Antenatal maternal education for improving postnatal perineal healing for women who have birthed in a hospital setting.

BACKGROUND: The female perineum becomes suffused and stretched during pregnancy, and further strain during vaginal childbirth contributes to approximately 85% of women experiencing some degree of trauma to the perineal region. Multiple factors play a role in the type and severity of trauma experienced, including parity, delivery method, and local practices. There is ongoing debate about best midwifery practice to reduce perineal trauma. Once perineal trauma has occurred, treatment also varies greatly, depending on its degree and severity, local practice and customs, and personal preference. In

Committee Opinion No. 666 Summary: Optimizing Postpartum Care.

In the weeks after birth, postpartum care often is fragmented among maternal and pediatric health care providers, and communication between inpatient and outpatient settings is inconsistent. To optimize postpartum care, anticipatory guidance should begin during pregnancy. During antenatal care, it is recommended that the patient and her obstetrician-gynecologist or other obstetric care provider formulate a postpartum care plan and identify the health care professionals who will comprise the postpartum care team for the woman and her infant. Ideally, during the postpartum period, a single healt

Neonatal hemochromatosis: management, outcome, and prevention.

Neonatal hemochromatosis (NH) is a rare disorder but the most common cause of acute liver failure in neonates. NH is characterized by severe hepatic injury and iron overload and is associated with high perinatal mortality and morbidity rates. NH is often preceded by oligohydramnios and intrauterine growth restriction, suggesting an important impact of NH during fetal life. Stillbirth and prematurity are not uncommon. During the last decade, major discoveries on the etiology of NH have radically changed the management and outcome of this disease. NH is now regarded as an alloimmune disease and

Effectiveness of discharge planning interventions on health-related outcomes among postpartum women: a systematic review and meta-analysis.

BACKGROUND: Postpartum women frequently experience psychological distress, physical morbidities, and sub-optimal maternal-neonatal care skills. Structured discharge planning may mitigate these problems, yet its overall effectiveness and the active ingredients underpinning benefit remain unclear. OBJECTIVE: To synthesis randomized and quasi-experimental evidence on the effectiveness of discharge planning interventions for improving postpartum women's health outcomes and to identify intervention characteristics that modify this effectiveness. METHODS: Nine electronic databases (including PubMe

Critical signs and symptoms for self-assessment in the immediate postnatal period: an international Systematic Scoping Review and Delphi consensus study.

BACKGROUND: Every two minutes a woman dies from complications of pregnancy and childbirth. Most maternal deaths occur within the first 24 hours following birth, highlighting the importance of immediate postnatal care (iPNC). Self-care strategies are increasingly being employed to promote women-centred, continuous care provision. Despite international calls for development of strategies promoting self-care, none have been developed for self-monitoring in the immediate postnatal period. Fundamental to the development of a self-monitoring strategy, is an understanding of which signs and symptoms

Implementation of Virtual Antenatal and Postnatal Urgent Midwifery Visits: Evaluation of a Quality Improvement Initiative.

Women seeking care during the perinatal period often face delays or long waits at healthcare facilities due to lack of providers and/or resources, leading to sub-optimal outcomes. We implemented a program whereby patients with concerns could receive same-day care virtually from a midwife rather than presenting to the clinic or hospital for care. Implementation strategies included virtual training, a staged increase in patient volume, and frequent communication between the midwives via text, email, and monthly meetings. Virtual visits included a variety of complaints, the five most common being

Committee Opinion No. 650 Summary: Physical Activity and Exercise During Pregnancy and the Postpartum Period.

Physical activity in all stages of life maintains and improves cardiorespiratory fitness, reduces the risk of obesity and associated comorbidities, and results in greater longevity. Physical activity in pregnancy has minimal risks and has been shown to benefit most women, although some modification to exercise routines may be necessary because of normal anatomic and physiologic changes and fetal requirements. Women with uncomplicated pregnancies should be encouraged to engage in aerobic and strength-conditioning exercises before, during, and after pregnancy. Obstetrician-gynecologists and othe

Comprehensive Patient-centered Postpartum Care for Placenta Accreta Spectrum.

Placenta accreta spectrum survivors face unique physical, psychological, and social challenges extending beyond traditional postpartum care. These patients experience prolonged physical recovery and difficult psychological recovery. Many struggle with grief over fertility loss following hysterectomy. Comprehensive care requires an expanded multidisciplinary team with coordinated transitions between specialists. Recovery often extends months to years, necessitating individualized care pathways that address the full spectrum of needs to optimize the long-term well-being for these patients and th

Prenatal hydronephrosis: postnatal evaluation and management.

Congenital hydronephrosis is one of the most common anomalies identified on antenatal ultrasound. The underlying etiology of congenital hydronephrosis is multifold, ranging from transient hydronephrosis in utero to clinically significant congenital anomalies of the kidney and urinary tract. While traditional management of hydronephrosis was aimed at relieving symptoms, the advent of routine prenatal ultrasound has led to a shift in the goal of treatment to prevention of renal injury in the asymptomatic patient. However, despite this focus on renal preservation, the diagnostic criteria for iden

Postpartum and interpregnancy care of women with a history of hypertensive disorders of pregnancy.

Hypertensive disorders of pregnancy (HDP) are common complications associated with maternal and neonatal morbidity and mortality worldwide. Insights gained from long-term cohort studies have revealed that women with a history of HDP are predisposed to recurrent HDP in subsequent pregnancies and face heightened risks for cardiovascular and metabolic diseases later in life. Pregnancy is a unique condition that overloads maternal cardiac and metabolic functions, and is recognized as a "maternal stress test" for future cardiovascular and metabolic diseases. Pregnancy and postpartum period provide

Postpartum care services and multi-disciplinary collaboration in Japan.

The Postpartum Care Services (PCS) programme in Japan is intended to promote physical recovery and psychological rest for mothers and their children after discharge from the delivery facility, as well as nurture the mothers' own self-care skills and support healthy childrearing for mothers, children and their families. The subsidies for PCS are based on cooperation between psychiatry and obstetrics and between multiple professions, including the local government. The services should also be implemented based on the instruction to medical institutions and the local governments that they should

Can a community health worker administered postnatal checklist increase health-seeking behaviors and knowledge?: evidence from a randomized trial with a private maternity facility in Kiambu County, Kenya.

BACKGROUND: Since the 2009 WHO and UNICEF recommendation that women receive home-based postnatal care within the first three days after birth, a growing number of low-income countries have explored integrating postnatal home visit interventions into their maternal and newborn health strategies. This randomized trial evaluates a pilot program in which community health workers (CHWs) visit or call new mothers three days after delivery in peri-urban Kiambu County, Kenya. METHODS: Participants were individually randomized to one of three groups: 1) early postnatal care three days after delivery p

Obstetric Care for Patients With Visual Impairments: Preconception Counseling Through Postpartum Care.

Visual impairment affects millions of reproductive-aged individuals worldwide, yet obstetric care for this population remains underrepresented in clinical guidelines and medical education. Patients with visual impairment face unique barriers throughout the reproductive continuum. This review provides a comprehensive, obstetrician-gynecologist-focused framework for the care of the visually impaired. Key topics include communication, optimization, screening for coercion and violence, prenatal care, labor and delivery, postpartum care, breast/chest feeding, and contraception. Emphasis is placed o
