Pathophysiology and epidemiology of peripartum cardiomyopathy.

Cardiovascular diseases are a major cause of complications in pregnancy worldwide, and the number of patients who develop cardiac problems during pregnancy is increasing. Peripartum cardiomyopathy (PPCM) is a potentially life-threatening heart disease that emerges towards the end of pregnancy or in the first months postpartum, in previously healthy women. Symptoms and signs of PPCM are similar to those in patients with idiopathic dilated cardiomyopathy. The incidence varies geographically, most likely because of socioeconomic and genetic factors. The syndrome is associated with a high morbidit

Peripartum takotsubo cardiomyopathy after an emergency Caesarean delivery-diagnosis and specialist management.

We report the case of a 32-yr-old primiparous patient who underwent an emergency Caesarean delivery and subsequently developed peripartum takotsubo cardiomyopathy necessitating transfer to a tertiary referral critical care unit and the heart failure service. Takotsubo cardiomyopathy, also called stress cardiomyopathy, is a form of left ventricular dysfunction with distinct wall motion abnormalities in the absence of coronary artery disease. It is considered extremely rare in the peripartum period, most commonly presenting in postmenopausal women with increased myocardial sensitivity to excess

Trauma-focused cognitive behavioral therapy for peripartum-onset post-traumatic stress disorder: a narrative review.

INTRODUCTION: Peripartum-onset post-traumatic stress disorder is a serious condition that warrants prompt diagnosis and treatment due to its adverse health outcomes for mothers and infants. When the trauma is associated with pregnancy and childbirth, it is known as childbirth-related post-traumatic stress disorder. Currently, psychotherapy is considered a first-line treatment for post-traumatic stress disorder and is also recommended for the peripartum population. One of the psychotherapeutic modalities whose effectiveness has been tested in the perinatal population is trauma-focused cognitive

Antimicrobial Component Concentrations in the Milk of Peripartum Goats.

The aim of this study was to investigate changes in milk conditions and antimicrobial components in goats during 8 weeks prepartum and 1 week postpartum. Milk was collected weekly from seven prepartum goats 8 weeks before the due day, immediately, and 1 week after parturition. Milk color scores and the concentration of antimicrobial components (cathelicidin-7 and S100A8) were significantly increased prepartum. There was a significant decrease in milk viscosity, color scores, somatic cell count, and antimicrobial components (lactoferrin and cathelicidin-2, 7) after kidding. However, S100A7 leve

Treatment of Peripartum Depression with Antidepressants and Other Psychotropic Medications: A Synthesis of Clinical Practice Guidelines in Europe.

This study examined (1) the availability and content of national CPGs for treatment of peripartum depression, including comorbid anxiety, with antidepressants and other psychotropics across Europe and (2) antidepressant and other psychotropic utilization data as an indicator of prescribers' compliance to the guidelines. We conducted a search using Medline and the Guidelines International Network database, combined with direct e-mail contact with national Riseup-PPD COST ACTION members and researchers within psychiatry. Of the 48 European countries examined, we screened 41 records and included

Pregnancy, a unique case of heterochronic parabiosis and peripartum cardiomyopathy.

Introduction: A loss of endogenous stem cells capable of tissue repair and regeneration drives the biological process that we recognize as "aging". Recovery of stem cell-mediated repair and regenerative functions in aged animals has been reported in murine heterochronic parabiosis experiments. Objectives: Herein we will review how pregnancy is an unusual form of heterochronic parabiosis, as the placenta prevents the exchange of most blood cells between parabionts. Instead, plasma and its content, including small extracellular vesicles, can readily cross the placental barrier. These nanosized e

[Peripartum cardiomyopathy or takotsubo cardiomyopathy? The clinical case with arterial hypotension and pulmonary edema].

A 29-year old female patient without a history of cardiovascular diseases was admitted on emergency to a surgical hospital with acute calculous cholecystitis in 3 months after uncomplicated term birth. During laparoscopic cholecystectomy, she developed arterial hypotension with pulmonary edema, which required intravenous sympathomimetics. On the next day, after improvement of the condition and stabilization of hemodynamics, cardiac ultrasound showed diffuse left ventricular (LV) hypokinesis with the ejection fraction (EF) of 38 %. Electrocardiogram detected transient left bundle branch block f

Access to medical care globally for patients with peripartum cardiomyopathy: a clinical consensus statement of the Heart Failure Association of the ESC.

Peripartum cardiomyopathy (PPCM) can be a serious condition, presenting with heart failure with reduced ejection fraction towards the end of pregnancy or in the months following delivery. Less than half of the patients fully recover their cardiac function within 6 months of diagnosis, with substantial regional variation. This clinical consensus statement addresses the global and regional heterogeneity of epidemiological data on PPCM, substantial variation in access to medical care, and the contributing factors to poor adherence, as well as the impact of socioeconomic factors. The scope of this

Prognostic value of late gadolinium enhancement in peripartum cardiomyopathy: a systematic review and meta-analysis.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a rare but life-threatening condition occurring during late pregnancy or the postpartum period. Although late gadolinium enhancement (LGE) has prognostic value in other cardiomyopathies, its role in PPCM remains unclear. This study aimed to evaluate the prognostic significance of LGE in PPCM. METHODS: Three electronic databases were systematically searched to identify studies assessing the association between LGE and clinical outcomes in PPCM. Outcomes included reduced left ventricular ejection fraction (LVEF), arrhythmia, heart failure decompen

Prepartum organic selenium supplementation influences postpartum inflammatory response by regulating plasma oxylipin profiles in periparturient dairy cows.

