Introduction of intradermal rabies vaccination - A paradigm shift in improving post-exposure prophylaxis in Asia.

The cost of cell culture vaccines for intramuscular administration limits their widespread use in many areas where rabies is present. The innovation of cost-effective multi-site intradermal (ID) vaccination technique was an impetus for high burden countries to phase out production and use of rabies vaccine of nerve tissue origin in public hospitals in subsequent years. The WHO Expert Committee in 1991 recommended intradermal application of modern rabies vaccines for post-exposure prophylaxis. There are many challenges in promotion of ID schedule. Poor patient compliance due to the need for sev

Healthcare utilization, provisioning of post-exposure prophylaxis, and estimation of human rabies burden in Madagascar.

In Madagascar, dog-mediated rabies has been endemic for over a century, however there is little data on its incidence or impact. We collected data over a 16-month period on provisioning of post-exposure prophylaxis (PEP) at a focal clinic in the Moramanga District and determined the rabies status of biting animals using clinical and laboratory diagnosis. We find that animal rabies cases are widespread, and clinic-based triage and investigation are effective ways to increase detection of rabies exposures and to rule out non-cases. A high proportion of rabies-exposed persons from Moramanga sough

Effectiveness of varicella vaccine as post-exposure prophylaxis during a varicella outbreak in Shanghai, China.

OBJECTIVES: Varicella vaccine (VarV) is recommended as effective post-exposure prophylaxis (PEP) within 3-5days to control outbreaks. However, the effectiveness of PEP at >5days after exposure and the administration of a second dose to those with a history of one dose prior to exposure have not been fully examined. This study evaluated the vaccine effectiveness (VE) of PEP in preventing disease during a varicella outbreak in Shanghai, China in 2013. METHODS: Self-administered questionnaires were used to obtain the students' varicella history, vaccination status, and willingness to receive PEP

Reducing Cost of Rabies Post Exposure Prophylaxis: Experience of a Tertiary Care Hospital in Pakistan.

BACKGROUND: Rabies is a uniformly fatal disease, but preventable by timely and correct use of post exposure prophylaxis (PEP). Unfortunately, many health care facilities in Pakistan do not carry modern life-saving vaccines and rabies immunoglobulin (RIG), assuming them to be prohibitively expensive and unsafe. Consequently, Emergency Department (ED) health care professionals remain untrained in its application and refer patients out to other hospitals. The conventional Essen regimen requires five vials of cell culture vaccine (CCV) per patient, whereas Thai Red Cross intradermal (TRC-id) regim

Assessing safety and immunogenicity of post-exposure prophylaxis following interchangeability of rabies vaccines in humans.

Rabies post exposure prophylaxis with cell culture vaccines by either intramuscular route or intradermal route spans over a period of one month. World Health Organization recommends completing post exposure prophylaxis against rabies with the same cell culture or embryonated egg rabies vaccine and with same route of administration and any deviation from this shall be an exception. In the present study, the safety and immunogenicity of rabies post-exposure prophylaxis was studied prospectively in 90 animal bite cases that had interchangeability of rabies vaccines either by route of administrati

The cost of preventing rabies at any cost: post-exposure prophylaxis for occult bat contact.

Investigations conducted by public health in Quebec, Canada, following report of human exposure to a bat were reviewed to evaluate the implementation of the recommendation for rabies post-exposure prophylaxis (RPEP) for household bat exposure (without documented direct contact). Of all RPEP recommended, 12% was for direct bat contact with bite, 7% for direct bat contact without known bite and 81% for household exposure. When bat was not available for testing, RPEP was almost always recommended. Household bat exposure has become the most frequent reason for RPEP administration. Given the rarity

The uptake of HIV post-exposure prophylaxis within a sexual assault setting in Sydney, Australia.

Our aim was to compare the assault characteristics of victims presenting to a sexual assault service who were prescribed HIV post-exposure prophylaxis (HIV PEP) with those not prescribed HIV PEP. A retrospective review was carried out of the medical records of victims who were seen over a 12-month period in 1999/2000.HIV PEP may have been potentially appropriate for 117 victims, of whom nine (7.7%) were prescribed PEP (eight women, one man). There was a trend for prescription of PEP to depend on the type of assault, with those suffering anal penetration most likely to be prescribed PEP, follow

Preventing laboratory-acquired Toxoplasma gondii infection: literature review and case-based post-exposure prophylaxis proposal.

Occupationally acquired cases of the intracellular parasite Toxoplasma gondii have been reported, even though there is uncertainty on which infection prevention measures should be implemented in such event. We report a clinical case of laboratory-acquired toxoplasmosis and propose a protocol for post-exposure prophylaxis, given there are no formal guidelines. In this community case study, we describe the management and prevention protocol following an occupational laboratory exposure by a healthy 30-year-old Toxoplasma-seronegative female researcher who suffered an accidental needle puncture w

Rabies viral RNA and antibody response after post-exposure prophylaxis: Implications for blood donor deferral.

