Risk stratification for CNS infection: A potential tool to avoid unwarranted lumbar punctures - An observational study.

OBJECTIVE: Central nervous system (CNS) infection poses a diagnostic challenge especially in elderly patients who frequently exhibit atypical symptoms. Our study aimed to identify patients with a low risk of CNS infection, in whom lumbar puncture (LP) could be avoided. METHODS: Observational study of consecutive adult patients who underwent a LP in the emergency room (ER) of Hospital Universitari Vall d'Hebron between January 2017 and December 2021. We performed a univariate and multivariate analysis to identify factors associated with non-CNS infection. These factors were used to create a co

Clinical Impact and Safety of Non-Target Punctures (NTP) during Portal Vein Access in TIPS Procedure.

BACKGROUND: Although non-target puncture (NPT)-related complications are well known to clinicians performing TIPS, there is no NTP-focused study to assess the true clinical sequalae of NTP-related complications. In this study, the aim was to evaluate the incidence, safety, clinical outcomes and complications related to NTPs during the portal access of TIPS procedures. METHODS: A retrospective review of 369 TIPS procedures from October 2007 to September 2019 was performed. We identified inadvertent NTPs, including biliary, hepatic artery, lymphatic and capsular punctures. Next, the medical rec

Octreotide: The IIH therapy beyond weight loss, carbonic anhydrase inhibitors, lumbar punctures and surgical/interventional treatments.

OBJECTIVE: Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure without a space-occupying cerebral lesion, venous sinus thrombosis or hydrocephalus and with normally composited cerebrospinal fluid (CSF). Main symptoms are headache, sight disturbances and potential visual impairment. Weight loss, carbonic anhydrase inhibitors, lumbar punctures with CSF drain, CSF shunting, optic nerve sheath fenestration, and venous sinus stenting are common IIH therapies. Octreotide, a synthetic somatostatin analogue, also effectively suspends IIH symptoms. Here, treatm

Association of transseptal punctures with isolated migraine aura in patients undergoing catheter ablation of cardiac arrhythmias.

BACKGROUND: Transseptal catheterization (TSC) is performed during catheter ablation involving the lefthand side of the heart. TSC causes a transient iatrogenic atrial septal defect that can predispose patients to migraine episodes. However, isolated migraine aura episodes in patients undergoing TSC have not been described. METHODS: Five hundred seventy-one procedures involving TSC were performed over a 3-year duration. Of these, 3 patients presented with visual symptoms in the first month after the procedure. One patient underwent a TSC during catheter ablation of left-sided accessory pathway

Display-based augmented reality navigation for CT-Guided thoracoabdominal punctures: phantom study of accuracy and workflow efficiency.

Accurate needle placement is critical in percutaneous thoracoabdominal interventions. Conventional CT-guided procedures are limited by iterative scans, operator-dependent variability, and two-dimensional visualization. This preclinical study evaluates a novel display-based augmented reality (AR) navigation system integrating virtual-to-real registration via deltille grid position sensing (DGPS) markers for enhanced needle guidance. Experiments were conducted on a CIRS abdominal phantom with three operators: one expert interventional radiologist and two novices. Puncture accuracy, angular devia

Reducing General Anesthesia for Lumbar Punctures in Pediatric Patients With Leukemia and Lymphoma, a Quality Improvement Project.

PURPOSE: Curative therapy for most children with leukemia and lymphoma includes multiple lumbar punctures (LPs) for the administration of intrathecal chemotherapy, commonly facilitated with deep sedation or general anesthesia (GA). There is ongoing concern about the potential for neurocognitive deficits induced by GA in the developing brain. The objective of this quality improvement (QI) project was to implement and evaluate a process whereby children received the level of sedation they need for their LPs and not GA by default. METHODS: We included children at our center aged 5-18 years who r

Evolution of Radiofrequency Transseptal Puncture Devices.

Transseptal puncture (TSP) is an indispensable step for establishing left atrial access and plays a pivotal role in complex left-heart interventions, including catheter ablation for atrial fibrillation, left atrial appendage occlusion, and transcatheter mitral valve therapy. As such, TSP ranks among the core techniques in contemporary cardiovascular interventional cardiology. Since its first description in the 1950s, TSP tools have undergone decades of iterative refinement, evolving from conventional mechanical needles to diverse energy-assisted puncture systems. The performance of a TSP syste

Postdural puncture headache: Beyond the evidence.

Despite advances in procedural techniques and equipment, postdural puncture headache (PDPH) remains a serious complication of labour epidural analgesia after accidental dural puncture (ADP). Often considered a temporary inconvenience, PDPH can be debilitating in the short term. It can also be associated with chronic manifestations and serious complications. The precise underlying mechanisms of PDPH are still incompletely understood, and longstanding beliefs of dysregulation of cerebrospinal fluid (CSF) homeostasis due to CSF fluid loss are currently being challenged. The existence of orthostat

Anatomical morphometry for Cricothyrotomy puncture and incision.

PURPOSE: Emergency surgical airway securing techniques include cricothyrotomy, puncture, and incision. While the instruments used for these methods vary in size, no index of laryngeal morphology exists to guide instrument selection. Therefore, we measured the morphology of the cricothyroid ligament in Japanese individuals and assessed its correlations with height. METHODS: This retrospective study used 61 anatomical practice specimens. The cricothyroid ligament of the laryngeal area was dissected, and a frontal image was recorded. Next, images of the midsagittal sections of the larynx and tra

Positioning for lumbar puncture in newborn infants.

BACKGROUND: Lumbar puncture is a common invasive procedure performed in newborns for diagnostic and therapeutic purposes. Approximately one in two lumbar punctures fail, resulting in both short- and long-term negative consequences for the clinical management of patients. The most common positions used to perform lumbar puncture are the lateral decubitus and sitting position, and each can impact the success rate and safety of the procedure. However, it is uncertain which position best improves patient outcomes. OBJECTIVES: To assess the benefits and harms of the lateral decubitus, sitting, and

Transseptal Puncture for Catheter Ablation in Children.

Transseptal puncture (TP) is used in pediatric patients to access the left atrium in left-sided arrhythmia catheter ablation. Performing this procedure can be difficult and risky, especially in small children. In this study, we aimed to evaluate the safety and feasibility of TPs in children ≤ 30 kg. Between April 2012 and April 2018, a retrospective evaluation was conducted of the clinical features, procedural outcomes, and follow-ups of ≤ 30 kg pediatric patients who required TPs for left-sided ablations at a pediatric electrophysiology center in which a three-dimensional mapping system was r
