OBJECTIVE: To analyze factors associated with the discontinuation of pre-exposure prophylaxis (PrEP) for HIV among users in Brazil.
METHODS: An analytical case-control study was conducted using retrospective data (2018-2020) from PrEP users monitored by Brazil's Unified Health System (SUS) via the Medicines Logistics Control System (SICLOM). Cases (interrupted PrEP, n = 629) were matched 1:10 by sex at birth and age to controls (maintained PrEP, n = 6290). Logistic regression identified factors associated with discontinuation.
RESULTS: Factors significantly associated with PrEP discontinuati

Retention in HIV pre-exposure prophylaxis (PrEP) care and adherence to PrEP have been suboptimal in some populations, despite evidence that high adherence dramatically enhances PrEP efficacy. A comprehensive PrEP Clinic with a retention specialist and clinical pharmacist could impact patient's retention and adherence in PrEP care. A retrospective electronic medical record review of patients attending an academic PrEP Clinic was conducted between June 2018 and June 2019 (at least one visit attended for PrEP was required). Retention was defined as a medical or laboratory visit every 3 months ±30

BACKGROUND: We undertook an audit of a province-wide HIV pre-exposure prophylaxis (PrEP) program in Alberta (Canada).
METHODS: A retrospective record review of individuals accessing PrEP in Alberta included demographics, PrEP indication(s), and reported non-prescription drug and alcohol use from March 2016 to June 2019. Hepatitis A, B, C, HIV and syphilis serology, serum creatinine, and nucleic acid amplification tests testing for chlamydia and gonorrhea were collected. Descriptive statistics, incidence, and prevalence were calculated.
RESULTS: A total of 511 participants were seen at STI, s

BACKGROUND: HIV has affected gay men disproportionately in the U.S. for four decades. Pre-exposure prophylaxis (PrEP) was developed as a prevention strategy for individuals at high risk of HIV infection. Although highly effective, many gay and other men who have sex with men continue not to take PrEP. Researchers have focused on sexual risk behaviors as the primary determinant of who should be on PrEP and identified various objective systemic and societal barriers to PrEP access. Public health measures have promoted PrEP based on these objective criteria. Researchers have recently begun to inq

BACKGROUND: There is extensive evidence to show that pre-exposure prophylaxis (PrEP) using tenofovir disoproxil fumarate (TDF)-based formulations dramatically reduces the risk of HIV acquisition among individuals without HIV infection. Here, the authors aim to compare tenofovir plasma predose concentrations in subjects taking PrEP daily versus on demand and using different TDF-based generic formulations.
METHODS: Subjects providing informed signed consent for the measurement of tenofovir plasma levels were included in the study. Predose drug concentrations were stratified according to PrEP ad

OBJECTIVE: HIV Prevention Trials Network 069/AIDS Clinical Trials Group A5305 was a study of 48-week oral pre-exposure prophylaxis (PrEP) regimens in MSM and transgender women. A rectal substudy was included to evaluate drug concentrations in rectal compartment vs. blood, gut-associated lymphoid tissue (GALT) responses to four antiretroviral PrEP regimens [maraviroc (MVC), MVC + emtricitabine (FTC), MVC + tenofovir (TFV) disoproxil fumarate, and TFV disoproxil fumarate + FTC], and to determine whether ARV exposure was associated with ex-vivo suppression of HIV infection in colorectal explants.

Daily use of pre-exposure prophylaxis (PrEP) effectively prevents HIV, yet uptake remains uneven, particularly among lower socioeconomic and minoritized populations. Approximately 20% of newly approved PrEP prescriptions are reversed (a newly prescribed oral PrEP prescription that is not picked up at the pharmacy within 7-14 days), and most are subsequently abandoned (reversed prescription that is not picked up within a year). The roles of race and income in these outcomes are not well characterized. We analyzed Symphony HealthCare Analytics data on first-time, insurance-approved 30-day oral P

Despite proven oral pre-exposure prophylaxis (PrEP) efficacy, adherence barriers among men who have sex with men (MSM) persist. Economic viability of digital adherence tools in low- and middle-income countries (LMICs) requires validation. We assessed the 6-month within-trial cost-effectiveness of a digital intervention for real-time enhancement of oral PrEP adherence (DIRECT-PrEP) in China, from health system, multipayer, and societal perspectives. Costs were derived from hospital records, and effectiveness was evaluated using quality-adjusted life-years (QALYs) and PrEP-adherence days (PADs).

BACKGROUND: Pregnant and breastfeeding women are at elevated risk of HIV acquisition. The World Health Organization (WHO) recommends use of oral TDF/FTC and long-acting injectables cabotegravir (CAB-LA) and lenacapavir (LEN) for HIV pre-exposure prophylaxis (PrEP) in pregnancy. However, safety data for emerging regimens remain limited due to the exclusion of pregnant women from early clinical trials and insufficient routine, program-embedded surveillance, especially in resource-constrained settings. In March 2024, as Malawi introduced CAB-LA, the PrEP Pregnancy Registry was established to gene

BACKGROUND: Oral human immunodeficiency virus (HIV) pre-exposure prophylaxis (PrEP) effectively reduces HIV transmission. However, PrEP remains underutilized due to structural, provider, and individual-level barriers, with significant disparities in access. Emergency departments (EDs) serve high-risk populations often disconnected from primary care, offering a strategic setting for initiating PrEP services. In spite of the potential, ED-based PrEP programs face challenges, and the feasibility of initiating PrEP in this setting is unclear.

