Internal Fixation of a Lateral Inverted Osteochondral Fracture of the Talus (LIFT) Lesion Using an Innovative Surgical Approach: Inverting the Capsulo-Lateral Fibulotalocalcaneal Ligament (LFTCL)-Fibular Periosteum Complex.

Osteotomy of the distal fibula or anterolateral corner of the tibia is usually required to fix a displaced osteochondral fracture of the talus that is located central to posterior area of the lateral talar dome. However, osteotomy is an invasive procedure and is associated with complications, including nonunion, persistent pain, and hardware-related problems. Lateral inverted osteochondral fracture of the talus (LIFT) lesion is an extremely rare type of displaced osteochondral lesion of the talus. We describe a case in which a LIFT lesion was fixed using an innovative surgical approach, invert

Strength and histological characteristics of periosteal fixation to bone after elevation.

BACKGROUND: Loss of brow elevation in the early postoperative period has been well documented; however, there has been no study quantifying the minimum time necessary to achieve lasting brow elevation. Previous work in our laboratory has demonstrated that complete readherence of periosteum to bone can take 12 weeks to occur after periosteal elevation. The correlation between adherence and the development of strength has never been examined. OBJECTIVE: To correlate histological characteristics of raised forehead periosteum with the strength of the periosteum-bone union postoperatively. SUBJEC

[Part III: free periosteal flaps as a treatment for mandibular osteoradionecrosis].

INTRODUCTION: The vascularized periosteal free flap transposes a non-irradiated soft tissue with neo-angiogenesis, bone induction, and osteogenesis qualities. Its technique and interest are studied for early osteoradionecrosis. MATERIAL AND METHOD: A retrospective study was made between 1992 and 2002. Thirty-four patients were selected among the 59 cases operated for osteoradionecrosis; all patients were classified stage II and I on the Epstein scale. Twenty-six patients were reconstructed using a non-periosteum free flap (bone, muscular or cutaneous flap), and eight patients were reconstruct

[Repairing bone defect with autogenous periosteal pieces in coagulum: experimental study and clinical application].

A 1.5 cm-long bony defect was established in rabbits one each on both sides. The defected area was filled with coagulum of peripheral blood or bone marrow blood, then autogenous pieces of the periosteum was put in the coagulum on one side, while the other side was served as the control without periosteal autograft. The osteogenetic capacity of the periosteum and the influence of different blood coagulum were evaluated by means of X-ray film, histology, the contents of calcium and phosphate in new osteocytes in the defected area, bone tissue morphometry, and computer-assisted image analysis. [Periosteal bone resorption in the area of the metaphysis of growing bone as a precursor of epiphyseal injuries. A polarization optical and scanning electron microscopy study].

Macerated epiphyses of the growing infant bone exhibit an irregular surface which can already be observed at the macroscopic level. Polarizing microscopy demonstrates extended zones of bone resorption on the cortical surface, deductable from numerous lacunae of Howship. Ensuing experimentally induced epiphysiolyses, cortical bone fragments adhering to the inner surface of the periosteum are demonstrable by scanning electron microscopy. Having been torn out of the bone together with the periosteum, these cortical fragments leave corresponding defects on the bone surface. Due to the remodelling

Reconstruction of a functional gliding surface with extensor retinaculum in extensor tendon reconstruction in the digits.

BACKGROUND: Tendon gliding is key to optimal recovery of hand function after complex tendon injuries. Gliding is facilitated by a low-friction gliding surface that is normally provided by the periosteum of the underlying bone. However, significant injuries may damage the periosteum, which must be reconstructed to allow uninterrupted tendon excursion. The absence of the periosteum may compromise a tendon's ability to glide freely, thereby limiting the range of motion. METHODS: Six digits in 4 patients with complex, composite soft tissue defects involving tendon, periosteum, and skin of the han

Wnt/OPG signaling axis drives ectopic mandibular regeneration: Mechanism mediated by tibial periosteal stem cell sheets.

Mandibular reconstruction presents substantial clinical challenges due to the jaw's complex anatomical and functional requirements. This study investigated gene expression differences between tibial and mandibular periosteum to understand their distinct osteogenic properties and evaluate the therapeutic potential of tibial periosteum-derived stem cell (PSC) sheets for mandibular defect repair.Transcriptomic analysis revealed differential expression patterns: tibial periosteum demonstrated higher OPG expression, while mandibular periosteum exhibited increased RUNX2 and BMP-2 expression. Periosteal thickness and cellularity in mid-diaphyseal cross-sections from human femora and tibiae of aged donors.

Due to lack of access in healthy patients, the structural properties underlying the inherent regenerative power and advanced material properties of the human periosteum are not well understood. Periosteum comprises a cellular cambium layer directly apposing the outer surface of bone and an outer fibrous layer encompassed by the surrounding soft tissues. As a first step to elucidating the structural and cellular characteristics of periosteum in human bone, the current study aims to measure cambium and fibrous layer thickness as well as cambium cellularity in human femora and tibiae of aged dono

The potential for vertical bone regeneration via maxillary periosteal elevation.

