Malignant renal angiomyolipoma without tuberous sclerosis.

A 68-year-old woman presented with abdominal pain, fatigue, anorexia and night sweats. Imaging studies identified a vascular mass extending from the upper pole of the right kidney with anterior displacement to the abdominal midline, consistent with renal cell carcinoma. A radical nephrectomy and right pelvic lymph node dissection was performed. Pathology determined a grade 4 malignant epithelioid angiomyolipoma that invaded hilar and perinephric adipose tissue. Gross tumour was also present within the renal vein, sinus, pelvis, capsule and perinephric fat. The tumour was HMB-45 positive, suppo

CT urograms in pediatric patients with ureteral calculi: do adult criteria work?

BACKGROUND: Secondary signs of urinary obstruction associated with ureteral calculi are useful adjuncts to diagnosis in adults with renal colic evaluated by unenhanced helical CT. OBJECTIVE: Our purpose was to evaluate the frequency of secondary signs of obstruction in children with renal colic undergoing unenhanced helical CT. MATERIALS AND METHODS: Ureteral calculi were identified in 20 of 61 children with acute flank pain examined by unenhanced helical CT. Each imaging study was evaluated for the presence of secondary signs of urinary obstruction. The frequencies of individual signs were

Ureaplasma urealyticum disseminated multifocal abscesses in an immunocompromised adult patient: a case report.

BACKGROUND: Ureaplasma urealyticum is a fastidious bacteria which lacks a cell wall. Extragenital infections are rare in immunocompetent adults. There are few literature reports of perinephric abscess. We present a case of non-resolving multifocal "culture-negative" abscesses in a hypogammaglobulinemic adult female due to U. urealyticum. CASE PRESENTATION: 66-year-old female with a one-week history of fever, malaise and new right hip and leg pain. Past medical history was notable for chronic pancytopenia secondary to in remission B cell follicular lymphoma, ESRD on intermittent hemodialysis w

Perinephritic and psoas abscess by an unusual coinfection with Trichomonas vaginalis and Lactobacillus johnsonii.

Trichomoniasis, caused by Trichomonas vaginalis, is a common nonviral sexually transmitted infection that presents with vaginitis, urethritis, cystitis, prostatitis, and rarely perinephric abscess. Here we presented a 49-year-old female with fever and lumbago diagnosed with a coinfection of perinephric and psoas abscess caused by Lactobacillus johnsonii and T. vaginalis, with the etiological diagnosis established using pus culture and metagenomic next-generation sequencing (mNGS). The patient achieved complete recovery following abscess drainage, incision, and combined therapy with metronidazo

Evaluating the Association of Nontumor Kidney Perirenal Fat Characteristics With Long-Term Renal Function and Mortality After Radical Nephrectomy.

OBJECTIVE: To evaluate the impact of perinephric fat thickness and stranding around the unaffected kidney on progressive chronic kidney disease (CKD) and renal cell carcinoma (RCC) outcomes after radical nephrectomy. METHODS: We conducted an observational cohort study of 1276 patients with RCC who underwent radical nephrectomy at Mayo Clinic between January 1, 2000, and December 31, 2021. Regression models assessed the risk of progressive CKD and noncancer and cancer mortality with lateral and posterior fat thickness and perinephric fat stranding, adjusting for age, sex, and comorbidities. Ultrasonography-based prediction of carotid artery atherosclerosis using multiple abdominal fat indices.

AIM: Abdominal obesity is recognized as the best predictor of cardiovascular disease risk. While body mass index has traditionally been used to measure obesity, recent evidence suggests that visceral adipose tissue may be a better indicator of cardiovascular disease risk. Various surrogate imaging markers of visceral adipose tissue have recently been described, such as posterior right perinephric fat thickness, preperitoneal fat thickness, and the abdominal wall fat index. This study aimed to examine the link between atherosclerosis through measurement of carotid intima-media thickness and mar

Primary retroperitoneal fibrosis presenting as a renal mass.

A 67-year-old male was incidentally found to have a large left perinephric mass with a poorly functioning left kidney. A differential diagnosis of renal cell carcinoma, lymphoma, retroperitoneal fibrosis (RPF), and IgG4 renal disease was suggested on imaging studies and biopsy of the mass. A left radical nephrectomy was performed as malignancy could not be ruled out. The final diagnosis was RPF without periaortitis and the patient is doing well at 9 months of follow-up. RPF, although characterized as a manifestation of periaortitis and large vessel vasculitis, might also present as an isolated

Wunderlich Syndrome as an unusual presentation of Microscopic Polyangiitis: A Case report with Review of Literature.

Wunderlich syndrome, characterized by spontaneous perinephric hematoma with subcapsular extension has been scarcely reported in microscopic polyangiitis (MPA). We report the case of a 45-year-old woman, who presented with constitutional symptoms, left-eye episcleritis, and rapidly progressive glomerulonephritis. She developed sudden, severe left flank pain with hemoglobin drop two days after admission. Both computed tomography (CT) and non-contrast magnetic resonance imaging revealed large left-sided perinephric hematoma. CT angiography failed to demonstrate intrarenal aneurysms. A remarkable

Spermatic cord abscess: Case report and imaging findings.

