Mode of death in Shapiro syndrome: a case report.

BACKGROUND: Shapiro syndrome is extremely rare and is characterized by the triad of spontaneous periodic hypothermia, hyperhidrosis and agenesis of the corpus callosum, resulting in neurological and psychological disorders. The exact mechanism of this syndrome is unknown and treatment consists of controlling the periodic attacks. This case report describes a case of Shapiro syndrome presenting with ventricular fibrillation (VF) who was treated with dual chamber implantable cardioverter defibrillator (ICD) therapy. CASE SUMMARY: A 45-year-old man, suffering from Shapiro syndrome with frequent

Patient Care Alterations After Point-of-Care Laboratory Testing During Critical Care Transport.

OBJECTIVE: Point-of-care laboratory testing (POCT) is associated with a reduced time to testing results and critical decision making within emergency departments. POCT is an essential clinical assessment tool because laboratory data are used to support timely critical decisions regarding acute medical conditions onditions ; however, there is currently limited research to support the use of POCT in the critical care transport environment. Few studies have evaluated the changes in patient care that occur after POCT during critical care transport. This study aims to contribute to the limited data

Severe Hand Ischemia Following Distal Transradial Coronary Intervention for Cardiac Arrest.

BACKGROUND: Despite growing interest in the distal transradial approach (dTRA) for coronary interventions, the risk of hand ischemia remains insufficiently elucidated. CASE PRESENTATION: A 72-year-old woman who experienced an out-of-hospital cardiac arrest was successfully resuscitated and diagnosed with acute myocardial infarction. Primary percutaneous coronary intervention was performed via dTRA with intra-aortic balloon pump support. Post-percutaneous coronary intervention, she developed systemic inflammatory response syndrome due to hypoperfusion during resuscitation, requiring high-dose

QT interval prolongation associated with sibutramine treatment.

AIMS: To investigate a possible association of sibutramine with QT interval prolongation. METHODS: Post-marketing surveillance using prescription event monitoring in the New Zealand Intensive Medicines Monitoring Programme (IMMP) identified a case of QT prolongation and associated cardiac arrest in a patient taking sibutramine for 25 days. This patient was further investigated, including genotyping for long QT syndrome. Other IMMP case reports suggesting arrhythmias associated with sibutramine were assessed and further reports were obtained from the World Health Organisation (WHO) adverse dru

Post-transplant seizures in infants with hypoplastic left heart syndrome.

Seizures are common in infants undergoing cardiac transplant and are usually attributed to a non-specific "post-pump" phenomenon. In this study, we determined which variables were associated with the occurrence of post-transplant seizures in infants with hypoplastic left heart syndrome and the need for continued treatment with antiepileptic medication. Of 127 infants studied over an 11-year period, 27 (21%), ages 9 to 90 days, had post-transplant seizures. These patients were compared to 27 age-matched transplanted infants without seizures. We compared multiple variables before, during, and af

Left cardiac sympathetic denervation in long QT syndrome: analysis of therapeutic nonresponders.

BACKGROUND: Long QT syndrome (LQTS) is a potentially lethal but highly treatable cardiac channelopathy. Treatment options include pharmacotherapy, device therapy, and left cardiac sympathetic denervation (LCSD). Here, we sought to determine the characteristics of LQTS patients who have had ≥1 LQTS-related breakthrough cardiac event (BCE) after LCSD. METHODS AND RESULTS: We performed retrospective chart review for 52 consecutive patients (24 males; mean age at diagnosis, 10.0±10 years; mean QTc, 528±74 ms) with LQTS who underwent LCSD between 2005 and 2010 (mean age at LCSD, 14.1±10 years) and

Admission Society for Cardiovascular Angiography and Intervention shock stage stratifies post-discharge mortality risk in cardiac intensive care unit patients.

BACKGROUND: The five-stage Society for Cardiovascular Angiography and Intervention (SCAI) cardiogenic shock classification scheme can stratify hospital mortality risk in patients admitted to the cardiac intensive care unit (CICU). We sought to evaluate the SCAI shock classification for prediction of post-discharge mortality in CICU survivors. METHODS: We retrospectively analyzed hospital survivors admitted to a single CICU between 2007 and 2015. SCAI CS stages A through E were classified using CICU admission data using a previously published algorithm. All-cause post-discharge mortality was c

Positron emission tomography myocardial perfusion imaging (PET MPI) findings predictive of post-liver transplant major adverse cardiac events.

Positron emission tomography myocardial perfusion imaging (PET MPI) is a noninvasive diagnostic test capable of detecting coronary artery disease, structural heart disease, and myocardial flow reserve (MFR). We aimed to determine the prognostic utility of PET MPI to predict post-liver transplant (LT) major adverse cardiac events (MACE). Among the 215 LT candidates that completed PET MPI between 2015 and 2020, 84 underwent LT and had 4 biomarker variables of clinical interest on pre-LT PET MPI (summed stress and difference scores, resting left ventricular ejection fraction, global MFR). Post-LT

Grey-to-White Matter Ratio Values in Early Head Computed Tomography (CT) as a Predictor of Neurologic Outcomes in Survivors of Out-of-Hospital Cardiac Arrest Based on Severity of Hypoxic-Ischemic Brain Injury.

