National trends in oncology specialists' EHR inbox work, 2019-2022.

BACKGROUND: Electronic health record (EHR) burden is an important driver of the ongoing physician burnout crisis. In particular, EHR-based messaging (also known as "inbox")-including messages from patients-is associated with burnout and decreased well-being. Little is known about EHR messaging burden for oncologists. To address this gap, we assessed trends in oncologist EHR messaging volume and EHR time from 2019 to 2022 across oncology subspecialties. METHODS: This study used EHR metadata for all US oncology physicians (including medical oncologist/hematologists, radiation oncologists, pedia

Noninvasive breast cancer.

Noninvasive breast cancer comprises two distinct entities, lobular carcinoma in situ (LCIS) and ductal carcinoma in situ (DCIS). The increased use of screening mammography has caused more cases of noninvasive breast cancer to be discovered. However, the increased detection of LCIS and DCIS has posed problems in patient care for the radiologist, pathologist, surgeon, radiation oncologist, and medical oncologist. The authors review the past history of LCIS and DCIS, as well as the diagnostic challenges and therapeutic considerations for the multidisciplinary breast cancer team.

Advances and Challenges in Conducting Clinical Trials With Proton Beam Therapy.

Advances in proton therapy have garnered much attention and speculation in recent years as the indications for proton therapy have grown beyond pediatric, prostate, spine, and ocular tumors. To achieve and maintain consistent access to this cancer treatment and to ensure the future viability and availability of proton centers in the United States, a call for evidence has been heard and answered by proton radiation oncologists. Answers provided in this review include the evolution of proton therapy research, rationale for proton clinical trial design, challenges in and barriers to the conduct o

A 65-Year-Old Man with Recent History of Radiation Therapy to the Head and Neck Reporting for Treatment of a Non-restorable Tooth.

Patients with a history of head and neck radiation involving or adjacent to tooth-bearing areas are at increased risk of developing osteonecrosis following dental procedures. The dental provider should thus aim to preserve the patient's dentition after radiation therapy. Root canal therapy with coronectomy may be an option for a nonrestorable tooth, whereas atraumatic extraction can be considered if retaining the tooth is impossible. When treating a patient with a history of head and neck radiation, it is recommended that the dental provider reviews the patient's radiation records and consults

Piling Gold in the Prostate.

Radiation beams precisely directed at a tumor can attenuate the radiation to contiguous tissues. Image-guided radiation therapy using fiducials allows accurate delineation of tumor location. Traditionally, fiducials in the prostate have been placed by urologists or radiation oncologists. With the evolution of endoscopic ultrasound (EUS), fiducials have been successfully placed under the EUS guidance in different organs. In this case series, fiducials were placed in 3 patients with prostate cancer. All patients completed their radiation therapy, and no complications were reported except mild dy

Chemotherapy use in triplet approach for metastatic hormone-sensitive prostate cancer: French practice survey and expert opinion.

INTRODUCTION: Triplet therapy, combining androgen deprivation therapy (ADT) with docetaxel and either darolutamide or abiraterone/prednisone, has significantly improved outcomes in metastatic hormone-sensitive prostate cancer (mHSPC), especially in high-volume disease. However, the absence of standardized eligibility criteria for chemotherapy in current guidelines brings a challenge, potentially leading to underuse in appropriate patients. This study aimed to assess real-world clinical practices in France, evaluating the criteria guiding triplet therapy and leading to docetaxel chemotherapy ex

Radiotherapy in cutaneous lymphomas: Recommendations from the EORTC cutaneous lymphoma tumour group.

The number of primary cutaneous lymphoma patients receiving low-dose radiotherapy is increasing, though controlled clinical trials defining the standard radiation dose for each specific entity have not yet been completed. Radiation oncologists are left with making highly individualized decisions that would be better enriched by additional clinical evidence. In this expert opinion, we aim to provide a clear recommendation to improve the current practice of radiation oncology. In addition, existing literature has been reviewed to develop recommendations for all types of primary cutaneous lymphom

Radiation treatment for the right naris in a pediatric anesthesia patient using an adaptive oral airway technique.

Radiation therapy for pediatric patients often includes the use of intravenous anesthesia with supplemental oxygen delivered via the nasal cannula. Here, we describe the use of an adaptive anesthesia technique for electron irradiation of the right naris in a preschool-aged patient treated under anesthesia. The need for an intranasal bolus plug precluded the use of standard oxygen supplementation. This novel technique required the multidisciplinary expertise of anesthesiologists, radiation therapists, medical dosimetrists, medical physicists, and radiation oncologists to ensure a safe and repro

Sustainability in German radiotherapy: professionals' perspectives on energy savings-results of a DEGRO working group survey.

PURPOSE: The healthcare sector is a major source of carbon dioxide (CO2) emissions, with radiotherapy (RT) being a particularly significant contributor due to its high energy consumption and patient-related travelling. Our survey aimed to gather opinions on energy savings and CO2 reduction in various RT areas. We asked radiation oncologists and medical physicians about the importance of these issues in their day-to-day work as well as about their personal views on them. METHODS: An online survey was conducted via the German Society for Radiooncology (DEGRO) mailing list. The questionnaire cov

A case of cetuximab-induced radiation recall skin dermatitis and review of the literature.

Radiation recall presents as an acute inflammatory reaction triggered by systemic therapy, usually chemotherapy, and is typically limited to an area that was previously irradiated. Radiation recall reactions are generally self-limiting and most commonly occur in the skin. Many systemic agents have been described to elicit a radiation recall reaction, but the exact pathogenesis is largely unknown. Here, we describe the first reported case of radiation recall dermatitis following cetuximab. While cetuximab is associated with other skin reactions, oncologists should not exclude radiation recall d

Factors Impacting Provider Treatment Decision-Making in Early Glottic Cancer.

OBJECTIVES: Transoral laser microsurgery (TLMS) and radiotherapy (XRT) are mainstays of treatment for early glottic carcinoma (EGC). Here, we investigated case-dependent provider treatment preferences and identify factors which impact decision-making in EGC. METHODS: This cross-sectional survey of laryngologists, head-and-neck surgeons, and radiation oncologists presented five diagrammatic cases of progressively advanced EGC (T1/2, N0). Respondents indicated preference for TLMS or XRT and ranked factors which influenced their recommendation for each case. Analysis utilized descriptive statist

High local control and low ocular toxicity using ultra-low-dose "boom-boom" radiotherapy for indolent orbital lymphoma.

BACKGROUND: The first line definitive treatment for early-stage indolent B-cell lymphoma is radiation therapy (RT). Due to the sensitivity of orbital structures to radiation, ultra-low-dose RT (4 Gy in 2 fractions, "boom-boom") has and been utilized as an attractive option for orbital lymphoma. In this retrospective study, we evaluated the outcome and toxicity of "boom-boom" RT for indolent orbital lymphoma with an emphasis on ophthalmologic toxicity. METHODS: This is a retrospective case series with 17 patients with orbital lymphoma who received boom-boom RT at a single tertiary referral cen