Spermatic cord abscess is a rare condition usually associated with other genitourinary infections. We present a case of a 49-year-old male with past medical history of uncontrolled diabetes mellitus. He presented with right groin swelling and pain for a week. Ultrasound showed a tubular hyperemic structure extending into the scrotal sac. Computed tomography showed concurrent perinephric abscess and right spermatic cord abscess. The spermatic cord abscess was managed surgically by incision and drainage. To our knowledge, this is the first case report of a patient with spermatic cord abscess and

Beyond Hydronephrosis: Perirenal Fluid on Screening Ultrasound in Patients With Renal Colic at a Basic Emergency Service.

Screening ultrasound has been progressively integrated into the evaluation of renal colic in emergency settings, enabling rapid identification of indirect signs of urinary obstruction, such as hydronephrosis, and supporting clinical triage when laboratory testing is limited. This article reports a case of recurrent severe renal colic refractory to analgesia, in which screening ultrasound demonstrated moderate unilateral hydronephrosis associated with perinephric/perirenal fluid. We discuss the clinical significance of this combined finding in light of the available evidence, acknowledging that

Hybrid eclipse: a case of isolated renal tuberculosis with an unusual infectious-inflammatory-neoplastic triad.

Isolated renal tuberculosis (RTB) is a rare extrapulmonary manifestation of tuberculosis that often mimics common urinary tract infections, resulting in delayed diagnosis and complications. We report an exceptionally rare case of isolated RTB presenting with a spectrum of destructive sequelae-including a perinephric abscess with a nephrocutaneous fistula, renal replacement lipomatosis (RRL), and synchronous clear cell renal cell carcinoma (RCC) within a single kidney. We report the case of a 77-year-old diabetic man who presented with left loin pain and chronic lumbar sinus discharge. Multipha

Detected Huge Urinoma With 99mTc-PSMA Activity on Initial Prostate Cancer Staging.

A 62-year-old man with newly diagnosed prostate adenocarcinoma underwent initial staging with 99mTc-PSMA and 99mTc-MDP whole-body scans. The bone scan revealed a small zone of tracer activity superior to the left kidney. Subsequent 99mTc-PSMA SPECT/CT revealed a fluid density collection in the left perinephric and pararenal space, extending from the left subdiaphragmatic area to the pelvic inlet. PSMA tracer activity was seen around the left kidney. Contrast-enhanced CT confirmed a perinephric fluid collection consistent with urinoma. This case underscores the importance of recognizing atypica

Effect of castration and cryptorchidism on fatty acid content of ovine adipose tissue.

Fatty acid analyses of perinephric and subcutaneous adipose tissue were made on 50 of Mehraban and Bakhriari breeds lambs to determine the effect of castration and cryptorchidism on ovine lipid composition. The level of stearic acid was significantly higher (p < 0·05) in cryptorchids than in wethers. Rams were also higher (p < 0·05) in the percentage of linoleic acid of the internal adipose tissue than wethers. The percentage of palmitic acid of subcutaneous tissue was higher (p < 0·05) in cryptorchid than rams. There was a tendency toward less saturated (palmitic and stearic) acids, but more

Unenhanced helical CT of ureteral stones: incidence of associated urinary tract findings.

OBJECTIVE: The objective of this study was to determine the incidence of urinary tract findings associated with ureteral stones on unenhanced helical CT scans of patients with acute renal colic. MATERIALS AND METHODS: One hundred forty-one consecutive patients with suspected renal colic were referred by the emergency department for a helical CT scan that was obtained without oral or IV contrast. The CT scans of 54 of these patients were interpreted as positive for ureteral calculi, and these CT scans were reviewed retrospectively. The size and location of each ureteral calculus and of any con

Delayed diagnosis of xanthogranulomatous pyelonephritis in a quadriplegic patient with uncontrolled cutaneous fistula: A case report.

RATIONALE: Xanthogranulomatous pyelonephritis (XGP) is a chronic destructive granulomatous inflammation that is characterized by urinary tract obstruction and invasion of the renal parenchyma. Although rare, XGP can lead to fatal complications, including perinephric inflammation, psoas abscess, and cutaneous fistula. PATIENT CONCERNS: A quadriplegic patient initially presented to the hospital with a chronic open wound and cutaneous fistula. DIAGNOSES: Abdominal computed tomography revealed a renal obstructing stone and enlarged right kidney with a perinephric fluid collection that communicat

The management of psoas abscess.

The presentation and management of psoas abscess was studied prospectively in 5 patients and retrospectively in 4. 3 patients had bilateral abscesses. All patients had back pain and a mass in loin or iliac fossa. 7 patients had no hip findings. One patient had a perinephric abscess and another had radiological features of tuberculosis of the spine. In the other seven no cause for the abscess could be identified. Ultrasonography demonstrated the abscess in all patients; CT scanning done in 5 patients was confirmatory. Drainage was done by an extraperitoneal route. Biopsy of the abscess wall in

Multimodality staging of renal cell carcinoma-pearls & pitfalls.

Accurate staging of renal cell carcinoma (RCC) is critical because small differences in imaging interpretation can alter operative planning, systemic treatment decisions, and prognosis. This educational review highlights the practical role of multimodality imaging in RCC staging, with emphasis on common interpretive pitfalls and reporting strategies that improve clinical decision-making. Contrast-enhanced CT and MRI remain the foundation of local staging, enabling assessment of tumour size, renal sinus and perinephric extension, venous tumour thrombus, and spread beyond the perinephric fascia.
