Pregnancy: Effect on Multiple Sclerosis, Treatment Considerations, and Breastfeeding.

Multiple sclerosis (MS) commonly affects women in childbearing years making pregnancy issues important for patients with MS and their families. Pregnancy is a naturally occurring disease modifier of MS associated with a 70% reduction in relapse rates in the third trimester. This relapse rate reduction during the last trimester is roughly equal to the most effective disease-modifying treatments for MS. Given this efficacy, various pregnancy factors have been tested to determine which play a part in pregnancy's protection, and some have been translated to completed and ongoing phase II clinical

Multiple intracranial hemorrhages in pregnancy: A common autoimmune etiology.

Systemic lupus erythematosus (SLE) is an autoimmune disorder, primarily affect female in fertile age. Pregnancy in SLE female is a high-risk situation which can adversely affect maternal-fetal dyad. SLE can flare during pregnancy or in postpartum period. We describe a case of a young pregnant female who presented because of right hemiparesis due multiple hemorrhages in the brain. The first presentation of the SLE with multiple intracranial hemorrhages in pregnancy, preceding the other characteristic clinical symptoms is rare. Here, we high lighten the major neurological issues and maternal-fet

Pregnancy and multiple sclerosis--a 3-year prospective study.

A prospective study of the level of disability, severity and distribution of relapses in 15 women with multiple sclerosis prior to, during and up to 3 years following pregnancy was undertaken. The pregnant group was compared with 22 nulliparous women attending the same clinic and matched for age and severity and duration of disease. In the pregnant group, relapses were more frequent and severe than expected values during the first 6 months post partum, but were below expected values 6-24 months post partum. Over the total study period, therefore, there were no significant differences in relaps

Birth-weight discordance and neonatal morbidity in twin pregnancy: analysis of STORK multiple pregnancy cohort.

OBJECTIVES: To investigate the relationship between weight discordance and neonatal morbidity in twin pregnancy progressing to at least 34 weeks of gestation. The secondary aim was to determine the predictive accuracy of different weight discordance cut-offs in predicting neonatal morbidity in twin pregnancy. METHODS: This was a retrospective multicenter cohort study of all twin pregnancies booked for antenatal care at four hospitals in the Southwest Thames region of London Obstetric Research Collaborative (STORK) over a period of 10 years. Ultrasound data were obtained by a search of each ho

The Knowledge About the Impact of Multiple Sclerosis on Pregnancy and Maternity Among Patients with Multiple Sclerosis.

Background/Objectives: Multiple sclerosis (MS) is most frequently diagnosed in young adults of reproductive age. Although current evidence indicates that MS itself does not usually preclude pregnancy or parenthood, patients may still have insufficient knowledge in this area. The aim of this study was to assess knowledge about the relationship of MS and pregnancy, childbirth, breastfeeding, fertility, and parenthood among women and men with MS. Methods: This single-center questionnaire-based study included 194 patients with MS: 144 women and 50 men. Participants completed a 12-item paper-and-pe

A protocol for the use of enoxaparin during pregnancy: results from 85 pregnancies including 13 multiple gestation pregnancies.

Many practitioners consider low-molecular-weight heparin (LMWH) an alternative to unfractionated heparin, although there are limited safety data regarding maternal and fetal outcomes in patients using an LMWH during pregnancy. A retrospective chart review was performed on 72 patients with thrombophilia exposed to the LMWH, enoxaparin, during pregnancy. Eighty-five pregnancies resulted in 93 of 99 potential live births. Eleven of 12 twin pregnancies and one triplet pregnancy were successful. One preterm live birth infant of 33 weeks' gestation did not survive. Three patients with thrombophilia

Why Pregnancy Weight Gain Guidelines Need to Differ for Multiple Versus Single Pregnancies.

