BACKGROUND: Idiopathic chylopericardium is a rare disease characterized by filling of the pericardial cavity with chylous fluid and has no evident cause. Secondary chylopericardium usually results from injury or damage to the thoracic duct. The most common causes of secondary chylopericardium are trauma, thoracic or cardiac surgery, and congenital lymphangiomatosis. Conservative or surgical treatment can be pursued; however, surgical treatment is required if conservative treatment is unsuccessful. Pericardiocentesis plays a crucial role in the definitive diagnosis of chylopericardium. However,

BACKGROUND: Pericardial effusion is a late manifestation of HIV more commonly observed in individuals with depressed CD4 counts. Although Mycobacterium tuberculosis remains to be one of the most frequently identified pathogens in the pericardial fluid among people living with HIV, less commonly described etiologies include SARS-CoV-2 that causes coronavirus disease and human herpesvirus-8 which is associated with Kaposi sarcoma. Isolation of more than one pathogen in normally sterile sites remains challenging and rare. We report the first documentation of both SARS-CoV-2 and HHV-8 in the peric

Cardiac tamponade is a life-threatening emergency caused by the accumulation of pericardial fluid, resulting in impaired cardiac filling. Although malignancy is a recognized cause of pericardial effusion, cardiac tamponade is a rare initial presentation of high-grade B-cell lymphoma (HGBCL) and may pose a diagnostic challenge because of its nonspecific clinical manifestations. A 67-year-old man with a history of hypertension, hyperlipidemia, and schizophrenia presented from a group home with acute altered mental status and dyspnea. Initial computed tomography of the head was negative for acute

Purulent pericarditis is characterized by a purulent pericardial fluid, which usually originates from the extension of a nearby bacterial infection site or by blood dissemination. Candida species is a rare cause of pericarditis; and if not treated, it is extremely fatal. In this report, we describe a 54-year-old man who had esophagojejunostomy due to gastric adenocancer 2 months before his admission into our emergency department with dyspnea, orthopnea, chest pain, and somnolence. Physical and echocardiographic examinations revealed massive fibrinous pericardial effusion, causing pericardial t

Objective: We aim to characterize patients undergoing pericardiocentesis for clinically significant pericardial effusion and explore differences in inflammation-based biomarkers according to malignancy status and one-year outcome. Methods: We retrospectively evaluated 113 consecutive patients who underwent pericardiocentesis between January 2021 and January 2026. Clinical, laboratory, pericardial fluid, cytopathological, recurrence, and mortality data were analyzed. NLR, PLR, SII, SIRI, and PIV were assessed as secondary exploratory biomarkers. Results: Malignancy-associated pericardial effusi

Eight (23%) of 35 children with acute pericardial effusions due to infection or juvenile rheumatoid arthritis (JRA) had associated transient pulmonary edema demonstrated on plain chest radiographs. The presence or absence of radiographic pulmonary edema correlated well with clinical and hemodynamic parameters in patients with JRA but not in those with infectious pericarditis. There was no definite relationship between radiographic edema and amount of pericardial fluid as estimated echocardiographically or removed at pericardiocentesis. Rapidity of pericardial fluid accumulation could not be as

BACKGROUND: Tuberculous pericarditis (TP) remains a challenge for endemic countries. In developing countries, one to two percent of patients with pulmonary tuberculosis develops TP.
CASE SUMMARY: A 49-year-old woman presented with dyspnea, chest pain and dry cough. On physical examination, veiled heart sounds were found. The electrocardiogram showed low-voltage complexes and the transthoracic echocardiography revealed a large and free-looking pericardial effusion. The patient was taken for an open pericardiotomy. The pericardial fluid revealed high levels of adenosine deaminase and Ziehl-Neel

Pericardial effusion is an uncommon extra-pulmonary manifestation of tuberculosis, tamponade being even rarer. Here, a 14-year female presented with cough, chest pain and fever. She had raised jugular venous pressure, hypotension, and muffled heart sound, suggestive of cardiac tamponade, confirmed by echocardiogram. She underwent pericardiocentesis with continuous pericardial fluid drainage. Her jugular venous pressure normalized after the aspiration. The high adenosine deaminase level in pericardial fluid analysis was suggestive of tuberculosis for which she was treated with antitubercular th

Pericardial effusions can occur owing to a variety of reasons such as trauma, infection, autoimmune disease, and malignancy. Cardiac tamponade depends on the rate of fluid accumulation and not solely on the volume of the pericardial effusion. Rapid accumulation of pericardial fluid can lead to impaired cardiac filling and output with hemodynamic consequences, requiring urgent or emergent intervention. Despite initial intervention on patients with cardiac tamponade, recurrence of pericardial effusions has been estimated at approximately 20%, with a mean interval to recurrence of approximately 1

Tuberculous pericardial effusion is uncommon in the developed countries. However, it remains one of the main causes of presentation with a pericardial presentation with pericardial effusion in the developing world. We present the case of a 24-year-old male patient who presented with a weekly history of diarrhoea, vomiting, shortness of breath and feeling hot. Chest computed tomography revealed a large pericardial effusion with significant haemodynamic compromise. The patient underwent emergency pericardiocentesis, and the pericardial fluid interferon-gamma assay result was positive for tubercu

Extrameningial manifestations in patients with meningococcal meningitis are rare. One of these manifestations is immune-mediated or purulent pericarditis. We present a case where a patient with meningococcal meningitis developed pericarditis on the 11th day after admission. A pericardial puncture was performed but the pericardial fluid regenerated after two days despite antibiotic treatment. The pericardial fluid showed no bacterial growth and PCR for bacterial DNA was negative. Subsequent steroid treatment proved to be effective, indicating a case of immune-mediated pericarditis.

