Histological and immunohistochemical analysis of human periapical lesions: a study of TGF-β1 and CD68 markers.

BACKGROUND: Various inflammatory and anti-inflammatory mediators, along with diverse cell types, are implicated in the development and progression of periapical lesions. This work aimed to assess the immuno-expression of transforming growth factor-beta 1 (TGF-β1) and CD68 (a macrophage marker), elucidating their roles and potential correlations. Additionally, histological analysis was conducted to evaluate the intensity of inflammatory infiltrates in chronic periapical lesion samples. METHODS: Tissue samples from fifty individuals with chronic periapical lesions [25 radicular cysts (RCs) and

Development of periapical lesions.

The purpose of the present study was to follow the development of periapical lesions both radiographically and histologically in infected teeth with open and sealed root canals. The mandibular premolars from five adult monkeys were used in the experiment. Sealed infected teeth developed radiographic signs of periapical pathosis significantly earlier than unsealed teeth. Although, histological signs of pathology could be seen periapically at earlier observation periods, sealed teeth consistently developed these changes earlier than unsealed teeth. Furthermore, the histological periapical pathol

Periodontal tissue regeneration including cementogenesis adjacent to dentin-bonded retrograde composite fillings in humans.

The roots of two incisors were treated by apicoectomy and a retrograde dentin-bonded composite filling. Periapical healing was observed a few months later by radiography. Later, due to root fracture and marginal bone resorption, the apical parts were removed including adjacent periapical bone. A histological and scanning electron microscopic examination of the tissue showed reformation of periodontium adjacent to the composite, including reformation of a lamina dura, inserting Sharpey's fibers and cementum deposited in intimate contact with the composite. This finding is in agreement with prev

Inhibiting periapical lesions through AAV-RNAi silencing of cathepsin K.

Dental caries, one of the most prevalent infectious diseases worldwide, affects approximately 80% of children and the majority of adults. Dental caries may result in endodontic disease, leading to dental pulp necrosis, periapical inflammation and bone resorption, severe pain, and tooth loss. Periapical inflammation may also increase inflammation in other parts of the body. Although many studies have attempted to develop therapies for this disease, there is still an urgent need for effective treatments. In this study, we applied a novel gene therapeutic approach using recombinant adeno-associat

Comparative Metabolomics Reveals the Microenvironment of Common T-Helper Cells and Differential Immune Cells Linked to Unique Periapical Lesions.

Periapical abscesses, radicular cysts, and periapical granulomas are the most frequently identified pathological lesions in the alveolar bone. While little is known about the initiation and progression of these conditions, the metabolic environment and the related immunological behaviors were examined for the first time to model the development of each pathological condition. Metabolites were extracted from each lesion and profiled using gas chromatography-mass spectrometry in comparison with healthy pulp tissue. The metabolites were clustered and linked to their related immune cell fractions.

Porphyromonas gingivalis-induced glucose intolerance during periapical lesions requires its LPS throught a Th17 immune response.

This study investigates the role of Interleukin 17 (IL-17) in exacerbating periapical lesions caused by Porphyromonas gingivalis (Pg) lipopolysaccharides (LPS) in the context of metabolic disease and its potential impact on glucose tolerance. Researchers developed a unique mouse model where mice were monocolonized with Pg to induce periapical lesions. After 1 month, they were fed a high-fat diet (HFD) for 2 months to simulate metabolic disease and oral microbiota dysbiosis. To explore the role of LPS from Pg, wild-type (WT) mice were challenged with purified LPS from Porphyromonas gingivalis,

Visualization of clinically silent, odontogenic maxillary sinus mucositis originating from periapical inflammation using MRI: a feasibility study.

OBJECTIVES: Maxillary sinus mucositis is frequently associated with odontogenic foci. Periapical inflammation of maxillary molars and premolars cannot be visualized directly using radiation-based imaging. The purpose of this study was to answer the following clinical question: among patients with periapical inflammatory processes in the maxilla, does the use of magnetic resonance imaging (MRI), as compared to conventional periapical (AP) and panoramic radiography (OPT), improve diagnostic accuracy? METHODS: Forty-two subjects with generalized periodontitis were scanned on a 3 T MRI. Sixteen a

[Evidence-based clinical analysis of tissue regeneration in endodontic microsurgery].

Endodontic microsurgery, a highly effective intervention for complex periapical lesions, is performed to achieve true regeneration of periapical soft and hard tissues, including alveolar bone, periodontal ligament, and cementum. In recent years, various tissue regeneration strategies and prominently guided tissue regeneration have been integrated into endodontic microsurgery to enhance success rates and optimize defect healing. In this article, the necessity and clinical impact of employing tissue regeneration techniques across diverse endodontic microsurgery cases are evaluated by through a s

[Subtotal vital pulp extirpation using different medical preparations (experimental morphological research)].

Pulp stump and periapical tissue reaction to root canal filling with material containing calcium hydroxide and potassium nitrate after subtotal vital extirpation of the pulp was studied in 12 teeth of 6 green monkeys. Use of the material containing potassium nitrate was found to result on the 90th day of the experiment in the formation of a biologic barrier of secondary cement at the dental root apex. Such result was not achieved with calcium hydroxide-containing material. Other advantages of potassium nitrate-containing material were detected, consisting in a more rapid recovery of structural

[Pathogenesis of apical periodontitis and its effects on the body].

During the last 25 years there have been major advances in understanding the etiology, pathogenesis and maintenance of inflammatory processes taking place in the periapical space. Polymicrobial infection of the pulp chamber is of primary importance in initiating periapical inflammation. Egress of bacteria and their antigenes stimulate the immune system to form a granulation tissue around the apical area. Local immune response eliminates excess number of invading organisms. However, in parallel with protective reactions, local activity of immunocompetent cells and their soluble products also co

Histopathological condition of the remaining tissues after endodontic infection of rat immature teeth.

INTRODUCTION: Recently, case reports have shown that immature teeth diagnosed with necrotic pulp and periapical periodontitis can be repaired through a regenerative endodontic procedure. True regeneration depends on the presence of stem cells in the remaining vital tissues. The aim of this study was to evaluate the histologic condition of the pulp tissue, root apical papilla, and periapical tissues after inducing endodontic infection in immature rat teeth for different periods. METHODS: This study evaluated 18 first upper rat molars (36 roots). Periapical lesions were induced and were confirm

Pathogenesis of apical periodontitis and the causes of endodontic failures.

Apical periodontitis is a sequel to endodontic infection and manifests itself as the host defense response to microbial challenge emanating from the root canal system. It is viewed as a dynamic encounter between microbial factors and host defenses at the interface between infected radicular pulp and periodontal ligament that results in local inflammation, resorption of hard tissues, destruction of other periapical tissues, and eventual formation of various histopathological categories of apical periodontitis, commonly referred to as periapical lesions. The treatment of apical periodontitis, as