PURPOSE OF THE REVIEW: The purpose of this review was to compare physiological and metabolic changes in singleton and twin gestations and to review pregnancy outcomes with respect to current weight gain guidelines. RECENT FINDINGS: Reviews in singleton gestations show that weight gain below the recommended guidelines has been associated with small for gestational age infants (less than the 10th percentile) which place these infants at increased risk for neonatal morbidities. Observational studies have shown that adequate and excessive gestational weight gain in twin gestations has been associ

Multiple pregnancy after in-vitro fertilization and embryo transfer: report of a quadruplet pregnancy and delivery.

The first baby from in-vitro fertilization (IVF) was born in England in 1978 as a result of retrieval of a single preovulatory oocyte in the course of a natural cycle (Steptoe and Edwards, 1978). At present most programmes of IVF throughout the world do not use natural cycles producing only one oocyte, but rather multiple oocyte cycles produced by clomiphene citrate (CC), human menopausal gonadotrophin (HMG), or pure follicle stimulating hormone (FSH), either separately or in combination, sequentially or concomitantly, for the induction of multiple follicular maturation.

An Updated Assessment of Patient Knowledge in Pregnancy and Multiple Sclerosis.

UNLABELLED: Abstract. BACKGROUND: Multiple sclerosis (MS) is more prevalent in women and is frequently diagnosed during their childbearing years. Prior inquiry into patient knowledge regarding family planning has revealed that patients feel inadequately informed about topics surrounding pregnancy. METHODS: We designed and implemented a 13-question survey to assess patient comprehension surrounding pregnancy and treatment options at the Duke Comprehensive MS Center. RESULTS: Forty-four surveys were returned, with responses from 1 survey not included in the analysis due to most of the questio

Pregnancy and reproductive health in women with multiple sclerosis: an update.

PURPOSE OF REVIEW: Multiple sclerosis (MS) is a chronic immune-mediated, inflammatory, neuro-degenerative disease of the central nervous system, prevalent in women of reproductive age. Today, many women want to start a family after MS diagnosis. There are over 20 treatments for MS, and safely navigating family planning is important. We review updated information on family planning, preconception, and peri-partum considerations, and reproductive concerns in special populations with MS. RECENT FINDINGS: There are no MS-related restrictions on any available and appropriate contraceptive method i

Maternal thyroid function in multiple pregnancies - a systematic review.

BACKGROUND: The function of the thyroid gland during pregnancy undergoes physiological changes to ensure the proper amount of thyroid hormones for both the pregnant woman and the fetus. Multiple pregnancies (MP) are characterized by specific differences compared to single pregnancies, e.g., higher concentrations of human chorionic gonadotropin, which also affect thyroid function. The aim was to collect available knowledge on maternal thyroid function in MP. METHODS: We have systematically searched three databases: the PubMed/MEDLINE, Scopus and the Cochrane Library. The last search was run on

The Influence of Multiple Pregnancies on Gait Asymmetry: A Case Study.

Gait asymmetry is a predictor of fall risk and may contribute to increased falls during pregnancy. Previous work indicates that pregnant women experience asymmetric joint laxity and pelvic tilt during standing and asymmetric joint moments and angles during walking. How these changes translate to other measures of gait asymmetry remains unclear. Thus, the purpose of this case study was to determine the relationships between pregnancy progression, subsequent pregnancies, and gait asymmetry. Walking data were collected from an individual during 2 consecutive pregnancies during the second and thir

Outcomes of higher-order multiple pregnancies in an Australian setting.

BACKGROUND: Higher-order multiple (HOM) pregnancies are associated with significant maternal and neonatal morbidity, especially consequent to preterm birth. Multi-fetal pregnancy reduction (MFPR) may be provided, though its benefits in prolonging gestation and improving neonatal outcomes must be weighed against its risks. AIMS: The aim was to compare outcomes of HOM pregnancies where expectant management was chosen (EM) with those where MFPR was provided. METHODS: The method involved a retrospective study of HOM pregnancies referred to a single quaternary hospital between 2007 and 2016. The
