A demonstration of mobile phone deployment to support the treatment of acutely ill children under five in Bushenyi district, Uganda.

BACKGROUND: Benefits of mobile phone deployment for children <5 in low-resource settings remain unproven. The target population of the current demonstration study in Bushenyi District, Uganda, presented with acute fever, pneumonia, or diarrhoea and were treated by community health workers (CHWs) providing integrated community case management (iCCM). METHODS: An observational study was conducted in five parishes (47 villages) served by CHWs well versed in iCCM with supplemental training in mobile phone use. Impact was assessed by quantitative measures and qualitative evaluation through househo

Factors influencing chronic disease self-management behaviors: a national multilevel analysis in China.

OBJECTIVE: This study employed multilevel analysis to examine the impact of the prefecture-level city level variable (number of physicians per 1,000 population) on chronic disease self-management behavior (CDSMB) among Chinese adults, while also investigating the effects of individual-level control variables on CDSMB. METHODS: A cross-sectional survey of 1,916 adults across 120 cities, using data from the "Psychology and Behavior Investigation of Chinese Residents" (PBICR), evaluated Chronic Disease Self-Management Behavior (CDSMB) via CDSMS. Key predictors included individual-level control v

Investigating safety of vulnerable road users in selected EU countries.

PROBLEM: Vulnerable road users comprise over half of all road accident victims in the EU and their safety situation is not improving as fast as for motorists. The paper examines factors affecting fatality risk of pedestrians, cyclists, motorcyclists, and moped riders in seven EU countries using data from CARE database. METHOD: Comparing accident severity indicators between countries is problematic because of data quality issues, different degree of underreporting, and different exposure levels. To avoid bias arising from these issues, fatality risk is modeled with binary logistic regression.

Origins and elements of the population policies of Cameroon Republic.

The author discusses the pro-natalist policies of Cameroon, measures that are not explicit population policies but are instead implied in other government policies and pronouncements. It is argued that "this pronatalist policy partly has its roots in the inherent cultures of the people which have been forged from several years of ravages from epidemic and endemic diseases, inter-tribal conflicts and slave trade. These have been further authenticated, constitutionalised and perpetuated by official policy which bears the stamp of its French colonial masters...." These policies include "legislati

Development and tailoring of a peer-administered asthma management intervention for urban Latino middle school children.

OBJECTIVE: Children from Latino backgrounds face unique challenges in managing asthma. Barriers are compounded when children live in urban settings and are exposed to urban stressors (e.g. barriers to medication use, neighborhood stress). Asthma management in schools is often directly implicated. This study aims to address these gaps in care by developing a 4-session, peer-facilitated, culturally tailored asthma self-management group intervention in urban public-school settings. METHODS: We developed and tested the ASMAS (Asthma Self-Management in Schools) intervention, utilizing an iterative

Epidemiology and Management of the 2013-16 West African Ebola Outbreak.

The 2013-16 West African Ebola outbreak is the largest, most geographically dispersed, and deadliest on record, with 28,616 suspected cases and 11,310 deaths recorded to date in Guinea, Liberia, and Sierra Leone. We provide a review of the epidemiology and management of the 2013-16 Ebola outbreak in West Africa aimed at stimulating reflection on lessons learned that may improve the response to the next international health crisis caused by a pathogen that emerges in a region of the world with a severely limited health care infrastructure. Surveillance efforts employing rapid and effective poin

Community case management of fever due to malaria and pneumonia in children under five in Zambia: a cluster randomized controlled trial.

BACKGROUND: Pneumonia and malaria, two of the leading causes of morbidity and mortality among children under five in Zambia, often have overlapping clinical manifestations. Zambia is piloting the use of artemether-lumefantrine (AL) by community health workers (CHWs) to treat uncomplicated malaria. Valid concerns about potential overuse of AL could be addressed by the use of malaria rapid diagnostics employed at the community level. Currently, CHWs in Zambia evaluate and treat children with suspected malaria in rural areas, but they refer children with suspected pneumonia to the nearest health

Differences in cancer survival by remoteness of residence: an analysis of data from a population-based cancer registry.

