[Environment and child health: from health transition to shared risk?].

Children under the age of 18 account for almost half of the world's population, with most living in developing countries. Young people are especially sensitive to acute and chronic environmental conditions and 43% of environmental diseases occur in the 12% of the world's population under age 5. The main environmental threats to the health of children in developing countries are inadequate access to clean water for drinking and hygiene, exposure to air pollution: primarily indoors and secondarily outdoors, risk of accidents and wounds, and poisoning due to toxic products. Recent data suggest th

Characterizing multi-criteria decision analysis, risk stratification, and hotspot analysis methods to optimise health policymaking and evidence translation for medicines in low- and middle-income countries: A scoping review.

BACKGROUND: Implementing new medicines or health interventions in lower-income and middle-income countries (LMICs) may be complex and challenging, and there is a need for practical structured approaches and tools to support the translation of evidence into practice, including the implementation of evidence-informed interventions and health policy making. OBJECTIVE: This scoping review aimed to characterise three data analytical methodologies - multi-criteria decision analysis (MCDA), risk stratification, and hotspot analysis - to evaluate their use in guiding the implementation of medicines f

Digital inclusion in China's national smart health and older adult care policy design: an access-oriented and uneven embedding pattern, 2013-2025.

BACKGROUND: Population ageing and rapid digitalisation of health and care services have raised concerns about whether older adults are adequately considered in digital policy design. Although digital inclusion is an increasingly visible policy goal, little is known about how national policy instruments embed digital inclusion or respond to older adults' multidimensional needs. METHODS: We developed a Policy Alignment Framework linking policy instruments, digital inclusion orientations, and older adults' needs. A quantitative content analysis was conducted on 35 national-level smart health and

Capacity for national health policy modelling, Bahrain, Kuwait, Oman, Qatar, Saudi Arabia, United Arab Emirates.

Gulf Cooperation Council countries (Bahrain, Kuwait, Oman, Qatar, Saudi Arabia and United Arab Emirates) have similar governance structures, economic profiles and health system organization, creating an opportunity for collective action on health. Similarly, these countries have the same challenges including ageing populations, climate and heat risks, immigration, service delivery and disease profiles. We argue for the need to shift from opinion-driven policy discourse to computer-based, decision-focused policy analysis that is participatory, aware of inherent uncertainties surrounding decisio

"We were caught sleeping": formulation of national policy to decentralise management of drug-resistant tuberculosis in South Africa - a qualitative policy analysis study.

OBJECTIVE: In response to growing numbers of people with drug-resistant tuberculosis (DR-TB), South Africa was, in 2011, among the first countries to launch a national policy on the decentralisation of DR-TB management, preceding normative guidance from the World Health Organization. The objective was to understand the process of formulation of the South African policy on DR-TB service decentralisation. DESIGN: Qualitative policy analysis. We reviewed policy-related documents to create a timeline, then conducted in-depth interviews with key individuals who had influence over or insights into

Beyond partisan rhetoric: Institutional constraints and local COVID-19 policies in Brazil.

BACKGROUND: Political polarization can influence policy responses, particularly during crises. While institutions aim to counterbalance these effects, their capacity to do so under stress remains underexplored. This study examines whether mayoral partisan ideology affected pandemic policymaking in Brazilian municipalities, a highly polarized context with a severe COVID-19 health burden. METHODS: We used a regression discontinuity design in 599 closely contested mayoral elections in 2016 to identify causal effects of partisan ideology on municipal pandemic responses in 2020. The analysis focus

A scoping review of national policies for hierarchical medical system in China since the 2009 health reform.

BACKGROUND: Universal health coverage (UHC) is driving the global health agenda. In 2009, the Chinese government launched a new round of healthcare reform toward UHC, and made remarkable progress in UHC through the establishment of a hierarchical medical system (HMS). We aim to summarize and review the evolution of China's hierarchical medical system policy. METHODS: Eligible policies were identified between March 7, 2009 and December 31, 2022, by searching the official websites of the PKULAW Database, the Chinese State Council, and related ministries. Policy screening and data extraction wer

Actor sensemaking and its role in implementation of the decentralized drug-resistant tuberculosis policy in South Africa.

South Africa has a high burden of drug-resistant tuberculosis (DR-TB). A policy to decentralize DR-TB treatment from specialized central hospitals to more accessible district facilities was introduced in 2011, but to date implementation has been suboptimal, with variable pace, coverage, and models of care emerging. This study explored multilevel policy implementation of DR-TB decentralization in two provinces of South Africa, Western Cape and KwaZulu-Natal. Applying interpretive policy analysis, this paper describes how actors across health system levels and geographies made sense of the DR-TB

Towards a social harm approach in drug policy.

In this paper, we explore how the social harm approach can be adapted within drug policy scholarship. Since the mid-2000s, a group of critical criminologists have moved beyond the concept of crime and criminology, towards the study of social harm. This turn proceeds decades of research that highlights the inequities within the criminal legal system, the formation of laws that protect the privileged and punish the disadvantaged, and the systemic challenge of the effectiveness of retribution and punishment at addressing harm in the community. The purpose of this paper is to first identify parall

The Epidemic Diseases Act (1897): A study of international and domestic pressures on British epidemic policy formation in India.

