[Chronic hepatic encephalopathy treated by long-term intravenous administration of flumazenil in ambulatory care].

Treatment of chronic hepatic encephalopathy is difficult. Oral flumazenil has been proposed for long-term treatment but is not presently available on the market. We report the successful treatment of one case of chronic hepatic encephalopathy in cirrhosis without precipitating factors by low dose intravenous injections of flumazenil (Anexate, 1 mg/4 hours). Flumazenil is expensive and not always effective. We suggest reserving this treatment to highly selected patients (without precipitating factor, or after failure of first choice treatment).

Management of delirium in Parkinson's disease.

Delirium is an acute and fluctuating disturbance of attention and awareness. Pre-existing cognitive disturbances or dementia are the most significant risk factors for developing delirium and precipitating factors such as drug treatment, infections, trauma, or surgery may trigger delirium. Patients with Parkinson's disease (PD) are at an increased risk for delirium which may be underdiagnosed due to phenomenological overlap between delirium and chronic neuropsychiatric features of PD or side effects of dopaminergic medication. Prognosis of delirium is detrimental in many cases including permane

Epidemiology and clinical features of pustular psoriasis: A 15-year retrospective cohort.

Pustular psoriasis (PuP) is a rare variant of psoriasis with a unique immunopathogenesis, unlike its more prevalent plaque-type counterpart. However, data available are limited due to its low prevalence. This study aimed to describe the demographic profile, precipitating factors, clinical presentations, and treatments among patients with different PuP subtypes from a 15-year retrospective cohort study in Thailand. A total of 60 patients were included in this study. There was female predominance (73.3%) and mean age of onset was 38.1 ± 17.6 years. Generalized PuP (GPP) was the most prevalent su

Internal jugular vein reflux in patients with transient global amnesia: a meta-analysis of case-control studies.

Our aim was to quantitatively evaluate the role of internal jugular venous reflux (IJVR) in transient global amnesia (TGA). Meta-analysis of case-control studies was conducted using Medline, Current contents connect, ISI Web of Knowledge, Biological abstracts and Scopus, plus citations of selected studies. We performed sensitivity and subgroup analyses to assess the effect of methods of IJVR determination, blinding of studies, and definition of IJVR on the results. Seven studies were included in the first meta-analysis which investigated the frequency of IJVR in 312 patients and 261 controls.

Delirium is associated with an increased morbidity and in-hospital mortality in cancer patients: Results from a prospective cohort study.

OBJECTIVE: Delirium is a frequent complication in advanced cancer patients, among whom it is frequently underdiagnosed and inadequately treated. To date, evidence on risk factors and the prognostic impact of delirium on outcomes remains sparse in this patient population. METHOD: In this prospective observational cohort study at a single tertiary-care center, 1,350 cancer patients were enrolled. Simple and multiple logistic regression models were utilized to identify associations between predisposing and precipitating factors and delirium. Cox proportional-hazards models were used to estimate

Acute cervical spondylytic myelopathy.

Myelopathy is a common consequence of cervical spondylosis. Typically, the syndrome develops gradually over a long period. Rapidly progressive deterioration in the absence of trauma or other precipitating factors is exceptional. We report a case of a rapidly progressive acute myelopathy secondary to cervical spondylosis without identifiable precipitating cause. The importance of recognizing this syndrome lies in the fact that rapid operative intervention may result in significant neurological recovery. The value of magnetic resonance imaging preoperatively and postoperatively is emphasized.

Idiopathic superior mesenteric vein thrombosis resulting in small bowel ischemia in a pregnant woman.

Background. Small bowel ischemia due to superior mesenteric vein thrombosis (MVT) is rare during pregnancy. However, additional precipitating factors should usually be identified. Case. A 31-year-old woman, pregnant at 34 weeks, was sent to the emergency department because of acute peritonitis. An emergency exploration revealed a segmental gangrene of the small intestine without any mechanical obstruction. Together with the termination of pregnancy, resection of the damaged small bowel was performed, and an end-to-end enterostomy was followed. Based on the operative and pathological findings,

Frontiers and controversies in the pathobiology of vitiligo: separating the wheat from the chaff.

The pathogenesis of vitiligo is complex and not well understood. Genes play a role in all aspects of vitiligo pathogenesis, and studies are ongoing to identify these genes and understand their biology. There is a body of interlocking, compelling evidence supporting an autoimmune basis for most or all cases of generalized vitiligo. The development of an autoimmune disease generally involves three components; the immune system, environmental triggers and other exogenous precipitating factors, and the target tissue. In vitiligo, precipitating factors could induce melanocyte damage in genetically

[Factors contributing to falls in elderly subjects leading to acute-care hospitalization].

OBJECTIVE: Falls are a frequent reason for hospitalization in short-stay geriatric units. Paradoxically, the factors determining such hospitalization remain unknown. The objective of our study was to identify the explanatory factors of falls leading to acute-care hospitalization of elderly subjects. PATIENTS AND METHODS: We prospectively compared patients hospitalized for falls and those hospitalized for another reason in a short-stay geriatric unit. Data were collected over a 6-month period starting with a questionnaire and a physical examination. RESULTS: Patients admitted for falls were p

The peri-operative management of atrial fibrillation.

