Bridging the gap between development of point-of-care nucleic acid testing and patient care for sexually transmitted infections.

The incidence rates of sexually transmitted infections (STIs), including the four major curable STIs - chlamydia, gonorrhea, trichomoniasis and, syphilis - continue to increase globally, causing medical cost burden and morbidity especially in low and middle-income countries (LMIC). There have seen significant advances in diagnostic testing, but commercial antigen-based point-of-care tests (POCTs) are often insufficiently sensitive and specific, while near-point-of-care (POC) instruments that can perform sensitive and specific nucleic acid amplification tests (NAATs) are technically complex and

The SPaCE diagnostic: a pilot study to test the accuracy of a novel point of care sensor for point of care detection of burn wound infection.

BACKGROUND: Wound infection in burn patients is common and has an impact on outcomes. There is no objective method to diagnose infection at point of care (PoC). Early diagnosis prevents progression to sepsis. Diagnostic subjectivity supports over-diagnosis, unnecessary hospitalization, and antibiotic overuse. AIM: This pilot study aimed to investigate the accuracy of a novel PoC wound infection diagnostic in burn patients. METHODS: We produced, and in vitro tested, a PoC diagnostic for early wound infection diagnosis. The prototype SPaCE diagnostic uses a patented lipid vesicle suspension in

Perspectives of clinicians caring for people experiencing homelessness on point of care tests: an online survey.

OBJECTIVES: People experiencing homelessness (PEH) face multiple barriers to seeking healthcare and have poorer health outcomes. Point of care tests (POCTs) provide a potential solution to improving access to diagnostics for this population. This survey aimed to understand if these technologies could address unmet needs in primary care for PEH. DESIGN AND SETTING: An online survey was circulated via dedicated inclusion healthcare newsletters to professionals providing community-based care for PEH in England. The survey focused on experiences of diagnostics and opinions on the use of POCTs for

Pro: Point-of-care Viscoelastic Testing Should Be Routinely Used in All Cardiac Surgery Patients.

This "Pro" argument supports the routine incorporation of point-of-care viscoelastic testing (VET) into perioperative bleeding management algorithms for all cardiac surgery patients. This group of patients remains among the highest consumers of allogeneic blood products, and transfusion is independently associated with increased morbidity, mortality, and cost. Standard laboratory tests have significant limitations in diagnosing coagulopathy and are not designed to guide intraoperative bleeding management. VET enables rapid identification of specific coagulation abnormalities and supports targe

Saliva point-of-care testing for suboptimal levofloxacin and linezolid exposure in multidrug-resistant tuberculosis.

BACKGROUND: Therapeutic drug monitoring may optimise treatment outcomes in patients with multidrug-resistant tuberculosis, yet current methods are invasive and resource intensive. This study evaluated the feasibility of using saliva point-of-care testing to screen for suboptimal exposure to levofloxacin and linezolid. METHODS: We conducted a community-based cross-sectional study in Vietnam (2021-2023). Drug concentrations were measured in paired plasma and saliva samples at 0, 2 and 5 h post-dose, using a point-of-care test (NanoPhotometer NP80) in comparison with liquid chromatography-mass s

Point-of-care testing for diagnosing hypofibrinogenemia in postpartum hemorrhage: Systematic review and meta-analysis.

BACKGROUND: Postpartum hemorrhage (PPH) can rapidly cause hypofibrinogenemia, requiring prompt coagulation factor replacement. Point-of-care testing (POCT) is gaining interest for early diagnosis for hypofibrinogenemia, despite limited evidence. This systematic review evaluated the diagnostic accuracy of POCT for hypofibrinogenemia in PPH. MATERIALS AND METHODS: A literature search was conducted using MEDLINE, Embase, Cochrane, and Web of Science. Studies using POCT for PPH diagnosis, both observational and interventional, were included. Risk of bias was assessed using the QUADAS-2 tool. A me

One-Pot Assay Based on CRISPR/Cas13a Technology for HEV RNA Point-of-Care Testing.

Hepatitis E virus (HEV) poses a serious threat to both public health and animal food safety, thereby highlighting the demands for rapid, sensitive, and easy-to-use detection. This study aimed to develop a One-Pot assay using CRISPR/Cas13a for detecting HEV RNA, suitable for point-of-care testing (POCT) in resource-limited settings. CRISPR/Cas13a combined with reverse transcription polymerase chain reaction (RT-PCR) and reverse transcription recombinase-aided amplification (RT-RAA) was applied to a One-Pot assay device. Additionally, a large cohort of HEV-infected patient (154) and animal (104)

Temporal discrepancies in "rapid" HIV testing: explaining misdiagnoses at the point-of-care in Zimbabwe.

BACKGROUND: Rapid diagnostic tests have revolutionized the HIV response in low resource and high HIV prevalence settings. However, disconcerting levels of misdiagnosis at the point-of-care call for research into their root causes. As rapid HIV tests are technologies that cross borders and have inscribed within them assumptions about the context of implementation, we set out to explore the (mis)match between intended and actual HIV testing practices in Zimbabwe. METHODS: We examined actual HIV testing practices through participant observations in four health facilities and interviews with 28 r

Newborn Screening for Sickle Cell Disease Using Point-of-Care Testing in Low-Income Setting.

BACKGROUND: Newborn screening provides early diagnosis for children with sickle cell disease (SCD), reducing disease-related mortality. We hypothesized that rapid point-of-care (POC) Sickle SCAN would be reliable in Haiti and would assist newborn screening. METHODS: Dried blood specimens were obtained from infant heel sticks and analyzed by isoelectric focusing (IEF) at a public hospital in Cap-Haïtien during a 1-year period. A total of 360 Guthrie cards were also analyzed for quality assurance by high-performance liquid chromatography at the Florida Newborn Screening Laboratory. In addition,

Progress and Perspectives in Point of Care Testing for Urogenital Chlamydia trachomatis Infection: A Review.

Worldwide, genital infection with Chlamydia trachomatis (C. trachomatis) is one of the most common sexually transmitted infections. Most infections are asymptomatic. However, particularly in women, untreated infection with C. trachomatis can lead to complications that include pelvic inflammatory disease, infertility, and tubal ectopic pregnancy. Rapid methods for early and accurate diagnosis for infection with C. trachomatis that can be performed in the clinic would allow for earlier treatment to prevent complications. Traditional laboratory-based tests for C. trachomatis infection include cul

A decentralised point-of-care testing model to address inequities in the COVID-19 response.

The COVID-19 pandemic is growing rapidly, with over 37 million cases and more than 1 million deaths reported by mid-October, 2020, with true numbers likely to be much higher in the many countries with low testing rates. Many communities are highly vulnerable to the devastating effects of COVID-19 because of overcrowding in domestic settings, high burden of comorbidities, and scarce access to health care. Access to testing is crucial to globally recommended control strategies, but many communities do not have adequate access to timely laboratory services. Geographic dispersion of small populati

Point-of-Care Hemostatic Testing in Cardiac Surgery: A Stepped-Wedge Clustered Randomized Controlled Trial.

BACKGROUND: Cardiac surgery is frequently complicated by coagulopathic bleeding that is difficult to optimally manage using standard hemostatic testing. We hypothesized that point-of-care hemostatic testing within the context of an integrated transfusion algorithm would improve the management of coagulopathy in cardiac surgery and thereby reduce blood transfusions. METHODS: We conducted a pragmatic multicenter stepped-wedge cluster randomized controlled trial of a point-of-care-based transfusion algorithm in consecutive patients undergoing cardiac surgery with cardiopulmonary bypass at 12 hos
