Nigrosome 1 imaging in REM sleep behavior disorder and its association with dopaminergic decline.

OBJECTIVES: Rapid eye movement sleep behavior disorder (RBD) patients have a high risk of developing a Parkinsonian disorder, offering an opportunity for neuroprotective intervention. Predicting near-term conversion, however, remains a challenge. Dopamine transporter imaging, while informative, is expensive and not widely available. Here, we investigate the utility of susceptibility-weighted MRI (SWI) to detect abnormalities of the substantia nigra in RBD, and explore their association with striatal dopaminergic deficits. METHODS: SWI of the substantia nigra was performed in 46 RBD patients,

Follow-up study of cardiac ¹²³I-MIBG scintigraphy in idiopathic REM sleep behavior disorder.

BACKGROUND: Cardiac (123) I-metaiodobenzylguanidine ((123) I-MIBG) uptake in patients with idiopathic rapid eye movement (REM) sleep behavior disorder (iRBD) is markedly reduced, as in Parkinson's disease (PD). METHODS: We performed (123) I-MIBG scintigrams on patients with iRBD and PD. After the initial (123) I-MIBG scintigram, we retested subjects after a mean of 2.8 years. RESULTS: The delayed heart-to-mediastinum (H/M) ratio of cardiac (123) I-MIBG uptake was not significantly reduced between the first and second study in either group (P = 0.050, P = 0.091, respectively). The implication of nigrostriatal dopaminergic degeneration in the pathogenesis of REM sleep behavior disorder.

BACKGROUND AND PURPOSE: The pathogenesis of rapid eye movement (REM) sleep behavior disorder (RBD) is not clear despite its frequent association with Parkinson's disease (PD). We investigated whether the nigrostriatal dopaminergic system is involved in the development of idiopathic RBD. METHODS: Fourteen patients with RBD, 14 patients with PD and 12 normal controls were included in the study. The diagnosis of RBD was confirmed on polysomnography. All the participants performed single-photon emission computed tomography imaging 3 h after injection of [(123)I]FP-CIT. During REM sleep of the RBD

Visual hallucinations as REM sleep behavior disorders in patients with Parkinson's disease.

To clarify whether visual hallucinations in patients with Parkinson's disease (PD) are related to rapid eye movement (REM) sleep, nocturnal polysomnographic variables were compared between a group with hallucinations (hallucinators, n = 14) and a group without hallucinations (nonhallucinators, n = 8). A multiple sleep latency test (MSLT) was performed on 3 hallucinators, and the content of dreams during daytime REM sleep was investigated. The efficacy of clonazepam, a standard treatment choice for REM sleep behavior disorders, was investigated in 8 hallucinators. Nocturnal polysomnograms of th

REM sleep behavior disorder in sleep-disordered patients with versus without Parkinson's disease: is there a need for polysomnography?

We reviewed the polysomnograms (PSGs) of 292 consecutive patients with sleep disorders (Parkinson's disease (PD), n=19, other sleep disorders, n=273) to investigate the sensitivity and specificity of the clinical diagnosis of rapid eye movement behavior disorder (RBD) compared with polysomnographic diagnosis. Patients with dementia, multiple system atrophy, or any other neurodegenerative disease were excluded. RBD was diagnosed clinically if the minimal criteria, according to the guidelines given in the International Sleep Disorders Classification, were fulfilled. The following PSG criteria we

123I-MIBG cardiac scintigraphy provides clues to the underlying neurodegenerative disorder in idiopathic REM sleep behavior disorder.

OBJECTIVE: RBD is considered to be a manifestation of an evolving synucleinopathy, such as Parkinson disease (PD), dementia of Lewy bodies (DLB), and multiple system atrophy (MSA). We tested whether the degree of accumulation of cardiac 123I-MIBG scintigraphy can distinguish the clinical syndromes associated with Lewy body-related disease from the syndrome of PSP (a tauopathy) and MSA. DESIGN: Cross-sectional. SETTING: University-based sleep disorders laboratory. PATIENTS: Subjects comprised 95 patients (31, idiopathic RBD; 26, PD; 10, MSA; 6, DLB; 13, progressive supranuclear palsy [PSP])

Melatonin for treatment of REM sleep behavior disorder in neurologic disorders: results in 14 patients.

