BACKGROUND: Understanding the trajectory of complications following total pharyngolaryngectomy (TPL) with free jejunal transfer (FJT) is crucial for enhancing patient management and improving surgical outcomes. However, the traditional Clavien-Dindo classification captures only the highest grade of complication and is unable to capture the progression of complications, limiting its utility for longitudinal assessments. This study utilized the comprehensive complication index (CCI) to provide a continuous evaluation of complications over time.
METHODS: This retrospective observational study in

BACKGROUND: One of the challenges after total pharyngolaryngectomy (TPL) is to restore the swallowing function. The aim of this study was to compare swallowing outcomes between patients who underwent reconstruction with jejunum free flap (JFF) and other free flaps (OFFs).
METHODS: This retrospective study included patients who underwent TPL and free flap reconstruction. The endpoints were the evolution of swallowing outcomes during the first five years after treatment assessed by the Functional Oral Intake Scale (FOIS), and outcomes associated with complications.
RESULTS: One hundred and ele

PURPOSE: Early persistent/recurrent hypopharyngeal tumours represent a challenge for surgeons who have to balance the need for oncological radicality and the desire to maintain a functioning larynx with preservation of the patient's quality of life. The aim of this study was primarily to understand the technical feasibility, functional outcomes, and the possibility of obtaining oncological radicality using lateral hypopharyngectomy with laryngeal preservation in early recurrent post-radio/(chemo)therapy hypopharyngeal tumours.
METHODS: Patients with recurrent T1 hypopharyngeal squamous cell c

INTRODUCTION: Given the few reports regarding the salvage methods for managing jejunal necrosis, finding candidates for a meta-analysis or systematic review is difficult. Thus, this study aimed to describe the interventions for jejunal necrosis and investigate important points relating to this condition.
METHODS: The interventions used to treat free jejunal necrosis are external fistula formation with jejunal debridement, secondary reconstruction of the local site, and overall status improvement, and re-free jejunal transfer with removal of the necrotic jejunum. Selecting the optimal procedur

BACKGROUND: Surgical treatment of advanced hypopharyngeal tumors is still a surgical challenge. We report a case of a hypopharyngeal tumor treated with a pharyngolaryngo-esophagectomy (PLE) and laparoscopic gastric tubulization and interposition.
METHODS: A 56-year-old man presented with a relapsing hypopharynx carcinoma, after primary chemoradiation therapy. Preoperative workup showed a stage IV cancer with esophageal invasion and multiple cervical lymph node metastases. Surgical treatment consisted of a cervical phase, with larynx, pharynx, and esophagus dissection, radical lymph node disse

After repeated failures of classic oesophagoplasties (gastroplasty and coloplasty) a last chance reconstruction could rely on a free jejunal transplant. We report an exceptional and original case where digestive tract was re-established between the orostoma and the epigastric antrum. The patient has four neoplasms in the throat, thoracic oesophagus. After circular pharyngolarynectomy and total oesophagectomy, all the usable vascularized plasties had necrosed. The ultimate reconstruction was performed with a free jejunal transplant carrying two vascular bundles revascularized with the cervical

We propose a new technique of wide vertical hemipharyngolaryngectomy which combines resection of laterally localized extended pharyngolaryngeal tumors with immediate microsurgical reconstruction using a radial forearm free flap, including the tendon of the palmaris longus. It eliminates the limitations of resections, which are usually performed to avoid closure difficulties of the resulting defect. In addition, the new technique avoids the use of tumor-neighboring structures for closure, fulfilling the oncological principles of surgery performed in this region. Our preliminary results show goo

The object of this paper is to analyse our experience at the Hospital for Sick Children, Great Ormond Street (London, U.K.) in the management of 10 patients with posterior laryngeal clefts during the past 5 years. Of these 10 patients, 4 had minor clefts not causing significant laryngeal incompetence. The other 6 patients had clefts extending below the cords requiring repair. Other congenital abnormalities were prevalent in this series, particularly oesophageal abnormalities. The frequency of this association is stressed, the problems of diagnosis and management discussed and recommendations m

Eighty-eight cases of cancers of the cervical esophagus were treated at the Institut Gustave-Roussy (France) between 1968 and 1980. They included 67 epitheliomas of the esophageal opening excluding those of the hypopharynx extending into the esophagus, and 21 epitheliomas of the actual cervical portion of the esophagus. Whenever possible, basic therapy was by radiotherapy (64 cases). There were no survivors after 2 years, possible explanations for this poor result being the selection criteria for treatment, the choice of radiotherapy as basic treatment, and the doses employed. A new therapeuti

Out of 129 cancers of the cervical esophagus, 74 (57%) were resected. After laryngopharyngectomy and segmentary esophagectomy, the reconstruction was performed by substernal coloplasty in 18 cases and by free bowel autotransplantation in 18. When total esophagectomy was performed, pharyngogastroplasty was performed in 32 cases and pharyngocoloplasty in seven. The total postoperative death rate was 18.9%. In cases of segmentary resection the three-year survival rate was 14.34% and 15.87% in cases of total esophagectomy.

The gastric pull-through procedure is a relatively new surgical technique for carcinomas of the hypopharynx and cervical esophagus and benign diffuse stricture of the thoracic esophagus, that require total esophagectomy. This technique is original both in the exeretic and reconstructive phase: total esophagectomy is performed by blunt dissection; thoracotomy and pleural damage are avoided. The basic principle of the operation is the gastric transposition into the neck through the posterior mediastenum and a pharyngogastric or esophagogastric anastomosis, according to the type of disease. BACKGROUND: Published clinical data on minimally invasive tracheostomy (MIT) techniques in critically ill patients with prior neck surgery-particularly in the context of retracheostomy following surgical tracheostomy-remain limited. Generally, conventional percutaneous dilatational tracheostomy has not been recommended for such high-risk patients, owing to the potential for critical complications.
METHODS: This study presents data on MIT performed in 11 high-risk patients with previous neck surgery treated in a university hospital intensive care unit (ICU) specializing in hepatic and gastroin

BACKGROUND: Surgery remains the most common therapeutic strategy for Chinese patients with hypopharyngeal squamous cell carcinoma (HSCC), yet the indications for postoperative adjuvant treatment (POAT) remain unclear.
METHODS: We retrospectively analysed 385 patients with primary HSCC in our hospital between 2003 and 2014. Patients that received pharyngectomy without POAT were enrolled in this study for developing a nomogram that predicts their survival outcome.
RESULTS: Multivariate analyses showed that the tumour size, oesophagal invasion, extracapsular spread or internal jugular vein adhe

OBJECTIVES/HYPOTHESIS: Determine the rate, diagnoses, and risk factors associated with 30-day nonelective readmissions for patients undergoing surgery for oropharyngeal cancer.
STUDY DESIGN: Retrospective cohort study.
METHODS: We analyzed the Nationwide Readmissions Database for patients who underwent oropharyngeal cancer surgery between 2010 and 2014. Rates and causes of 30-day readmissions were determined. Multivariate logistic regression was used to identify risk factors for readmission.
RESULTS: Among 16,902 identified cases, the 30-day, nonelective readmission rate was 10.2%, with an

BACKGROUND AND OBJECTIVE: A pharyngocutaneous fistula is the most common complication after total laryngectomy. In Germany, a traditional recommendation is to use a nasogastric tube for feeding for 10-14 days postoperatively because many surgeons believe that oral feeding after surgery contributes to fistula development. However, there is no international agreement about when to begin oral feeding after total laryngectomy. Some authors begin oral feeding between the 1st and 4th postoperative day without any nasogastric tube, while others using a nasogastric tube delay oral feedings until 7-14
