A novel framework for integrating post-mortem images with 3D body models for automatic injury medical documentation.

Forensic medicine is critical in criminal investigations, providing essential insights into the nature and mechanisms of injuries. Traditional post-mortem examination remains mostly manual, which introduces risks of error and inefficiency, particularly in injury assessment. This study proposes a fully automated system for detecting and mapping injuries (bruises and abrasions) on a 3D human body model to improve forensic documentation. The developed automated system consisted of the following modules: (1) a module for predicting injuries in images, (2) a module for generating a 3D human body ba

Protocol for fixation and staining of postmortem human brain tissue optimized for focused ion beam-scanning electron microscopy.

Here, we present a protocol for generating postmortem human brain tissue samples suitable for volume electron microscopy (vEM) imaging via focused ion beam-scanning electron microscopy (FIB-SEM). We describe steps for immersion fixation, sectioning, long-term cryopreservation, and enhanced-contrast staining that produce samples suitable for high-resolution FIB-SEM. This approach generates ultrastructure of sufficiently high preservation to enable both dense and targeted three-dimensional (3D) reconstructions of cellular, subcellular, synaptic, and organellar features in postmortem human brain

Magnetic resonance imaging in a lamb with compression of the thoracic spinal cord by an abscess.

This report describes a two-month-old Ouessant ram lamb with hind limb ataxia, knuckling and falling attributable to an abscess in the thoracic vertebral region. A swelling palpated adjacent to the spinous processes of the 9th to 12th thoracic vertebrae was identified via ultrasonography as an abscess located on the transverse processes of these vertebrae. Magnetic resonance imaging of the lamb postmortem revealed that the abscess had invaded the vertebral canal through a channel at the level of the 9th thoracic vertebra causing extradural spinal cord compression. Postmortem examination confir

Prenatal diagnosis of Meckel-Gruber syndrome and Dandy-Walker malformation in four consecutive affected siblings, with the fourth one being diagnosed prenatally at 22 weeks of gestation.

We report a 23-week-old male fetus affected by Meckel-Gruber syndrome. Posterior encephalocele, post-axial polydactyly, and Dandy-Walker malformation were observed on ultrasonographic (USG) examination at 22 weeks' gestation, and lobar holoprosencephaly was demonstrated on postmortem magnetic resonance imaging (MRI) prior to autopsy. After the termination of the pregnancy, polycystic dysplastic kidneys were also noted at postmortem investigation. The proband was the product of the fourth pregnancy of a consanguineous family in which all three siblings were also similarly affected. Interestingl

Standardization of postmortem human brainstem along the rostrocaudal axis to accommodate for inter-specimen structural heterogeneity.

There is increasing recognition of the critical need for postmortem studies of human brainstem tissues for elucidating the pathophysiology of neurological disorders. While the brainstem is critical for a myriad of functions it is relatively small with a complex organization of tracts and nuclei. So, interindividual heterogeneity in brainstem structure that can derive from tissue procurement or be related to anthropometric features like height or weight can greatly impact studies of brainstem structure and function. In the absence of explicit approaches to account for these sources of heterogen

Radiomic analysis of postmortem lung changes: a PMCT-based approach for estimating the postmortem interval.

This study presents an investigation of the potential of radiomic features extracted from postmortem computed tomography (PMCT) scans of the lungs to provide valuable insights into the postmortem interval (PMI), a crucial parameter in forensic medicine. Sequential PMCT scans were performed on 17 bodies with known times of death, ranging from 4 to 108 h postmortem. Radiomic features were extracted from the lungs, and a mixed-effects model, tailored for sequential data, was employed to assess the relationship between feature values and the PMI. Four model variants were tested to identify the mos

3T MRI Whole-Brain Microscopy Discrimination of Subcortical Anatomy, Part 1: Brain Stem.