The Epidemic Diseases Act (EDA) was enacted in February 1897 by the Government of India to prevent and control the spread of the plague. Since then, the Act has become a key legal tool for the control of epidemics/pandemics in India. We attempted to understand the international and domestic pressures that led to the adoption of the EDA in three ways. First, we analyse the legislative structure (Bombay Municipal Act of 1888, Indian Railways Act of 1890, and Act I of 1870) that dealt with infectious or contagious diseases in colonial India before the EDA came into force. Second, we focus on the

Youth participatory research evidence to inform health policy: a systematic review protocol.

INTRODUCTION: Young people's participation in health research produces knowledge that is indispensable for creating appropriate and effective policies. However, how best to disseminate youth participatory research evidence to impact health policy is not known. Therefore, the objectives of this systematic review are to describe the evidence produced through youth participatory research, including the strategies used to disseminate youth participatory research evidence to health policymakers. These are necessary to improve policymakers' use of youth participatory research evidence and, thereby,

Alcohol use during pregnancy: considerations for Australian policy.

Although there is an extensive recorded history of concerns related to alcohol exposed pregnancies and possible outcomes of fetal alcohol spectrum disorder in recent scientific literature, Australia has only recently begun to accurately or systematically diagnose and record these conditions, or to provide comprehensive, coordinated, policy-guided funding, prevention, and treatment. This article discusses some considerations that can guide policy development within the Australian context including the social context and determinates of alcohol consumption during pregnancy and the need to consid

Choosing policy instruments for controlling ozone depleting substances in a developing context: the case of Chile.

Chile ratified the Montreal Protocol in 1990 which sets out an agenda for the reduction and then elimination of ozone depleting substances (ODS); however, by 1998 the country had not yet defined a strategy to encourage greater compliance, so that in the same year the National Environmental Commission decided to examine the policy options available. This paper examines the process followed to develop this strategy. As a first step it was necessary to look at how far the implicit policy of "business as usual" could be stretched without jeopardizing Chile's compliance obligations. A second step i

The One Health national policy network to combat antimicrobial resistance in Senegal.

Antimicrobial resistance (AMR) is a health threat globally and particularly in low- and middle-income countries. Senegal, like many other African countries, faces significant challenges in addressing the AMR crisis. This article aims to investigate the policy-making process that supports efforts to combat AMR in Senegal since the adoption of the country's 2017 National Multisectoral Action Plan Against Antimicrobial Resistance. A survey was conducted between January and August 2023 with key organizations involved in the policy-making process related to AMR at the national level. We collected b

Strong Bipartisan Support for Controlled Psilocybin Use as Treatment or Enhancement in a Representative Sample of US Americans: Need for Caution in Public Policy Persists.

The psychedelic psilocybin has shown promise both as treatment for psychiatric conditions and as a means of improving well-being in healthy individuals. In some jurisdictions (e.g., Oregon, USA), psilocybin use for both purposes is or will soon be allowed and yet, public attitudes toward this shift are understudied. We asked a nationally representative sample of 795 US Americans to evaluate the moral status of psilocybin use in an appropriately licensed setting for either treatment of a psychiatric condition or well-being enhancement. Showing strong bipartisan support, participants rated the i

Advocacy counterstrategies to tobacco industry interference in policymaking: a scoping review of peer-reviewed literature.

BACKGROUND: There has been remarkable tobacco control progress in many places around the globe. Tobacco industry interference (TII) has been identified as the most significant barrier to further implementation of the World Health Organization Framework Convention on Tobacco Control (WHO FCTC). Civil society has been recognised as a key actor in countering TII. While TII has been extensively studied for several decades now, there is little research that focuses on counteractions to limit it and their effectiveness to do so. This scoping review seeks to map the peer-reviewed literature on civil

Laying the foundation for Real-world evidence studies: a case study from Newfoundland and Labrador.

The Janssen and Newfoundland and Labrador Health Innovation Partnership (JANL-HIP) was established to carry out Real-World Evidence (RWE) projects to generate evidence about disease pathways, healthcare delivery, the effects of clinical interventions. Doing so will support and influence clinical decision-making in Newfoundland and Labrador (NL). This case study describes the foundational elements necessary for a real-world evidence generation project in NL and may provide learning for the effective execution of real-world studies in other jurisdictions. It uses an ongoing project in psoriatic

Smallpox models as policy tools.

Mathematical models can help prepare for and respond to bioterrorism attacks, provided that their strengths and weaknesses are clearly understood. A series of initiatives within the Department of Health and Human Services brought modelers together with biologists and epidemiologists who specialize in smallpox and experts in bioterrorism response and health policy and has led to the parallel development of models with different technical approaches but standardized scenarios, parameter ranges, and outcome measures. Cross-disciplinary interactions throughout the process supported the development

Trust in scientists and their role in society across 68 countries.

Science is crucial for evidence-based decision-making. Public trust in scientists can help decision makers act on the basis of the best available evidence, especially during crises. However, in recent years the epistemic authority of science has been challenged, causing concerns about low public trust in scientists. We interrogated these concerns with a preregistered 68-country survey of 71,922 respondents and found that in most countries, most people trust scientists and agree that scientists should engage more in society and policymaking. We found variations between and within countries, whi
