Pancreatic ascites and pleural effusions.

Two cases of pancreatic ascites are presented and reviewed, together with 92 cases of internal pancreatic fistula reported in the recent literature. Alcohol abuse is the predominant aetiological factor, and chronic pancreatitis with an associated pseudocyst the most common pathological finding. The diagnosis depends on clinical suspicion and can be confirmed by the estimation of amylase and protein levels in the aspirated fluid. Medical treatment includes the aspiration of fluid accumulations, inhibition of pancreatic secretion, and nutritional augmentation. The use of pancreatography is recom

A refractory pleural effusion caused by a pleural capillary hemangioma.

A 69-year-old man presented with a left pleural effusion. Even after repeated drainage, the pleural effusion had been increasing for more than two years. Thoracoscopy unexpectedly showed a pleural mass on the parietal pleura, and it was completely removed. The diagnosis was pleural capillary hemangioma, and the effusion has not recurred after the resection. Pleural hemangioma is one of the crucial differential diagnoses of refractory pleural effusion.

Pleural and serum markers for diagnosis of malignant pleural effusion.

BACKGROUND: Differentiation between benign and malignant exudative pleural effusion remains a clinical challenge. Recently, several markers have been reported to increase the diagnostic accuracy of malignant pleural effusion, with controversial results. METHODS: Patients with exudative pleural effusion were divided into 2 groups: a malignant pleural effusion group (39 patients) diagnosed by malignant cells in pleural fluid cytology or by malignant infiltration of the pleura on pleural biopsy, and a benign pleural effusion group (51 patients) with neither malignant cells in pleural fluid cytol

Indwelling Pleural Catheter Placement for Nonmalignant Pleural Effusions.

Pleural effusions account for significant symptoms and morbidity. Recent studies demonstrate a high mortality in patients with "benign" pleural effusions, now better characterized as nonmalignant pleural effusions (NMPEs) based on their prognosis. The most common nonmalignant clinical conditions with recurrent pleural effusions are congestive heart failure and hepatic hydrothorax, although many other diseases exist in isolation or as comorbid conditions. When conventional therapy fails, thoracentesis is often performed for relief of dyspnea. Many times, however, the effusions recur despite max

[The role of thoracoscopy in pleural biopsy in cases with pleural effusion].

Between April 1985 and July 1989, 125 cases with pleural effusion were admitted to our department. The causes of pleural effusion were carcinomatous pleurisy in 47 cases, infection without tuberculosis in 34 cases, tuberculous pleurisy in 17 cases, cardiac insufficiency and hypoproteinemia in 11 cases, trauma and pneumothorax in nine cases, collagen disease in two cases and unknown origin in five cases. Carcinomatous pleurisy and tuberculous pleurisy, the differential diagnosis of which is very important, comprised 37% and 14% of all cases, respectively. These diseases can be definitively diag

A scoring model based on the pleural effusion adenosine deaminase-to-serum C-reactive protein ratio for differentiating tuberculous pleural effusion from non-tuberculous benign pleural effusion.

BACKGROUND: Differentiating tuberculous pleural effusion (TPE) from non-tuberculous benign pleural effusion (non-TB BPE) can be challenging, especially in patients with low levels of pleural effusion adenosine deaminase (pADA) and negative etiological evidence. This study aimed to evaluate the diagnostic performance of the pADA to serum C-reactive protein ratio (pADA/sCRP) and to develop a scoring model for diagnosing TPE. METHODS: This retrospective study included 364 patients with pleural effusion, comprising 121 with TPE and 243 with non-TB BPE from Peking Union Medical College Hospital. C

Affordable use of indwelling pleural catheters for malignant pleural effusion.

BACKGROUND AND OBJECTIVE: Indwelling pleural catheters (IPC) relieve symptoms in malignant pleural effusion (MPE), while maintaining a reasonable quality of life. However, the high cost of the catheter and the drainage bottles limits their use in low- and middle-income countries. The objective of this study was to determine the time to pleurodesis, feasibility and safety while reusing a low-cost adaptor kit for IPC drainage in a retrospective cohort of patients with MPE. METHODS: Consecutive patients undergoing IPC insertion over 5 years at a tertiary cancer center were included. IPCs were pr

[Pancreatic pleural fistula with bloody pleural effusion: a case report].

Pancreaticopleural fistula (PPF) is a rare complication of chronic pancreatitis that is often overlooked in the initial differential diagnosis of pleural effusion, resulting in delays to diagnosis and treatment. This manuscript reported a case of a patient with recurrent hemorrhagic pleural effusion, in whom markedly elevated levels of amylase and lipase in the pleural fluid led to a diagnosis of pancreaticopleural fistula via magnetic resonance cholangiopancreatography (MRCP). This case aimed to raise awareness among clinicians about the importance of considering uncommon etiologies in the di

Syringo-pleural shunt: a rare cause of recurrent pleural effusion.

case of a patient with reported spinal tuberculosis (TB) that developed syringomyelia, a rare sequelae of the disease is presented in this report. He underwent syringo-pleural shunting for syringomyelia. After 15 years, the patient presented with recurrent pleural effusion. Diagnostic thoracentesis revealed fluid of transudative nature. Pleural fluid was positive for beta-2 transferrin. Pleural biopsy was negative for any malignancy or TB. Computed tomography scan focusing on the shunt showed that it was undisplaced. Video-assisted thoracoscopic exploration showed the distal tip of the shunt a

Recurrent pleural effusion: who benefits from a tunneled pleural catheter?

Recurrent malignant pleural effusion (MPE) is a common concomitant phenomenon of malignant disease, which can worsen the patient's quality of life and lead to significant morbidity. Tunneled indwelling pleural catheters (TIPC) offer new modalities in patients with recurrent MPE and impaired dilatability of the lung. We report on our experience with 100 consecutive patients suffering from recurrent benign (n = 12) and malignant pleural effusion (n = 88) who were treated with TIPC. The catheter was placed during a VATS procedure or under local anesthesia in an open technique. The median residenc

Rapidly filling pleural effusion due to a subarachnoid-pleural fistula.

Only five cases of subarachnoid-pleural fistula in children have been reported previously. All had a history of trauma and associated neurologic deficit which gave clue to the diagnosis. The authors present a child who presented with a massive, rapidly filling, clear pleural effusion in whom there was no neurologic deficit. The history of an automobile accident in the past had almost been forgotten. A subarachnoid-pleural fistula was discovered on exploratory thoracotomy. The entity should be thought of, and actively investigated, in a child with a history of trauma and a pleural effusion of o

Myelomatous Pleural Effusion: A Case Report.

Multiple myeloma is a relatively common hematologic malignancy; however, serous cavity effusions occur infrequently, and bona fide myelomatous effusions are exceedingly rare, typically representing an advanced, biologically aggressive variant associated with dismal outcomes. We report a compelling case of a 50-year-old patient with established multiple myeloma who developed leukemic transformation with concurrent pleural cavity infiltration while receiving systemic chemotherapy. The patient exhibited rapidly progressive dyspnea, a nonproductive cough, and debilitating fatigue in the days prece

Pleural effusion associated with clozapine treatment.

Clozapine is an effective antipsychotic for treatment-resistant schizophrenia but is associated with a range of adverse effects. We present the case of a patient in her 60s with treatment-resistant schizophrenia who developed dose-dependent eosinophilia and bilateral pleural effusion shortly after clozapine initiation. Extensive diagnostic evaluation excluded infectious, cardiac and thromboembolic causes. Clozapine was discontinued and replaced with an alternative antipsychotic, resulting in complete clinical and radiological resolution of the pleural effusions, along with normalisation of eos