BACKGROUND AND PURPOSE: The brain stem is compactly organized with life-sustaining sensorimotor and autonomic structures that can be affected by numerous pathologies but can be difficult to resolve on conventional MR imaging. MATERIALS AND METHODS: We applied an optimized TSE T2 sequence to washed postmortem brain samples to reveal exquisite and reproducible brain stem anatomic MR imaging contrast comparable with histologic atlases. This resource-efficient approach can be performed across multiple whole-brain samples with relatively short acquisition times (2 hours per imaging plane) using cl

Pictorial review of the postmortem computed tomography in neonaticide cases.

Neonaticide is defined by the deliberate killing or homicide of a child within 24 h of its birth. In this context, three fundamental questions are generally asked of the forensic pathologist: what is the cause of death of the neonate? Was the child viable (i.e., what is the gestational age of the neonate)? Finally, was the neonate stillborn or liveborn?Postmortem imaging can help answer these questions by conducting (1) a complete lesional analysis of the body and the placenta, (2) an estimation of the gestational age by measuring the lengths of the diaphyseal long bones, and (3) an analysis o

Postmortem cranial MRI and autopsy correlation in suspected child abuse.

We investigated the correlation between postmortem magnetic resonance imaging (MRI) of the head and autopsy findings in suspected child abuse. Postmortem MRI was performed within 24 h of death and before autopsy in 11 children 2 years old or younger whose deaths were unexplained or suspected to be due to child abuse. MRI findings were available to the pathologist at the time of autopsy. In eight cases of death from non-accidental trauma, cerebral edema, contusion, shearing injury, ischemia, and infarction were well demonstrated on MRI. In the three deaths determined not to be due to trauma, th

On the merits and potential of advanced neuroimaging techniques in COVID-19: A scoping review.

Many Coronavirus Disease 2019 (COVID-19) patients are suffering from long-term neuropsychological sequelae. These patients may benefit from a better understanding of the underlying neuropathophysiological mechanisms and identification of potential biomarkers and treatment targets. Structural clinical neuroimaging techniques have limited ability to visualize subtle cerebral abnormalities and to investigate brain function. This scoping review assesses the merits and potential of advanced neuroimaging techniques in COVID-19 using literature including advanced neuroimaging or postmortem analyses i

Forensic interpretation of dental trauma: Morphological criteria, differential diagnosis, and distinction between antemortem, perimortem, and postmortem alterations.

Perimortem traumatic dental injuries (PTDIs) can provide critical forensic information for reconstructing events occurring around the time of death. Accurate assessment and differentiation of traumatic dental injuries (TDIs) may contribute to the interpretation of injury patterns, the evaluation of possible causes and manners of death, and forensic investigations. However, accurately determining the timing of dental trauma, particularly distinguishing PTDIs from antemortem dental injuries (ADIs) and postmortem (PM) alterations, remains a significant forensic challenge. To provide a structured

Normal cranial postmortem CT findings in children.

INTRODUCTION: Postmortem imaging (both CT and MRI) has become a widely used tool the last few years, both for adults and children. If it would be known which findings are normal postmortem changes, interpretation of abnormal findings becomes less ambiguous. Our aim was to describe postmortem intracranial radiological findings on postmortem CT (PMCT) in children, which did not have a relationship with the cause of death, and to determine whether these findings have a relationship with the postmortem interval or with medical interventions. MATERIALS AND METHODS: We selected all consecutive pedi

Quantitative Susceptibility Mapping of Amyloid-β Aggregates in Alzheimer's Disease with 7T MR.

BACKGROUND: PET imaging is an established technique to detect cerebral amyloid-β (Aβ) plaques in vivo. Some preclinical and postmortem data report an accumulation of redox-active iron near Aβ plaques. Quantitative susceptibility mapping (QSM) at high-field MRI enables iron deposits to be depicted with high spatial resolution. OBJECTIVE: Aim of this study was to examine whether iron and Aβ plaque accumulation is related and thus, whether 7T MRI might be an additive diagnostic tool to Aβ PET imaging. METHODS: Postmortem human Alzheimer's disease (AD) and healthy control (HC) frontal gray matte

Pericardial Effusion Detection on Post-Mortem Computed Tomography Images Using Convolutional Neural Networks.