BACKGROUND: While many studies have been performed on the characteristics and regenerative capacity of long bone periosteum, the craniofacial periosteum remains poorly understood. AIM: The aim of this study was to investigate the potential for a maxillary periosteum tunnelling procedure to induce vertical alveolar bone regeneration. MATERIALS AND METHODS: We employed a murine injury model that activates skeletal stem cells in the periosteum without overtly damaging the underlying cortical bone, preserving the integrity of the long bone and maxilla, and avoiding the introduction of pathologic

Effects of palatine bone denudation repair with periosteal graft on maxillary growth: an experimental study in rats.

BACKGROUND: The long-term goal of cleft palate repair is to provide normal maxillary growth and speech capacity. However, most surgical repairs of cleft palate result in areas of bone denudation on lateral aspects of the hard palate. It is widely acknowledged that palatal bone denudation and subsequent scar contracture resulting from cleft palate surgery can inhibit maxillary growth. METHOD: This study is designed to investigate the effect of the periosteum on growth patterns of the maxilla. A total of 32 three-week-old Sprague-Dawley rats were randomly divided into a control group and three

Parosteal fasciitis of the clavicle.

A rare case of parosteal fasciitis arising from the periosteum of the left clavicle in a 27-year-old woman is reported. Magnetic resonance imaging demonstrated the lesion surrounding the periosteum of the clavicle. The lesion was iso-intense with muscle on T(1)-weighted images and hyperintense on T(2)-weighted images. At surgery, the lesion was discovered to be densely adherent with the periosteum, and excised along with the periosteum. Histopathological examination revealed the proliferation of myofibroblasts in a vague storiform or short fascicular pattern. A large amount of extravasated ery

Biomechanics of Spiral Fractures: Investigating Periosteal Effects Using Digital Image Correlation.

UNLABELLED: Spiral fractures are a frequent clinical manifestation of child abuse, particularly in non-ambulatory infants. Approximately 50% of fractures in children under one year of age are non-accidental, yet differentiating between accidental and abusive injuries remains challenging, as no single fracture type is diagnostic in isolation. The objective of this study is to investigate the biomechanics of spiral fractures in immature long bones and the role of the periosteum in modulating fracture behavior under torsional loading. METHODS: Paired metatarsal bone specimens from immature sheep

A Novel "Microscrew With Tie-Down Sutures" Technique for FGG Anchorage: A Case Report.

The most challenging and time-consuming step in the free gingival graft (FGG) for keratinized mucosa augmentation is the compression suture anchoring the FGG to the periosteum. This article proposed a novel "microscrew with tie-down sutures" technique to anchor the FGG to the recipient site without the traditional trans-periosteum suture. This patient's keratinized mucosa width (KMW) around the healing abutments of teeth #29 and #30 was less than 1 mm. After an apically positioned flap (AFP) was prepared, 2 microscrews were placed at the buccal plate of the alveolar ridge bone, which is the co

Multiple exostosis: a short study of abnormalities near the growth plate.

The pathogenesis of multiple exostosis has been controversial with many theories put forward including the structural/mechanical theory, which emphasizes that the osteochondroma arises in the displaced growth plate cartilage penetrating a defective periosteum. Recently, molecular genetics has offered the neoplastic model with tumor suppressor genes implicated in the development and pathogenesis of exostosis. In this study, we demonstrated the spectrum of histological abnormalities in the developing exostosis present on the surface of the bone at the physis. Seven skeletally immature patients w

Revisiting hemostasis strategy: the impact of excessive monopolar electrocautery on periosteal-driven spinal fusion.

BACKGROUND CONTEXT: The common surgical approach in posterior spinal fusion (PSF) conflicts with basic principles of bone repair and may lead to high failure rates. In recent years, experimental data has shown that the periosteum plays an indispensable role in fracture bone repair both for skeletal stem/progenitor cells (SSPCs) and for angiogenesis promotion. However, the role of the periosteum in spinal fusion remains experimentally poorly defined. Furthermore, current surgical approach often involves electrocautery to achieve subperiosteal dissection. However, the impact of this electrocaute

The effect of partial periosteal division on growth of the long bones. An experimental study in rabbits.

The mode of action of the periosteum in the growth of the long bones was investigated by comparing the activity of the growth plate after hemicircumferential and longitudinal periosteal incisions. Twenty-eight rabbits were divided into three groups. A longitudinal periosteum incision was made on the medial upper tibia in rabbits of Group A, and a hemicircumferential periosteum incision was made in Group B. An incision of the skin and superficial tissue only, similar to the skin incision of Groups A and B, was performed on rabbits in Group C. Certain differences in the development of the right