Excessive lipolysis and enhanced inflammation in periparturient dairy cows may lead to metabolic disorders and diseases. Membrane phospholipids and lipid mobilisation-released polyunsaturated fatty acids (PUFAs) can be oxidised into oxylipins that have pro- and/or anti-inflammatory roles. Selenium has immunomodulatory and anti-inflammatory effects, but the mechanisms are not yet fully understood. The objective of this study was to examine the effects of prepartum selenium-yeast (SY) supplementation on the inflammatory response in dairy cows during the periparturient period by regulating plasma

Early detection of subclinical ketosis and clinical diseases in dairy cows using peripartum motion activity.

Early detection of metabolic diseases like subclinical ketosis (SCK) and other disorders is crucial for animal welfare and economic efficiency. Automated monitoring systems become increasingly important here. This study examined motion activity of cows around calving and its suitability for early detection of SCK and clinical disorders. Activity data of 136 cows were collected over two weeks before to three weeks after calving and analyzed alongside blood test results for beta-hydroxybutyric acid (BHBA). Cows were categorized as clinically sick (S) or healthy (H), with group H further subdivid

Association of activin A and postpartum blood pressure in peripartum cardiomyopathy.

BACKGROUND: Activin A has been implicated in the pathogenesis of patients with chronic hypertension and heart failure as well as patients with hypertensive disorders of pregnancy (HDP). Whether activin A correlates with blood pressure in patients with peripartum cardiomyopathy (PPCM) and HDP history has not previously been explored. METHODS AND RESULTS: 82 women with PPCM w/ and w/out HDP or hypertension history were selected for analysis from the Investigations in Pregnancy Associated Cardiomyopathy (IPAC) study. Serum biomarkers and blood pressure were assessed at the time of enrollment (me

Neurobiology of peripartum mental illness.

At least one in seven pregnant or recently postpartum women will experience a mental illness such as an anxiety disorder, depressive disorder, or substance use disorder. These mental illnesses have detrimental effects on the health of the mother, child, and family, but little is known about the hypothalamic and other neural correlates of maternal mental health concerns. The transition to parenthood alone is a time of remarkable neural plasticity, so it is perhaps not surprising that current research is showing that maternal mental illness has unique neural profiles. Furthermore, the neural sys

Beginning to Optimize Peripartum Care for Somalia-Born Women by Evaluating Labor, Postpartum and Neonatal Outcomes in the Somali Population Delivering at a Tertiary Care Center in Vermont.

Vermont has a large population of Somali immigrants and previous research has shown outcome disparities in this population. Labor curves, pregnancy and delivery outcomes information is required and critical to optimize current labor management in an institution that provides obstetrical care for this population. Data were collected for both Somalia-born and non Somalia-born parturients in pre-pregnancy, pregnancy, delivery, neonatal and cervical change categories and were compared to non Somalia-born parturients to observe differences in demographic factors and outcomes as well as labor progre

Peripartum hysterectomy: A 9-year population-based study in Slovakia.

OBJECTIVE: To assess the incidence of peripartum hysterectomy (PH) in Slovakia. Additionally, we wanted to describe reasons for the procedure, associated clinical circumstances, and complications. METHODS: This was a descriptive, population-based study among women who underwent PH in Slovakia between January 2012 and December 2020. Peripartum hysterectomy was defined as surgical removal of the uterus from the peripartum period up to 42 days postpartum. Data were obtained retrospectively from the standardized questionnaires that are completed in Slovakia for any case of PH. The background popu

Peripartum Cardiomyopathy Leading to Multiorgan Dysfunction in a Previously Healthy 30-Year-Old Puerto Rican Woman.

Peripartum cardiomyopathy (PPCM) is a rare and potentially life-threatening cause of systolic heart failure that usually arises in the late stages of pregnancy or the early postpartum period. Its clinical presentation can range from moderate symptoms to severe forms of heart failure, such as cardiogenic shock. However, there is scarce data on severe forms of PPCM leading to multiple organ dysfunction and its management. We report a case of a 30-year-old G2P2 previously healthy Puerto Rican woman postpartum day #56 who presented to our Institution with respiratory symptoms. Further evaluation c

Use of cell salvage in obstetrics in Germany: analysis of national database of 305 610 cases with peripartum haemorrhage.

BACKGROUND: One of the leading causes of maternal death worldwide is severe obstetric haemorrhage after childbirth. Use of intraoperative cell salvage is strongly recommended by international guidelines on patient blood management. Recent data provide strong evidence that use of cell salvage in obstetrics is effective and safe in women with postpartum haemorrhage resulting in fewer transfusion-related adverse events and shorter hospital stay. We retrospectively analysed the use of cell salvage in bleeding women during delivery for a period of 10 yr in German hospitals. METHODS: Data from the