Pericardial effusion can be a sign of significant underlying diease and, in some cases, may lead to death. Post-mortem computed tomography (PMCT) is a well-established tool to assist death investigation processes in the forensic setting. In practice, the scarcity of well-trained radiologists is a challenge in processing raw whole-body PMCT images for pericardial effusion detection. In this work, we propose a Pericardial Effusion Automatic Detection (PEAD) framework to automatically process raw whole-body PMCT images to filter out the irrelevant images with heart organ absent and focus on peric

Postmortem diagnosis of hypothermia.

The identification of hypothermia as the cause of death has always been somewhat problematic in forensic pathology because of unspecific, inconstant, or even negative macroscopic and microscopic findings. Though the simultaneous presence of frost erythema, Wischnewski spots, hemorrhages into the synovial membrane, bloody discoloration of synovial fluid of the knee, and basal vacuolization of the renal tubular epithelial cells has been indicated as strongly supportive of fatal hypothermia, their absence does not allow the diagnosis of hypothermia to be ruled out. Medullary Tegmental Cap Dysplasia: Fetal and Postnatal Presentations of a Unique Brainstem Malformation.

BACKGROUND AND PURPOSE: Medullary tegmental cap dysplasia is a rare brainstem malformation, first described and defined by James Barkovich in his book Pediatric Neuroimaging from 2005 as an anomalous mass protruding from the posterior medullary surface. We describe the neuroimaging, clinical, postmortem, and genetic findings defining this unique malformation. MATERIALS AND METHODS: This is a multicenter, international, retrospective study. We assessed the patients' medical records, prenatal ultrasounds, MR images, genetic findings, and postmortem results. We reviewed the medical literature fo

Two forensic autopsy cases of death due to upper gastrointestinal hemorrhage: a comparison of postmortem computed tomography and autopsy findings.

In this report, we describe two autopsy cases of death due to upper gastrointestinal hemorrhage (Case 1: gastric ulcer, Case 2: aortoduodenal fistula). Postmortem computed tomography (CT) images from both cases revealed pooling of gastric fluid, which contained high attenuation areas, although these images also mirrored the different sources of the gastrointestinal hemorrhage. Fluid collection was observed in the small intestine for both cases, although the high attenuation areas were only remarkable in Case 2. The autopsy in Case 1 revealed a peptic ulcer, with small vessels exposed on the su

Severe Cardiomegaly in Congestive Heart Failure: Postmortem Evidence of Cardiac Remodeling.

BACKGROUND: Cardiomegaly is the gross manifestation of advanced cardiac remodeling, a compensatory response to prolonged hemodynamic stress. Direct postmortem examination of cardiac remodeling is rare, because cardiomegaly is clinically identified through radiologic imaging. CASE SUMMARY: An 81-year-old male cadaveric donor with congestive heart failure as the known cause of death was dissected under standard procedure. Gross examination revealed severe enlargement of the heart and marked dilation of the aorta. Microscopic examination demonstrated significant left ventricular myocyte hypertro

Limitations of ex vivo measurements for in vivo neuroscience.

A long history of postmortem studies has provided significant insight into human brain structure and organization. Cadavers have also proven instrumental for the measurement of artifacts and nonneural effects in functional imaging, and more recently, the study of biophysical properties critical to brain stimulation. However, death produces significant changes in the biophysical properties of brain tissues, making an ex vivo to in vivo comparison complex, and even questionable. This study directly compares biophysical properties of electric fields arising from transcranial electric stimulation

Unusual pattern of enhancement in cryptococcal meningitis: in vivo findings with postmortem correlation.

Leptomeningeal enhancement on CT and MR imaging studies secondary to cryptococcal meningitis is an uncommon finding. In immunocompromised patients, this meningitis incites only a mild inflammatory reaction and abnormalities are often absent on imaging studies. We recently encountered two patients with cryptococcal meningitis in whom postcontrast MR imaging showed thick enhancing subarachnoid spaces. Both had cryptococcal meningitis at autopsy. In a different patient with cryptococcal meningitis, postmortem MR imaging and pathologic examinations showed that these areas of enhancement correspond
