril following trauma to the right maxillary central incisor. He presented with a chief complaint of a painful mass inside the right nostril after the right maxillary central incisor was traumatized in a fall down the stairs. A diagnosis of a periapical abscess with a sinus tract on the right side of th

Facial sinus tract mimicking cutaneous cyst or scar: unveiling a dental infection.

A 39-year-old man with an unrecognized facial sinus tract under his chin initially presented with an innocuous lesion resembling an acne pimple. The patient's journey began with dermatologic intervention, involving systemic and topical antibiotics. Upon treatment failure, referral to a general dentist revealed a carious lesion on the lower right lateral incisor, prompting further investigation. A subsequent referral to an endodontic specialist led to the diagnosis of a chronic periapical abscess, and an endodontic intervention was initiated. Despite the persistence of intracanal purulent exuda

Apical infection spreading to adjacent teeth: a case report.

This case report describes apical infection on tooth number 24 that spread to adjacent teeth, resulting in devitalized teeth numbers 23 and 25. The 25-year-old Caucasian female patient was referred to the endodontic resident clinic because of uncontrolled apical infection. Root-end surgery and root-end filling of teeth numbers 23, 24, and 25 were performed. The histopathological diagnosis was a periapical cyst; however, the clinical surgical finding of a purulence-filled bone cavity also revealed a periapical abscess. After root-end surgery and regenerative therapy using Mineral Trioxide Aggre

The Resistance of Succedaneous Teeth to Periapical Infection: A Series of Seven Cases.

Pediatric dental procedures are carried out largely to prevent the adverse effects of primary teeth infection on succedaneous teeth (ST). The existing literature on how and when periapical infection (PI) from primary teeth affect ST is conflicted. A series of seven case reports on PI (abscesses and radicular cysts) from primary molars, until the eruption of the premolars, have been illustrated. The resistance of ST to PI based on their developmental stages is also discussed.

21st century endodontics. Part 1.

Root canal treatment techniques probably develop and change more frequently than any other area of dental practice and it can be hard for the busy general dental practitioner to keep up to date. The aim of this series of five papers is to give a thorough review of current teaching and practice. This will encompass the rationale of root canal treatment, guidance on the latest technical procedures for preparation and obturation of the entire root canal system, and consideration of how these principles may contribute to successful treatment. The series will address the following issues: diagnosis

Cervicofacial Actinomycosis and its Management.

Cervicofacial actinomycosis is an invasive destructive infectious syndrome, caused by Gram-positive, branching filamentous bacteria, Actinomyces. Most of the cases are traced to an odontogenic source with periapical abscess and posttraumatic or surgical complications with poor hygiene and immunosuppression as contributing factors. Diagnosis is often delayed because of nonspecific and prolonged symptoms usually mimicking a malignant or a granulomatous lesion. Solitary or multiple abscesses and fistula formation across normal tissue planes accompany chronic draining lesions and may lead to invas

Successful regenerative endodontic treatment in a tooth with incomplete root apex and posttreatment apical periodontitis: A case report.

This case report describes the procedure and outcome of regenerative endodontic treatment (RET) in a tooth with incomplete root apex and posttreatment apical periodontitis. A 44-year-old patient was referred to the endodontist because of a periapical lesion on tooth #21 and a recent episode of acute periapical abscess. On clinical and radiographic examination, this tooth presented with tenderness to percussion and palpation, periapical radiolucent lesion, external apical resorption, and incomplete apex formation. After coronal access, the filling material was removed, and the canal was gently

Dental disease masquerading as suppurative lesions of the neck.

Three female patients ages 16, 16, and 12, and an 8-yr-old male presented with neck pathology/2 with abscesses, 1 with a submandibular mass and 1 with a spontaneously draining abscess. In 2 instances, routine oral cavity examinations including percussion of the teeth, failed to reveal significant pathology. A detailed examination by a dentist, intraoral roentgenograms, and in the first child a sinogram, identified the etiology of the lesions as periapical infections. Review of the literature reveals that these patients characteristically receive multiple surgical procedures and courses of anti

Acute Exacerbation of Chronic Periapical Pathology in a Dentist: An Autobiographical Case Report.