PURPOSE: Cancer survival is generally lower for rural compared with urban residents, but findings have been inconsistent. We aimed to assess inequalities in cancer survival by remoteness of residence in Victoria, Australia. METHODS: Incident cancer cases diagnosed in 2001-2015 with 30 cancer types (n = 331,302) were identified through the Victorian Cancer Registry and followed to the end of 2015 through death registries. Five-year net survival was estimated using the Pohar-Perme method and differences assessed by excess mortality rate ratios (EMRRs) using Poisson regression, adjusting for sex

A Population Health Approach to America's Opioid Epidemic.

The United States is in the midst of a nationwide public health emergency: an epidemic of opioid misuse and abuse that has been called the deadliest drug crisis in American history. This article reviews the current status of the opioid epidemic, the trends over the last 30-40 years that may have contributed to the epidemic, and a population health approach to addressing the epidemic. The epidemic is conceptualized from a population health perspective-an upstream and midstream perspective focusing on prevention and a downstream perspective targeting access to evidence-based interventions and ma

Patients experiences of self-management and strategies for dealing with chronic conditions in rural Malawi.

BACKGROUND: The high burden of chronic communicable diseases such as HIV/AIDS, and an escalating rise of non-communicable diseases (NCDs) in Malawi and other sub-Saharan African countries, calls for a shift in how health care services are designed and delivered. Patient-centred care and patient self-management are critical elements in chronic care, and are advocated as universal strategies. In sub-Saharan Africa, there is need for more evidence around the practice of patient self-management, and how to best support patients with chronic conditions in the African context. Our study explored sel

Feasibility of using teleradiology to improve tuberculosis screening and case management in a district hospital in Malawi.

PROBLEM: Malawi has one of the world's highest rates of human immunodeficiency virus (HIV) infection (10.6%), and southern Malawi, where Thyolo district is located, bears the highest burden in the country (14.5%). Tuberculosis, common among HIV-infected people, requires radiologic diagnosis, yet Malawi has no radiologists in public service. This hinders rapid and accurate diagnosis and increases morbidity and mortality. APPROACH: Médecins Sans Frontières, in collaboration with Malawi's Ministry of Health, implemented teleradiology in Thyolo district to assist clinical staff in radiologic imag

A "family-school-hospital" collaborative oral health management model for preschool children: a prospective single-arm pre-post intervention study.

BACKGROUND: Early childhood caries (ECC) remains a global public health challenge, yet preschool children lack autonomy in health management and depend heavily on family and school environments. Population-based oral health management models have proven effective in other fields but are rarely implemented in Chinese preschool settings. This study aimed to establish and evaluate a "family-school-hospital" collaborative oral health management model for preschool children in Chengdu, China. METHODS: The oral health management model for preschool children was established through population screen

The clinical diagnosis and treatment of important childhood diseases in rural Papua New Guinea.

This study used multiple standardized instruments to evaluate the quality of the clinical management of the common childhood diseases in rural Papua New Guinea. The findings indicate major inadequacies in the quality of history taking, examination, record keeping, diagnosis and treatment of the common causes of childhood morbidity and mortality. To improve case management by health workers in rural clinical situations everything possible must be done to make good clinical management easy and practical. This should involve provision of practical reinforcing tools, training and supervision which

Perceptions about chronic health conditions, multimorbidity and self-management practices in rural northeast South Africa: findings from a qualitative study.

INTRODUCTION: Chronic health conditions are the leading causes of morbidity and mortality worldwide, with a disproportionately high burden in low-income and middle-income countries. The burden arising from these conditions presents immense challenges to countries with dysfunctional public healthcare systems, such as South Africa. This necessitates patients to have a good understanding of the conditions and optimal self-management approaches. We explored patients' understanding of chronic health conditions and self-management practices, including self-monitoring, in the rural South African comm
