Interpretation and use of intraoperative protective ventilation parameters: a scoping review.

Thirty years ago, the traditional approach to mechanical ventilation consisted of the normalization of PaCO2 and pH at the expense of using a tidal volume (VT) of 10-15 mL kg-1. But then, the use of 6-8 mL kg-1 became a dogma for ventilating patients either with acute respiratory distress syndrome (ARDS) or without lung disease in the operating theatre. It is currently recognized that even low tidal volumes may be excessive for some patients and insufficient for others, depending on its distribution in the aerated lung parenchyma. To carry out intraoperative protective mechanical ventilation,

Lung Response to a Higher Positive End-Expiratory Pressure in Mechanically Ventilated Patients With COVID-19.

BACKGROUND: International guidelines suggest using a higher (> 10 cm H2O) positive end-expiratory pressure (PEEP) in patients with moderate-to-severe ARDS due to COVID-19. However, even if oxygenation generally improves with a higher PEEP, compliance, and Paco2 frequently do not, as if recruitment was small. RESEARCH QUESTION: Is the potential for lung recruitment small in patients with early ARDS due to COVID-19? STUDY DESIGN AND METHODS: Forty patients with ARDS due to COVID-19 were studied in the supine position within 3 days of endotracheal intubation. They all underwent a PEEP trial, in

Patient self-inflicted lung injury and positive end-expiratory pressure for safe spontaneous breathing.

PURPOSE OF REVIEW: The potential risks of spontaneous effort and their prevention during mechanical ventilation is an important concept for clinicians and patients. The effort-dependent lung injury has been termed 'patient self-inflicted lung injury (P-SILI)' in 2017. As one of the potential strategies to render spontaneous effort less injurious in severe acute respiratory distress syndrome (ARDS), the role of positive end-expiratory pressure (PEEP) is now discussed. RECENT FINDINGS: Experimental and clinical data indicate that vigorous spontaneous effort may worsen lung injury, whereas, at t

Heated Humidifier versus Heat-and-Moisture Exchanger During Positive Pressure Ventilation With a T-Piece Resuscitator in Rabbits.

BACKGROUND: There are many proven benefits of the use of conditioned gases in mechanically ventilated patients. In spite of this, its use in the delivery room is limited, perhaps because of known difficulties with heated humidifiers (HH); moreover, there is no evidence regarding the use of heat-and-moisture exchangers (HME) in a delivery room setting. We sought to asess the airway's absolute humidity level using three diferents strategies: HH, HME and unconditioned gases. METHODS: We conducted an experimental study in 12 intubated rabbits ventilated with a T-piece resuscitator. Absolute humid

Assisted Ventilation.

Controlled Mechanical Ventilation may be essential in the setting of severe respiratory failure but consequences to the patient including increased use of sedation and neuromuscular blockade may contribute to delirium, atelectasis, and diaphragm dysfunction. Assisted ventilation allows spontaneous breathing activity to restore physiological displacement of the diaphragm and recruit better perfused lung regions. Pressure Support Ventilation is the most frequently used mode of assisted mechanical ventilation. However, this mode continues to provide a monotonous pattern of support for respiration

Noninvasive positive pressure ventilation in the acute care setting: where are we?

Noninvasive positive pressure ventilation (NPPV) is a technique used to deliver mechanical ventilation that is increasingly utilised in acute and chronic conditions. The present review examines the evidence supporting the use of NPPV in acute respiratory failure (ARF) due to different conditions. Strong evidence supports the use of NPPV for ARF to prevent endotracheal intubation (ETI), as well as to facilitate extubation in patients with acute exacerbations of chronic obstructive pulmonary disease and to avoid ETI in acute cardiogenic pulmonary oedema, and in immunocompromised patients. Weaker

The management of acute severe asthma.