Acute exacerbations of chronic periapical pathology require prompt surgical or endodontic intervention, supported by pharmacotherapy. The role of analgesic selection, escalation, and rotation in acute dental pain remains underexplored. A 39-year-old male dentist with a 27-year history of trauma to the lower left central incisor presented with acute pain of three hours' duration. The tooth was non-vital with a stable periapical granuloma documented for over 15 years. Self-medication began with paracetamol, escalating to ibuprofen, aceclofenac, and etoricoxib, when pain worsened. Amoxicillin and

Successful treatment of periprosthetic joint infection caused by Granulicatella para-adiacens with prosthesis retention: a case report.

BACKGROUND: Granulicatella and Abiotrophia spp. are difficult to detect due to their complex nutritional requirements. Infections with these organisms are associated with high treatment failure rates. We report the first implant-associated infection caused by Granulicatella para-adiacens, which was cured with anti-microbial treatment consisting of anti-biofilm-active rifampin and debridement, exchange of mobile parts and retention of the prosthesis. CASE PRESENTATION: Patient with a history of left hip arthroplasty presented with acute onset of fever, pain and limited range of motion of the l

Odontogenic (hematogenic) or sinusopathy (contiguous) brain abscess: Case report.

Brain abscess is a rare infectious condition, affecting 0.4 to 0.9 per 100,000 individuals annually, with classic symptoms of fever, headache, and neurological deficits. The origin can be contiguous, hematogenous, due to ruptures of brain barriers, or cryptogenic. Dental infections, such as those related to Gemella morbillorum, are atypical, and when related to odontogenic sinusitis, it is normally unilateral. This report describes a case of peculiar brain abscess, of unconfirmed source, possibly involving sinusitis or periapical odontogenic lesion in an immunocompetent young woman. A 22-year-

Cysts and sinuses of the face resulting from dental abscesses.

Cysts and sinuses of the face and neck should always bring to mind the possibility of causative underlying dental disease. This should be investigated by history, physical examination and appropriate roentgenograms of the teeth. Abscesses of the teeth are the most common dental causes for the suspicious facial mass or sinus tract. Appropriate management includes extraction or root canal therapy of the diseases tooth. Any residual facial scar can later be excised and reconstructed as appropriate.

Orthokeratinized odontogenic cysts presenting as a periapical lesion: report of a case and literature review.

INTRODUCTION: Inflammatory cysts, granulomas, abscesses, and fibrous scars represent most periapical radiolucencies. However, other less common lesions, such as orthokeratinized odontogenic cysts (OOCs), can be found at this region, and they deserve to be discussed because the prognosis for an OOC is different from that expected for the ordinary inflammatory periapical diseases. METHODS: An interesting case of OOC associated with a nonvital tooth in a 40-year-old woman is described. After a previous clinical diagnosis of a radicular cyst, the tooth was extracted, and the lesion was enucleated

Endodontic regeneration and tooth revitalization in immature infected sheep teeth.

AIM: To examine the response of immature sheep teeth with infected root canal systems to a commonly used pulp regeneration/revitalization protocol. METHODOLOGY: Immature mandibular right first incisors in four sheep were mechanically exposed and the pulps infected. The mandibular left first incisors remained intact as controls. Five weeks later, the experimental root canals were chemo-mechanically cleaned and dressed with a triple-antibiotic paste for 4 weeks, before bleeding was induced inside the canal by mechanically irritating the periapical tissues. A collagen dressing was packed coronal

Systemic antibiotics for symptomatic apical periodontitis and acute apical abscess in adults.

BACKGROUND: Dental pain can have a considerable detrimental effect on an individual's quality of life. Symptomatic apical periodontitis and acute apical abscess are common causes of dental pain and arise from an inflamed or necrotic dental pulp, or infection of the pulpless root canal system. Clinical guidelines recommend that the first-line treatment for teeth with symptomatic apical periodontitis or an acute apical abscess should be removal of the source of inflammation or infection by local, operative measures, and that systemic antibiotics are currently only recommended for situations wher
