Posterior Reversible Encephalopathy Syndrome and Pre-eclampsia/Eclampsia: Anesthetic Implications and Management.

Posterior reversible encephalopathy syndrome (PRES) is a rare neurologic disorder that has recently become more frequently diagnosed. While the exact etiology of PRES remains unclear, multiple diseases are associated with PRES. Moreover, there is increasing recognition of the association of PRES in pre-eclampsia/eclampsia with advancements in imaging techniques and increased awareness of the disorder. While pre-eclampsia/eclampsia alone presents unique perioperative challenges, PRES further complicates anesthetic management. Unfortunately, the anesthetic management for these critically ill and

Pre-eclampsia/eclampsia at Yekatit 12 Hospital, Addis Ababa, Ethiopia (1987-1989).

In Ethiopia, pre-eclampsia/eclampsia was reported to be uncommon. A clinical analysis of 327 (4.8%) pre-eclamptic and 21 (0.3%) eclamptic patients in 6766 deliveries at Yekatit 12 Hospital, Addis Ababa was made. Out of 327 pre-eclamptics, there were 52 (15.9%) cases of mild, 142 (43.4%) cases of moderate and 133 (40.7%) cases of severe pre-eclampsia. Among patients with pre-eclampsia/eclampsia, 78.2% had no antenatal care and only 39.3% of women from the total number of deliveries were unbooked. There were significantly high frequency of low birth-weight babies of patients with severe pre-ecla

Management and Prevention of Pre-Eclampsia in Nigeria.

This research paper analyses the management and prevention of pre-eclampsia in Nigeria. Although efforts have been made to reduce outcomes due to pre-eclampsia, it still rears its head in the form of high maternal and perinatal morbidity and mortality. The aim of this review was to identify the main obstacles, gaps, and interventions related to the prevention and management of pre-eclampsia in order to be fully knowledgeable of the magnitude of the issue at the national level, to assess if current government policies are adequate and to recommend solutions. A search was performed on online dat

Incidence of pre-eclampsia: effect of deprivation.

OBJECTIVES: To examine the relationship between the English index of multiple deprivation (IMD) and the incidence of pre-eclampsia (PE), evaluate the distribution of IMD in a cohort of ethnically diverse pregnant women in South East England and assess whether IMD improves the prediction of PE compared with that provided by the 'history-only' competing-risks model (based on maternal characteristics and medical history). METHODS: This was a prospective, observational study of 159 125 women with a singleton pregnancy who attended their first routine hospital visit at 11 + 0 to 13 + 6 weeks' gest

Potential role of folate in pre-eclampsia.

Dietary deficiencies of folate and other B vitamin cofactors involved in one-carbon metabolism, together with genetic polymorphisms in key folate-methionine metabolic pathway enzymes, are associated with increases in circulating plasma homocysteine, reduction in DNA methylation patterns, and genome instability events. All of these biomarkers have also been associated with pre-eclampsia. The aim of this review was to explore the literature and identify potential knowledge gaps in relation to the role of folate at the genomic level in either the etiology or the prevention of pre-eclampsia. Assessment, surveillance and prognosis in pre-eclampsia.

The hypertensive disorders of pregnancy (HDP) remain one of the major causes of maternal mortality and morbidity worldwide. Many international guidelines exist for the classification and assessment of women with hypertension in pregnancy, but definitions and recommendations within these documents are variable. Many recommended investigations do not actually correlate with increased risk of adverse outcomes, making it difficult to determine true prognosis. Although standardised assessment and surveillance has been shown to improve outcomes, the application of these monitoring strategies in many

An immunological insight into the origins of pre-eclampsia.

BACKGROUND: Pre-eclampsia is a syndrome of heterogeneous origin characterized by deficient placentation due to the inability of the cytotrophoblast to acquire an invasive phenotype and to remodel the uterine spiral arteries. One of the main problems observed early in pre-eclampsia is an altered regulation of the immune system, where the shift toward a Th2 cytokine profile observed in normal pregnancies, does not occur. In pre-eclampsia, high interferon (IFN)-gamma concentrations are present, along with transforming growth factor-beta cytokines, which retard migration of cytotrophoblasts. METH

Predictors and risk factors of pre-eclampsia.