OBJECTIVE: To describe the treatment response with melatonin for rapid eye movement (REM) sleep behavior disorder (RBD) associated with other neurologic disorders. BACKGROUND: Clonazepam has been considered the treatment of choice for RBD. However, an alternative treatment is desirable for those with RBD refractory to clonazepam, for those who experience intolerable side-effects with clonazepam, and for those in whom clonazepam precipitates or aggravates obstructive sleep apnea (OSA). To date, there is minimal published data and limited follow-up regarding the use of melatonin for patients wi

Validation of the Mayo Sleep Questionnaire to screen for REM sleep behavior disorder in a community-based sample.

OBJECTIVE: To validate a questionnaire focused on REM sleep behavior disorder (RBD) in a community-based sample. BACKGROUND: RBD is a parasomnia manifested by recurrent dream enactment behavior during REM sleep. While confirmation of RBD requires the presence of REM sleep without atonia on polysomnography (PSG), a screening measure for RBD validated in older adults would be desirable for clinical and research purposes. METHODS: We had previously developed the Mayo Sleep Questionnaire (MSQ) to screen for the presence of RBD and other sleep disorders. We assessed the validity of the MSQ by com

Exercise Intervention for REM Sleep Behavior Disorder in Parkinson's Disease: Mechanisms and Implications for Neurorehabilitation.

BACKGROUNDS: REM sleep behavior disorder (RBD) is a prodromal non-motor symptom of Parkinson's disease (PD) and other α-synucleinopathies, affecting approximately 30%-60% of PD patients. It is closely associated with α-synuclein (α-syn) aggregation, with its primary pathophysiology involving the impairment of brainstem circuits, resulting in movement disorders and circadian rhythm dysfunction. Although exercise is recognized for improving motor symptoms and delaying neurodegenerative disease progression, a comprehensive framework of its underlying mechanisms for RBD symptoms has not yet been t

Characterization of patients with isolated REM sleep behavior disorder and cerebrospinal fluid α-synuclein negativity.

Isolated REM sleep behavior disorder (IRBD) represents the prodromal phase of synucleinopathies. We investigated the characteristics of IRBD patients who tested negative for misfolded α-synuclein (αsyn) in CSF. CSF αsyn was analyzed by seed amplification assay (SAA) in 191 consecutive patients. Using logistic regression, demographic and clinical variables were compared between subjects with negative (αsyn-) and positive (αsyn+) αsyn. αsyn- occurred in 47 (24.61%) patients who were younger (p = 0.018), had less electromyographic REM sleep activity (p < 0.001), and more frequent normal DAT-SPECT

Evolution of Motor and Nonmotor Characteristics in an Idiopathic/Isolated REM Sleep Behavior Disorder Cohort.

BACKGROUND AND OBJECTIVES: Years before diagnosis of Parkinson disease (PD), dementia with Lewy bodies (DLB), or multiple system atrophy (MSA), mild prodromal manifestations can be detected. Longitudinal follow-up of people with prodromal synucleinopathy, particularly idiopathic/isolated REM sleep behavior disorder (iRBD), enables in-depth clinical phenotyping of early disease, which could facilitate stratification for clinical trials, provide the definition of appropriate end points, or predict phenoconversion more precisely. The aim of this study was to update and expand on previous studies

Enrichment of gut-derived metabolites in a Parkinson's disease subtype with REM sleep behavior disorder.

Recent reports indicate that Parkinson's disease (PD)-like changes in gut microbial signatures develop in idiopathic REM sleep behavior disorder (iRBD), a prodrome of α-synucleinopathies. However, <50% of PD cases exhibit RBD at onset, underscoring PD's heterogeneity. Using untargeted metabolomics, plasma samples from PD patients with and without RBD, iRBD subjects, and healthy controls were analyzed to characterize the metabolic differences in PD patients with and without RBD. Metabolomic analysis revealed the enrichment of gut microbial-origin metabolites, such as secondary bile acids and p-