Delivering HIV testing and pre-exposure prophylaxis (PrEP) in community pharmacies can expand access to HIV services for populations with limited access to care. However, few pharmacies in the United States have successfully implemented these services. We investigated implementation barriers and facilitators of pharmacy-based HIV testing and PrEP initiation in Philadelphia, an Ending the HIV Epidemic priority jurisdiction with high rates of HIV and bacterial STIs, located in a state with relatively restrictive laws governing pharmacy scope of practice. Using a sequential, exploratory mixed-met

Daily oral pre-exposure prophylaxis (PrEP) effectively reduces HIV infection risks. However, adherence has been suboptimal across the United States (US), particularly among African Americans and Hispanic populations. We assessed racial and ethnic differences in PrEP primary non-adherence using two pharmacy-based measures: PrEP reversal (failure to pick up the first insurance-approved prescription) and PrEP abandonment (a reversed prescription not picked up within 365 days). Data from 2015 to 2019 were obtained from Symphony HealthCare Analytics, which covers approximately 75% of US PrEP claims

INTRODUCTION: HIV pre-exposure prophylaxis (PrEP) access still appears to be disproportionate, with existing barriers. This study aims to describe the characteristics, retention and feasibility of TelePrEP, Italy's first remote PrEP service, which was launched in February 2023 to address inequities in PrEP access.
METHODS: TelePrEP provides online PrEP counselling, prescriptions and follow-ups via community healthcare workers and infectious disease physicians, targeting individuals unable to attend in-person services in existing local services. Between February 2023 and April 2025, all enroll

HIV remains a major global public health challenge, and pre-exposure prophylaxis (PrEP) has established itself as a pivotal evidence-based strategy for HIV prevention. While research on PrEP has expanded rapidly, a comprehensive synthesis of the existing literature is lacking. This review systematically analyzes trends, collaborative networks, and research hotspots in PrEP through a bibliometric approach to consolidate current knowledge and inform future directions. A total of 5,273 PrEPrelated publications (1992-2025) from the Web of Science Core Collection were included, showing a 15.99% ann

BACKGROUND: Pre-exposure prophylaxis (PrEP) and postexposure prophylaxis (PEP) prevent HIV among individuals at high risk for acquisition. Pre-existing structural barriers to PrEP/PEP access among rural patients may be exacerbated further if pharmacies do not keep PrEP/PEP in stock, constituting a significant barrier to mitigating the HIV epidemic.
OBJECTIVES: To compare PrEP/PEP availability for same-day pickup in rural vs urban Georgia and Pennsylvania pharmacies.
METHODS: We conducted a cross-sectional simulated patient caller study, calling pharmacists in Georgia and Pennsylvania to see

Since the world's first case series of human immunodeficiency virus (HIV) infection were reported, more than 40 decades have passed. The advancement of HIV treatment and prevention has progressed drastically. Especially, the efficacy of pre-exposure prophylaxis (PrEP) for HIV prevention has been proven by a number of trials and the number of new HIV cases has declined over the years due to the large-scale and rapid implementation of PrEP and universal HIV treatment in multiple countries. However, in Japan, PrEP is not approved or officially supported as of June 2024. Despite of the absence of

BACKGROUND: Black men who have sex with men (MSM) are disproportionately burdened by the HIV epidemic in the USA. The effectiveness of pre-exposure prophylaxis (PrEP) in preventing HIV infection has been demonstrated through randomized placebo-controlled clinical trials in several populations. Importantly, no such trial has been conducted exclusively among Black MSM in the USA, and it would be unethical and infeasible to do so now.
METHODS: To estimate the causal effects of PrEP access, initiation, and adherence on HIV risk, we utilized causal inference methods to combine data from two non-ra

BACKGROUND: The availability of HIV pre-exposure prophylaxis (PrEP) in Southeast Asia remains limited compared to regions like Western Europe or North America. Additionally, little is known about how key populations in Southeast Asia perceive the use of PrEP as a prevention tool for HIV. This scoping review aims to provide an overview of the literature on the acceptability and feasibility of HIV PrEP programs in Southeast Asia.
METHODS: We conducted a systematic search using nine electronic databases, gray literature searches, and hand searches of references in included full-text articles. Fo

Viruses cause a large burden of human infectious diseases. During the past 50 years, antivirals have been developed to treat many pathogenic viruses, including herpesviruses, retroviruses, hepatitis viruses, and influenza. Besides being used as treatment, antivirals have shown efficacy for preventing certain viral infections. Following the success in the HIV field, a renewed interest has emerged on the use of antivirals as prophylaxis for other viruses. The development of formulations with extended half-life has pushed further this consideration in persons at risk for a wide range of viral inf