Understanding the mechanisms of disease responsible for the syndrome of pre-eclampsia (PE) as well as early risk assessment is still a major challenge. Risk factors for PE are nulliparity, a family or own history of PE, pre-existing diabetes or increased body mass index, multiple pregnancy, maternal age, renal disease, hypertension or raised blood pressure at booking and chronic autoimmune disease. Other factors are thrombophilias and insulin resistance together with obesity. On the other hand identification of predictors of the development of pre-eclampsia would enhance the ability to diagnos

Pre-eclampsia--still a disease of theories.

Pre-eclampsia is still one of the leading causes of maternal and fetal morbidity and mortality. Despite active research for many decades, the etiology of this disorder exclusive to human pregnancy is an enigma. Recent evidence suggests there may be several underlying causes or predispositions leading to endothelial dysfunction and causing the signs of hypertension, proteinuria, and edema--findings that allow us to make the diagnosis of the "syndrome" of pre-eclampsia. It is obvious that a single mechanism responsible for the syndrome pre-eclampsia does not exist. Instead, several mechanisms ca

Seasonal variation in the occurrence of pre-eclampsia.

OBJECTIVE: To obtain evidence for seasonal variability in pre-eclampsia using the assumption that environmental factors may have a role in the causal mechanisms. DESIGN: Cross sectional population-based study. POPULATION: All 1,869,388 deliveries in Norway in the years 1967 to 1998. METHOD: For each month, the percentage of births complicated by pre-eclampsia was calculated. The relative risks of pre-eclampsia by month of delivery were estimated as odds ratios using the month with lowest risk as the reference category. RESULTS: Mothers of children born in August had the lowest risk of pre-

Pre-eclampsia affects the immunophenotype of neonates.

OBJECTIVE: We tried to estimate whether immunological changes are present in neonates born to mothers who had been suffering from pre-eclampsia. STUDY DESIGN: Eighteen neonates born to mothers with severe pre-eclampsia (between 35 and 40 weeks of gestation) and 20 full-term healthy newborns (between 38 and 40 weeks of gestation) were included in the study. The lymphocytes were isolated from umbilical cord blood. The specific lymphocyte antigens were determined using direct staining with monoclonal antibodies and analysed by flow-cytometry. RESULTS: We observed that neonates born to pre-eclam

[The autonomic nervous system and pre-eclampsia].

Alterations in the autonomic cardiovascular control have been implicated to play an important etiologic role in preeclampsia. Earlier assessments of sympathetic nervous system activity by measurements of urinary and plasma levels of catecholamines showed contradictory results, due to serious methodological problems. The microneurographic technique enables the assessment of sympathetic outflow to the vascular bed of skeletal muscles. By the use of this technique it has been shown that preeclampsia represents a state of sympathetic overactivity as compared to healthy pregnant women. Other studie

New insights into the etiology of pre-eclampsia.

Pre-eclampsia is a pregnancy-specific syndrome of unknown etiology, defined by clinical findings of elevated blood pressure combined with proteinuria and oedema. The decidual portion of the spiral arteries of women who later develop pre-eclampsia does not undergo the normal pregnancy-induced remodelling that converts these vessels to high volume-low resistance conduits. We have postulated that this failure leads to vascular spasm, restricted blood flow, placental ischaemia and the release of toxic substances that enter the maternal circulation, resulting in multi-organ disease. The complex and

An immunopathological study on the placenta in pre-eclampsia.

An investigation was done to clarify the pathological background of pre-eclampsia. Thirty four placentae were studied immunopathologically in comparison to control cases. The results showed slightly positive stains of hCG containing alpha- and beta-subunits in syncytiocytotrophoblastic layers especially in severe pre-eclampsia cases. The placentae in mild pre-eclampsia showed only weakly and partly positive by those stains. Other control cases were negative except earlier abortion ones. No any positive stains of hCG series could be seen in two placentas from mothers with spontaneous hypertensi