Asthma is a common cause of morbidity and mortality in the United States, with over two million Emergency Department (ED) visits each year. Airway inflammation is recognized as a major component in the pathophysiology of asthma. The classic presentation of asthma is that of wheezing, cough, and dyspnea, however, the severity of airflow limitation correlates poorly with clinical signs. Forced exhaled volume in 1 s (FEV(1)) and the peak expiratory flow rate (PEFR) are direct reflections of the severity of airflow obstruction and are the standard measures used in the ED to assess the severity of

Ventilatory stability during sleep studied with proportional assist ventilation (PAV).

We propose the use of proportional assist ventilation (PAV) as a method to quantitate ventilatory stability. The PAV amplification factor required to produce periodic breathing is the reciprocal of overall loop gain. Using this approach in 12 normal subjects we found that, after stabilization of upper airway with continuous positive airway pressure (CPAP) (5.2 +/- 2.0 cm H2O), overall loop gain was very low (< 0.3) indicating that sleep. per sc, is not destabilizing under normal conditions.

Perioperative ventilation support, what clinicians and searchers must know.

Postoperative pulmonary complications (PPC) affect over 10% of surgical patients, especially after abdominal, thoracic, or cardiac surgery. Understanding PPC's pathophysiology and risk factors provides an effective framework for prevention strategies. Significant alterations of the respiratory function occur in all patients undergoing major surgery, especially if the surgical site is close to the diaphragm. The main changes observed are a reduction in lung volumes, with the development of restrictive patterns of respiratory mechanics, atelectasis, and diaphragmatic dysfunction. Hypoxemia often

Treatment of intractable paradoxical herniation by invasive mechanical ventilation with increased positive end-expiratory pressure: a case report.

OBJECTIVE: Paradoxical herniation (PH) is a rare but potentially life-threatening complication after decompressive craniectomy. The core treatment strategy for PH focuses on increasing intracranial pressure. Here, we present the treatment course of intractable PH in a 59-year-old patient with a traumatic acute subdural hematoma. METHODS: The patient underwent two operations to evacuate intracranial hematomas followed by decompressive craniectomy within 48 h. Intractable PH was induced by persistent cerebrospinal fluid leakage due to multiple lumbar punctures. The condition was managed with co

Cardiorespiratory coupling in mechanically ventilated patients studied via synchrogram analysis.

Respiration and cardiac activity are strictly interconnected with reciprocal influences. They act as weakly coupled oscillators showing varying degrees of phase synchronization and their interactions are affected by mechanical ventilation. The study aims at differentiating the impact of three ventilatory modes on the cardiorespiratory phase coupling in critically ill patients. The coupling between respiration and heartbeat was studied through cardiorespiratory phase synchronization analysis carried out via synchrogram during pressure control ventilation (PCV), pressure support ventilation (PSV

Pediatric Simulation of Intrinsic PEEP and Patient-Ventilator Trigger Asynchrony During Mechanical Ventilation.

BACKGROUND: Intrinsic PEEP during mechanical ventilation occurs when there is insufficient time for expiration to functional residual capacity before the next inspiration, resulting in air trapping. Increased expiratory resistance (RE), too rapid of a patient or ventilator breathing rate, or a longer inspiratory to expiratory time ratio (TI/TE) can all be causes of intrinsic PEEP. Intrinsic PEEP can result in increased work of breathing and patient-ventilator asynchrony (PVA) during patient-triggered breaths. We hypothesized that the difference between intrinsic PEEP and ventilator PEEP acts a

The Influence of Positive End-Expiratory Pressure on Leakage and Oxygenation Using a Laryngeal Mask Airway: A Randomized Trial.

BACKGROUND: The value of positive end-expiratory pressure (PEEP) in maintaining oxygenation during ventilation with a laryngeal mask airway (LMA) mask is unclear. To clarify the potential benefit or harm to PEEP application during positive pressure ventilation with a ProSeal LMA® mask, we compared the effect of PEEP versus zero end-expiratory pressure (ZEEP) on gas leakage and oxygenation. We hypothesized that a PEEP of 8 mbar (8.2 cm H 2 O) would be associated with an increased incidence of gas leakage compared to ZEEP. METHODS: We designed a prospective, controlled, randomized, single-blind
