OBJECTIVES AND BACKGROUND: Older adults with multimorbidity and frailty frequently require palliative care, yet prescribing practices often remain focused on primary or secondary disease prevention rather than symptom relief. Polypharmacy and potentially inappropriate medications (PIMs) may undermine quality of life and alter the benefit-to-harm ratio in this population. This study evaluated the prevalence of polypharmacy, pharmacological prescribing patterns and PIMs in hospitalized older adults requiring palliative care.
STUDY DESIGN: This nested study of the Italian multicenter point-preva

Aging is often associated with an increasing number of comorbidities that warrant use of multiple drugs which increases the use of potentially inappropriate medications (PIMs), drug-drug interactions (DDIs) and drug-related problems (DRPs). The aim is to assess the prevalence of polypharmacy, PIMs, DDIs and DRPs among Faroese residents aged ≥90 years. In this population-based cross-sectional study, 494 individuals ≥90 years were invited and 298 (60%) participated. A pharmacist-led medication review was performed based on self-information, electronic patient journal and the Faroese Prescription

BACKGROUND & OBJECTIVES: Rational prescribing can prevent medication errors and the associated harm, especially in old age patients, as they are being frequently prescribed with drugs for various ailments. Moreover, polypharmacy is a common practice in them. Therefore, a significant threat of potential drug interactions and adverse effects exist. Current study focuses on assessment of Potentially Inappropriate Medication (PIM) in medication prescribed to old age patients.
METHODS: It was a forty days, descriptive and observational study conducted from August 15th 2017 to September 25th 2017 i

PURPOSE: Medication administration in long-term care facilities (LTCFs) is complicated by multimorbidity, frailty, cognitive impairment, polypharmacy, and dysphagia. Solid oral dosage form (SODF) manipulation is common but may compromise medication safety. We aimed to determine the prevalence of SODF manipulation and evaluate its appropriateness in Italian LTCFs.
METHODS: We conducted a national multicenter point-prevalence study (Prescription-Day LTCFs 2024) in 82 Italian LTCFs. Of the 3400 enrolled residents, 3174 were included after excluding residents with acute conditions and facilities

OBJECTIVES: This study evaluated the prevalence of prescribing drugs with constipation-inducing potential and the prophylactic use of laxatives in community-dwelling older adults.
METHOD: An audit of outpatient prescriptions ordered for older adults aged ≥65 years with one or more chronic morbidities in 24 governmental primary healthcare centres in Bahrain. The prescriptions were collected by pharmacist-in-charge of each health centre between December 2015 and June 2016. The proportion of prescriptions with constipation-inducing drugs and co-prescribed laxatives was determined.

BACKGROUND: Persons with dementia (PWD) have high rates of polypharmacy. While previous studies have examined specific types of problematic medication use in PWD, we sought to characterize a broad spectrum of medication misuse and overuse among community-dwelling PWD.
METHODS: We included community-dwelling adults aged ≥66 in the Health and Retirement Study from 2008 to 2018 linked to Medicare and classified as having dementia using a validated algorithm. Medication usage was ascertained over the 1-year prior to an HRS interview date. Potentially problematic medications were identified by: (1

BACKGROUND: Potentially inappropriate medication (PIM) administration in geriatric surgical patients is increasingly prevalent in the perioperative period. The relation between the variation in PIM exposure and other key postoperative geriatric outcomes, such as postoperative hospital length of stay (POLOS), however, has seldom been reported, and current researches mentioning perioperative PIM concentrate on preoperative long-term home medications or postoperative prescriptions that may not be actually taken. We aimed to investigate whether the level and the change of short-term PIM exposure i