Atrial fibrillation is a common arrhythmia frequently seen in surgical patients. The onset of new atrial fibrillation during the peri-operative period is less common. There are many possible precipitating factors, although volatile agents themselves may have an antifibrillatory action. The management of atrial fibrillation includes removal of any precipitating factors and treatment of the arrhythmia itself. Immediate management of acute-onset atrial fibrillation is usually direct current cardioversion. Alternatively, anti-arrhythmic drugs can be used to achieve cardioversion. In patients with

[Invagination of vermiform appendix in an asymptomatic adult: a case report].

Invagination of the vermiform appendix (appendiceal intussusception) is a very rare disease. Most cases occur in patients under 10 years of age. The clinical presentation of invagination of the vermiform appendix is extremely variable and most cases are symptomatic. The pathogenesis of invagination of appendix is still not clear, but some precipitating factors have been identified. Herein, we report one case of primary invagination of the vermiform appendix in a adult without any symptom. Etiology, precipitating factor, clinical presentation, classification, and diagnosis method are discussed.

Treatment of portal systemic encephalopathy: standard and new treatments.

The management of hepatic encephalopathy should be considered accordingly with the precipitating factor and the type of encephalopathy. Ideally the therapeutic approach must be useful for both acute and chronic forms of encephalopathy. Current treatment of hepatic encephalopathy consists of certain well-established measures attempting to identify and treat the precipitating factors, and to reduce the intestinal nitrogenous compounds formation and absorption by dietary restriction or bowel-cleansing with catartics or antibiotics such as neomycin, metronidazol, etc. This review describes briefly

Epileptic seizures precipitated by eating.

The case of a mentally retarded 18-year-old girl who for 2 years has experienced epileptic seizures exclusively during meals is discussed. Polygraphic recordings have defined the electrical and clinical features of these seizures. None of the possible precipitating factors considered in other cases in the literature (chewing, swallowing, conditioned reflex, etc.) seems to be involved in this case. We hypothesize that in our patient the precipitating factor is the complete act of eating a meal, which involves the satisfaction of a basic drive.

The role of inherited and acquired factors in the development of porphyria cutanea tarda in the Argentinean population.

BACKGROUND: Inherited and environmental factors are implicated in the expression of porphyria cutanea tarda (PCT); the contribution of each factor depends on the population. OBJECTIVE: To provide a review of PCT cases diagnosed in Argentina over 24 years and evaluate the role of different precipitating factors in its pathogenesis. Methods Plasma and urinary porphyrin levels and erythrocyte uroporphyrinogen decarboxylase (URO-D) activity were determined. Potential precipitating factors were identified in each patient. Additional tests for hepatitis C virus (HCV) and hemochromatosis gene mutati

Upper extremity vein thrombosis: etiologic categories, precipitating causes, and management.

Although an increasing incidence of upper extremity venous thrombosis (U/E-DVT) has been reported, a relative paucity of information regarding the etiologic categories, precipitating causes, and proper management for this disorder is available. To settle on a strategy for the management of U/E-DVT, retrospective analyses were performed using records from the authors' hospital. In 12 patients (seven men, five women), 61 (mean) years of age, diagnosed as having symptomatic venography-proved U/E-DVT and followed up for 41 (mean) months, etiologic factors, precipitating causes, treatments, and out

Lady With the Blue Hair: An Atypical Cause of Myasthenic Crisis.

A myasthenic crisis denotes a severe exacerbation of myasthenia gravis, leading a patient to enter a life-threatening state due to progressing muscle weakness that ultimately results in respiratory failure. A crisis can require intubation, mechanical ventilation, and additional critical care to prevent further decompensation and potentially death. Numerous well-documented precipitating factors exist, such as infections, surgery, stress, and various medications. We present the case of a 43-year-old woman recently diagnosed with myasthenia gravis who has experienced two myasthenic crises since d

The Significance of Rhabdomyolysis Secondary to Hypothyroidism.

Rhabdomyolysis is a rare but serious complication of hypothyroidism, typically associated with precipitating factors such as medication interactions, strenuous exercise, and illicit drug use. We present a unique case of rhabdomyolysis in an 89-year-old female due to severe hypothyroidism without identifiable precipitating factors. Laboratory results revealed markedly elevated creatine kinase (CK) levels and acute kidney injury (AKI). The diagnosis was confirmed by critically elevated thyroid-stimulating hormone (TSH) and low free thyroxine (FT4) levels. Prompt initiation of levothyroxine suppl

Demographic and clinical features of gout patients in Turkey: a multicenter study.

Gout results from multifactor interactions between gender, age, genetic and environmental factors. Environmental factors underlying gout and precipitating factors triggering acute attacks might vary in different populations with different lifestyles. In this study, we aimed to collect data regarding the demographic and clinical features, comorbid factors, and precipitating factors associated with the initiation of acute attacks in gout patients in Turkey. A total of 312 patients were included in this study (mean age, 58.8 ± 13.8 years; female/male ratio, 55/257). The demographic features, alco