BACKGROUND: Hypoxic-ischemic brain injury (HIBI) is a common complication of out-of-hospital cardiac arrest (OHCA). OBJECTIVES: We investigated whether grey-to-white matter ratio (GWR) values, measured using early head computed tomography (HCT), were associated with neurologic outcomes based on the severity of HIBI in survivors of OHCA. METHODS: This retrospective multicenter study included adult comatose OHCA survivors who underwent an HCT scan within 2 h after the return of spontaneous circulation. HIBI severity was assessed using the revised post-Cardiac Arrest Syndrome for Therapeutic hy

Surgical repair of left ventricular free-wall rupture complicating acute myocardial infarction: a single-center 30 years of experience.

BACKGROUND: Left ventricular free-wall rupture (LVFWR) is a catastrophic complication of acute myocardial infarction (AMI). Historically, cardiac surgery is considered the treatment of choice. However, because of the rarity of this entity, little is known regarding the efficacy and safety of surgical treatment for post-infarction LVFWR. The aim of this study was to report a single-center experience in this field over a period of 30 years. METHODS: Patients who developed LVFWR following AMI and underwent surgical repair at our Institution from January 1990 to December 2019 were considered. The

Suppression of PCK1 attenuates neuronal injury and improves post-resuscitation outcomes.

Cardiac arrest (CA) is a critical medical emergency that can occur in both patients with preexisting conditions and otherwise healthy individuals. Despite successful resuscitation through cardiopulmonary resuscitation (CPR), many survivors are at significant risk of developing post-cardiac arrest syndrome (PCAS), a complex systemic response to CA that includes brain injury as a major component. Phosphoenolpyruvate carboxykinase 1 (PCK1), the first rate-limiting enzyme in gluconeogenesis, has been implicated in various diseases. However, its role in neuronal damage following CA/CPR remains uncl

The Khmer 'Weak Heart' Syndrome: Fear of Death from Palpitations.

According to the Khmer conception, a person suffering 'weak heart' (khsaoy beh daung) has episodes of palpitations on slight provocation (e.g. triggered by orthostasis, anger, a noise, worry, an odor or exercise) and runs the risk of dying of heart arrest during these periods of palpitations; too, the sufferer typically has other symptoms attributed to the purported cardiac dysfunction: fatigue, shortness of breath, and orthostatic dizziness. Many Khmer refugees suffer this cultural syndrome, an anxious-dysphoria ontology, most probably of French colonial provenance. The syndrome demonstrates

Transesophageal echocardiography (TEE)-guided management of post-myocardial infarction complications.

Intramyocardial dissecting hematoma with cardiac rupture poses catastrophic risks following acute myocardial infarction (AMI). While transthoracic echocardiography (TTE) is standard for initial assessment, transesophageal echocardiography (TEE) provides critical anatomical delineation for surgical planning. A 78-year-old male with multiple cardiovascular risk factors presented with a three-day history of chest pain, diagnosed as ST-segment elevation myocardial infarction (STEMI) by electrocardiogram (ECG). TTE revealed segmental left ventricular wall motion abnormalities and cardiac troponin I

Construction of a Mortality Prediction Model for Cardiac Arrest Patients Using Lactate Clearance Rate and Albumin-Corrected Anion Gap.

BACKGROUND: Cardiac arrest (CA) causes systemic circulatory failure, leading to postcardiac arrest syndrome (PCAS) and high mortality. Lactic clearance rate (LCR) and albumin-corrected anion gap (ACAG) reflect metabolic status but lack combined validation for predicting short-term mortality in CA patients. OBJECTIVE: This study explores the predictive value of LCR at different time points and ACAG for 7-day mortality in post-CA patients and constructs a predictive model. METHODS: Using the MIMIC-IV 3.0 database, we included eligible CA patients and calculated LCR at 6, 12, and 24 h, along wi

Interleukin-6 Receptor Antibodies for Modulating the Systemic Inflammatory Response after Out-of-Hospital Cardiac Arrest (IMICA): study protocol for a double-blinded, placebo-controlled, single-center, randomized clinical trial.

BACKGROUND: Resuscitated out-of-hospital cardiac arrest (OHCA) patients who remain comatose at admission are at high risk of morbidity and mortality. This has been attributed to the post-cardiac arrest syndrome (PCAS) which encompasses multiple interacting components, including systemic inflammation. Elevated levels of circulating interleukin-6 (IL-6), a pro-inflammatory cytokine, is associated with worse outcomes in OHCA patients, including higher vasopressor requirements and higher mortality rates. In this study, we aim to reduce systemic inflammation after OHCA by administering a single inf
