Protocols for uncontrolled donation after circulatory death: a systematic review of international guidelines, practices and transplant outcomes.

INTRODUCTION: A chronic shortage of organs remains the main factor limiting organ transplantation. Many countries have explored the option of uncontrolled donation after circulatory death (uDCD) in order to expand the donor pool. Little is known regarding the variability of practices and outcomes between existing protocols. This systematic review addresses this knowledge gap informing policy makers, researchers, and clinicians for future protocol implementation. METHODS: We searched MEDLINE, EMBASE, and Google Scholar electronic databases from 2005 to March 2015 as well as the reference lists

[Socioeconomic impact of neurodevelopmental disorders in France].

INTRODUCTION: This study aims to assess the economic impact of delays and inadequate diagnosis and treatment for neurodevelopmental disorders (NDDs) in France, affecting 2 to 3 million individuals. Neurodevelopmental disorders, including autism spectrum disorder (ASD), learning disabilities, and attention deficit hyperactivity disorder (ADHD), are often described as "invisible" disabilities, but they generate significant societal costs. METHOD: The study was conducted with the support of Ashoka France as part of a collaboration between Goodwill-Management and the Aloïs Association. Goodwill-M

Beyond the Technology: Nursing Critically Ill Children on Extracorporeal Organ Support-A Hermeneutic Review of the Literature.

BACKGROUND: Using advanced forms of medical technology such as extracorporeal organ support to take over organ function in the face of critical illness is a manifestation of the technological expertise to support and prolong life. However, clinician focus on the technical aspects of extracorporeal organ support both in research and clinical practice has the tendency to relegate to the background and disregard multiple, non-technical components such as the meanings, interactions, and experiences of nurses, families and other healthcare professionals. AIM: To understand how experiences and soci

Interpreting the Health Care Financing Administration's mortality statistics.

In this article, the methodology used by the Health Care Financing Administration in the 1992 release of 1990 mortality statistics is described, and the performance of one outlier hospital is evaluated as a case study. The study hospital is compared to all other hospitals, and to a smaller cohort of 200-to-299-bed minor teaching hospitals, in terms of predicted and observed mortality rates and mortality model determinants. Proportionately more patients treated in the study hospital were women and had cerebrovascular degeneration or chronic renal disease; fewer patients had cardiovascular disea

Guidance for Documentation of Therapeutic Apheresis Interventions in the Medical Record: An American Society for Apheresis (ASFA) Practice Perspective.

In 2007, the American Society for Apheresis (ASFA) published guidance for physician documentation related to oversight of therapeutic apheresis (TA). Due to 21st century changes in healthcare delivery, the ASFA Board of Directors (BOD) charged its Public Affairs and Advocacy Committee (PAAC) to coordinate a review and update of the guidance. PAAC members reviewed minutes from various ASFA committees' earlier deliberations and sought additional information from the medical and general literature and regulatory/professional organizations involved with medical records documentation as well as fro

An Item-Level Analysis of Patient Health Questionnaire (PHQ-9) Among Adolescents in Primary Care.

OBJECTIVE: Adolescents are at increased risk for meeting diagnostic criteria for depression and chronic impairments in functioning. Despite the potential consequences, adolescents have poor access to mental health treatment. To increase access, professional organizations recommend pediatricians regularly screen for symptoms of depression with the goal of identifying, preventing, and treating depression. The Patient Health Questionnaire (PHQ-9) has become a frequently used screener in primary care due to its brevity and well-established psychometric properties among both adults and adolescents.

Outcomes of chemotherapy-induced nausea and vomiting guideline adherence in pediatric and adult patients: a systematic review.

PURPOSE: This study describes chemotherapy-induced nausea and vomiting (CINV) control rates in pediatric and adult patients who did or did not receive guideline-consistent CINV prophylaxis. METHODS: We conducted a systematic literature review of studies published in 2000 or later that evaluated CINV control in patients receiving guideline-consistent vs. guideline-inconsistent CINV prophylaxis and reported at least one CINV-related patient outcome. Studies were excluded if the guideline evaluated was not publicly available or not developed by a professional organization. Over-prophylaxis was d

Pushing beyond the silos: the obstetrician's role in perinatal depression care.

Perinatal depression is one of the most common medical complications during and after pregnancy, occurring in up to 20% of new mothers in the USA. It can have both short- and long-term effects on the mother, child, and family. Multiple etiologies have been proposed for perinatal depression, and there are many reported genetic, epigenetic, environmental, socioeconomic, and psychosocial risk factors. Perinatal depression can be treatable but remains underdiagnosed and undertreated for a variety of reasons, including inconsistent clinical definitions, inconsistent screening, social stigma, and lo

Evaluating inpatient glycemic management: the Quality Hyperglycemia Score.

BACKGROUND: Inpatient hyperglycemia has become a major focus at many hospitals. However, although several professional organizations have pushed for improved inpatient glucose management, glycemic control at many institutions remains suboptimal. There is a general consensus that improved quality of care is needed, but objective assessment of care quality remains a challenge. Lack of clear, effective performance feedback to clinicians is one element that may derail efforts to improve practice. METHODS: We developed a simplified grading system, the Quality Hyperglycemia Score (QHS), to allow cl