Pneumopericardium is defined by the presence of air in the pericardial sac. We present a 61-year-old cachectic woman who developed pneumopericardium after pericardiocentesis. She presented with complaints of fatigue and shortness of breath. The chest X-ray showed an increased cardiothoracic ratio, and echocardiographic examination showed a marked pericardial effusion. Pericardiocentesis was performed and a total of 860 ml hemorrhagic pericardial fluid was aspirated. At the end of the first week after removal of the catheter, control chest radiography showed air-fluid levels in the pericardial

BACKGROUND: Dialysate leakage into the pericardium is a rare but potentially life-threatening complication of peritoneal dialysis (PD). There has been one reported pediatric case of spontaneous peritoneo-pericardial fistula in a 2-year-old boy with tissue fragility due to malnutrition and two reported adult cases in PD patients with a history of previous cardiac surgery and/or pericardiocentesis.
CASE-DIAGNOSIS/TREATMENT: We describe a 15-year-old girl with end-stage renal disease secondary to granulomatosis with polyangiitis, with recurrent pericardial effusions secondary to a peritoneo-peri

We report about a patient with purulent pericarditis due to Neisseria meningitidis pretreated with antibiotics. Clinical signs were suggestive of pericardial tamponade. Cultures from blood and pericardial aspirate remained negative. Broad-range polymerase chain reaction from pericardial fluid detected Neisseria sp.. Latex agglutination assay from pleural fluid showed positive reaction with meningococcal antigen serogroup C. Meningococcal pericarditis without meningitis is a rare manifestation. Non-culture based diagnostic methods in patients with such severe infections and negative cultures pl

A 12-year-old boy presented with central chest pain, shortness of breath and type 1 respiratory failure. He had a background of graft versus host disease (GvHD), which was currently managed with imatinib therapy. A focused bedside ultrasound scan was performed revealing a large pericardial effusion. The child was referred to a tertiary paediatric cardiology centre where he underwent emergency pericardiocentesis, draining a total of 800 mL of pericardial fluid. Fluid analysis excluded infection, and with no other concerns for a GvHD flare the diagnosis of an imatinib-induced pericardial effusio

A male fisherman in his late 40s residing in a coastal region of South India, with a 2 year history of diabetes mellitus, presented with breathlessness and chest pain, along with a month-long history of dry cough and low-grade evening fever. Transthoracic echocardiography revealed a large pericardial effusion, and 900 mL of brownish fluid was drained. Given the elevated adenosine deaminase concentration in the pericardial fluid, tuberculous pericarditis was initially suspected. However, Burkholderia pseudomallei was subsequently isolated from the fluid culture, confirming melioidosis. The pati

INTRODUCTION: Pancreatic cancer is a deadly type of cancer with few symptoms until metastasis. It poses a high risk of cancer-associated thrombosis.
CASE PRESENTATION: A 73-year-old male presented with fatigue, shortness of breath, weight loss since 9 months, and blood clots recently in his legs. Chest radiography revealed fluid accumulation in pleural and pericardial cavities. Later, a fluid examination revealed the presence of malignant cells in the pericardial fluid. After immunological tests and an upper gastrointestinal endoscopy were performed, a pancreatic tumour was suspected. The pat

Severe acute respiratory syndrome coronavirus (SARS-CoV-2) emerged from Wuhan, China, in 2019, causing coronavirus disease 19 (COVID-19) and creating a global pandemic affecting millions of people worldwide. Though COVID-19 primarily affects the pulmonary structures, deleterious effects can also occur in the cardiac system. We present a case of a patient with recurrent pericardial effusions secondary to COVID-19 infection, an unusual cardiovascular manifestation of this disease. A 47-year-old man presented with altered mental status and tested positive for COVID-19. He left against medical adv

In many patients conventional radiographic procedures aid the detection of pericardial pathology. Echocardiography is undoubtedly the most satisfactory investigation for assessing pericardial thickening and fluid. However, in special circumstances CT of the mediastinum is able to offer additional information. Small quantities of pericardial air, associated tumor masses, and the extent of a pericardial effusion or pericardial calcification can be displayed most advantageously by CT.

We report a patient who developed adult respiratory distress syndrome following relief of pericardial tamponade. Because of increasing recognition of pulmonary edema in this situation, we recommend gradual removal of pericardial fluid with hemodynamic monitoring to limit the massive fluid shifts which appear to herald this dire complication.