BACKGROUND: Rabies post-exposure prophylaxis (PEP) using anti-rabies vaccine and immunoglobulin is widely recognized as highly effective. However, individuals who receive PEP are typically deferred from blood donation for a full year, largely due to theoretical concerns about transfusion safety. However, the evidence supporting such a prolonged deferral remains limited. OBJECTIVES: This study aimed to assess rabies real-time RT-PCR status and serological responses among PEP recipients at various intervals following treatment and to consider whether these findings might justify a re-examinatio

Implementing the new WHO guidelines on HIV post-exposure prophylaxis: perspectives from five African countries.

INTRODUCTION: Post-exposure prophylaxis (PEP) is an important component of comprehensive HIV prevention, yet its uptake has been suboptimal globally. In July 2024, the World Health Organization (WHO) updated its global guidance on PEP to include two new recommendations intended to increase timely access to and delivery of PEP. These recommendations specifically aim to expand both where PEP can be delivered, to include community settings, and who can provide PEP, to include community health workers and task-sharing. The practical realities of adopting new public health guidelines to achieve the

The Clinical Advantages of Anti-Rabies Monoclonal Antibodies in Post-Exposure Prophylaxis - Worldwide, 2016-2025.

Post-exposure prophylaxis (PEP) represents the critical intervention for preventing rabies and comprises three essential components: thorough wound cleansing, vaccination, and administration of rabies immunoglobulin (RIG) or monoclonal antibodies (mAbs) for category III exposures. The World Health Organization (WHO) endorses the use of mAb cocktails as an effective replacement for RIG in PEP protocols. Since 2016, four anti-rabies monoclonal antibodies (RmAbs) have received clinical approval for use in India and China. This article provides an overview of the current research status of RmAb. B

Doxycycline Post-Exposure Prophylaxis for Bacterial Sexually Transmitted Infections: The Current Landscape and Future Directions.

PURPOSE OF REVIEW: The incidence of bacterial sexually transmitted infections (STI) continues to rise particularly among men who have sex with men (MSM). Doxycycline post-exposure prophylaxis (doxy-PEP) has emerged as a promising biomedical prevention strategy. This review aims to summarize the results of recent studies, highlight the current normative guidance on the use of doxy-PEP, and discuss remaining questions. RECENT FINDINGS: In the past decade, there have been four randomized controlled trials and three real-world analyses of doxy-PEP, which consistently demonstrated a reduction in C

Doxycycline post-exposure prophylaxis for preventing bacterial sexually transmitted infections (STIs): Are clinical providers supportive?

BACKGROUND: Bacterial sexually transmitted infections (STIs) continue to increase in the United States. Despite evidence of the effectiveness of doxycycline post-exposure prophylaxis (Doxy-PEP) to prevent STIs, little is known about providers' attitudes and willingness to implement Doxy-PEP. METHODS: An online questionnaire was sent to 575 clinical providers in New York State in September 2022. RESULTS: Ninety-one eligible individuals responded. Most providers served men who have sex with men (MSM) (84%); reported willingness to recommend Doxy-PEP (98%, 77%, and 67% for chlamydia, syphilis,

Evaluating the benefit of serology during potential Australian bat lyssavirus and rabies post-exposure prophylaxis.

UNLABELLED: Post-exposure prophylaxis (PEP) for potential lyssavirus exposures consists of wound management, rabies vaccination and may include rabies immunoglobulin (RIG). Rabies serology is sometimes indicated if there is risk of PEP failure. OBJECTIVES: Evaluate the benefit of serology by indication. METHODS: Chart review of potential lyssavirus exposures managed at a Public Health Unit (June 2015 - December 2022) where serology was requested was conducted. The proportion of non-therapeutic titres was compared by sex, age, Indigenous status, serology indication, and whether RIG was given.

Moving fast but going slow: coordination challenges for trials of COVID-19 post-exposure prophylaxis.

An unprecedented volume of research has been generated in response to the COVID-19 pandemic. However, there are risks of inefficient duplication and of important work being impeded if efforts are not synchronized. Excessive reliance on observational studies, which can be more rapidly conducted but are inevitably subject to measured and unmeasured confounders, can foil efforts to conduct rigorous randomized trials. These challenges are illustrated by recent global efforts to conduct clinical trials of post-exposure prophylaxis (PEP) as a strategy for preventing COVID-19. Innovative strategies a

Improving access to appropriate post-exposure doxycycline for Lyme disease prophylaxis: role for community pharmacies.

The transmission of Borrelia burgdorferi to humans through tick bites results in Lyme disease. Appropriate therapy for Lyme disease is antibacterial drugs, most often doxycycline. Patients often approach community pharmacists for self-care assistance with the symptoms of Lyme disease: fever, headache, fatigue and skin rash. Pharmacists with the patient history are trained and capable of appropriately dispensing doxycycline to treat these patients and prevent the spread of infection to the joints, nerves or heart. We challenge restrictions to the appropriate and timely provision of therapy for
